3차원 CT 영상을 이용한 안면비대칭을 동반한 골격성 III급 부정교합 환자의 악교정 수술 시 하안면 연조직 변화

dc.description.abstract골격성 III급 부졍교합 환자의 악교정 수술 계획 시 수술 후 안모 변화에 대한 예측은 필수적이다. 하지만 악교정 수술은 경조직을 움직이며, 연조직은 경조직을 따라 이동하기 때문에 경조직 변화에 따른 연조직 변화를 예측하는 것이 중요하다. 특히 안면비대칭을 동반한 환자의 악교정 수술 계획 수립 시 이상적인 수술 결과를 위해서는 편위측과 비편위측에서의 술 후 연조직 변화에 대한 고려는 반드시 이루어져야 한다. 본 연구는 수술 전과 수술 후 경조직 변화에 따른 연조직 변화를 분석하고, 안면 비대칭을 동반한 환자와 그렇지 않은 환자 간의 비교와 함께, 안면비대칭 환자 내에서 편위측과 비편위측의 수술 후 연조직 변화에 어떤 차이가 있는지 분석하였다. 연구 대상은 골격성 III급 부정교합으로 진단되어 악교정 수술을 받은 환자 중 안모 정중선에 대해 2mm 이하의 이부 편위를 보이는 비대칭을 동반하지 않은 환자 10명 (남자 5명, 여자 5명, 평균 연령 21.1세)을 대칭군으로 하고 정중선에 대해 4mm 이상의 이부 편위를 보이는 비대칭을 동반한 환자 7명 (남자 2명, 여자 5명, 평균 연령 22.3세)을 비대칭군으로 하여, 총 17명을 대상으로 수술 4주 전, 수술 1년 후 시점에서 CT 영상을 촬영하고, 경조직과 연조직 계측점들의 수술 전과 수술 후 이동량의 상관관계와 비율을 조사하여 다음과 같은 결과를 얻었다. 1. 정중 시상면에 존재하는 계측점인 Id-Li, B-B’ 에서 대칭군과 비대칭군 모두 수평적, 전후방적인 이동에 대해 통계적으로 유의미한 상관관계를 보였다. Pog-Pog’의 경우 대칭군은 전후방적, 수직적인 이동에 대해 통계적으로 유의미한 상관관계를 보였으며 비대칭군은 수평적, 수직적인 이동에 대해 통계적으로 유의미한 상관관계를 보였다. Me-Me’ 에서는 대칭군과 비대칭군 모두 수평적, 전후방적, 수직적 이동 모두에서 통계적으로 유의미한 상관관계를 보였다. (P < 0.05) 2. 하순의 양측성 계측점인 L2 dev. – LL dev.와 L2 non. – LL non.에 대해 대칭군은 수평적, 전후방적, 수직적인 이동 모두에 대해 통계적으로 유의미한 상관관계를 보인 반면, 비대칭군은 수평이동에 대해서만 통계적으로 유의미한 상관관계를 보였다. (P < 0.05) 3. 이부의 양측성 계측점인 B dev. – B’ dev.에서 대칭군은 수평적, 전후방적인 이동에 대해 통계적으로 유의미한 상관관계를 보였으며, 비대칭군은 수평적, 수직적인 이동에 대해 통계적으로 유의미한 상관관계를 보였다. B non. – B’ non.에 대해서는 대칭군은 수평적, 수직적인 이동에 대해 통계적으로 유의미한 상관관계를 보인 반면, 비대칭군은 수평적, 전후방, 수직적인 이동에 대해 통계적으로 유의미한 상관관계를 보였다. (P < 0.05) 4. 비대칭군 내에서 양측성 계측점의 경조직과 연조직 수평이동 비율을 비교했을 때, 편위측 계측점인 L2 dev. – LL dev.의 이동비율은 1.06을 보였으며 B dev. – B’ dev.의 이동비율은 0.88의 이동비율을 보였다. 이에 반해 비편위측 계측점인 L2 non. – LL non.의 이동비율은 0.72을 보였으며, B non. – B’ non.의 이동비율은 0.78의 이동비율을 보여 편위측에 비해 비편위측에서 경조직과 연조직의 이동비율이 낮았다. 안면 비대칭을 동반한 골격성 III급 부정교합 환자의 악교정 수술 계획 시 본 연구의 결과인 경조직과 연조직의 이동비율과 편위측, 비편위측의 차이를 고려한다면, 수술 후 안모 예측을 위한 3차원적인 진단에 도움이 될 수 있을 것이다. The prediction of soft tissue changes after orthognathic surgery is essential when planning orthognathic surgery in patients with skeletal Class III. However, orthognathic surgery moves hard tissue and soft tissue moves along hard tissue. Especially, in patients with facial asymmetry, post-operative soft tissue changes in deviation and non-deviation side must be considered for ideal results. The aim of this study is to analyze soft tissue changes according to hard tissue changes before and after surgery, and to compare between patients with and without facial asymmetry. In addition, this study analyzes the difference between deviation side and nondeviation side in facial asymmetry patients. Total 17 patients were selected who were diagnosed as Skeletal Class III malocclusion and underwent orthognathic surgery. 10 patients (5 males, 5 females, average age 21.1 years old) without asymmetry showing a menton deviation less than 2mm were divided as symmetry group, and 7 patients (2 males, 7 females, average age 22.3 years old) with asymmetry showing a menton deviation more than 4 mm were divided as asymmetry group. CT images were taken 4 weeks before surgery and 1 years after surgery. The movements of hard tissue and soft tissue landmarks before and after surgery were analyzed. The results were as follows. 1. In the relationship between Id – Li and B-B’, Both symmetry and asymmetry group showed statistically significant correlations in horizontal and anteroposterior movements. In the relationship between Pog – Pog’, symmetry group showed statistically significant correlations in anteroposterior and vertical movements, and asymmetry group showed statistically significant correlations in horizontal and vertical movements. In the relationship between Me – Me’, both symmetry and asymmetry group showed statistically significant correlations in horizontal, anteroposterior, and vertical movements. (P<0.05). 2. In the relationship between L2 dev. - LL dev. and L2 non. – LL non., symmetry group showed statistically significant correlations in horizontal, anteroposterior, and vertical movements. However, asymmetry group showed statistically significant correlations only in horizontal movements. (P<0.05). 3. In the relationship between B dev.- B’ dev., symmetry group showed statistically significant correlations in horizontal and anteroposterior movements. asymmetry group showed statistically significant correlations in horizontal and vertical movements. In the relationship between B non.- B’ non., symmetry group showed statistically significant correlations in horizontal and vertical movements. asymmetry group showed statistically significant correlatios in horizontal, anteroposterior, and vertical movements. (P<0.05). 4. When comparing the horizontal movement ratio of the hard tissue and the soft tissue landmarks in asymmetry group, movement ratio of L2 dev. – LL dev. showed 1.06 and movement ratio of B dev. – B’ dev. showed 0.88. However, movement ratio of L2 non. – LL non. showed 0.72 and movement ratio of B non. – B’ non. showed 0.78. The movement ratio of hard tissue and soft tissue landmarks was lower in the non deviation side than in the deviation side. This study showed movement ratio of hard tissue and soft tissue, and difference between deviation side and non deviation side. The results of this study could be a reference for 3D diagnosis when predicting postoperative soft tissue changes in orthognatic surgery of skeletal Class III malocclusion patients with facial asymmetry.-
