dc.description.abstract전통적인 골재생술에서 신생골 유도를 촉진하고 치유기간을 단축시키는 목적의 약리학적 접근을 통한 다양한 시도가 이루어져왔다. 최근에는 골형성단백질 (Bone Morphogenetic Protein; BMP)과 같은 성장인자를 골이식재에 첨가 시 골형성세포 (Osteoblast)의 분화가 촉진되는 것이 입증되었고, 특히 유전자 재조합 기술로 추출된 recombinant human Bone Morphogenetic Protein (rhBMP) 는 임상적 효과가 검증되었다. 하지만 심한 부종, 파골세포 활성에 의한 골흡수, 높은 가격 등의 단점들이 여전히 문제가 되었고, 이러한 단점을 극복하고자 골형성에 관여하는 성장인자들을 대체할 수 있는 소분자를 첨가하는 방안이 대두되었다. Sodium butyrate (SB)은 히스톤 탈아세틸화 효소 (histone deacetylase)의 억제제로, 낮은 농도에서 조골세포의 분화와 혈관형성을 촉진하는 것으로 알려져 있고, Dimethyloxalyglycine (DMOG)는 조혈작용을 유도하는 전사인자를 촉진하여 혈관형성을 유도하고 골형성을 촉진하는 것으로 알려져 있다. 몇몇의 연구에서 SB와 DMOG를 각각 또는 함께 적용하였을 때 유효한 골형성을 보인 것으로 보고되었으나, 이들은 대부분 신생골형성이 다소 유리한 국소적인 골내 결손부에서 이루어진 실험으로, 골내 결손부가 아닌 환경에서 수직적인 신생골형성을 보고자 한 연구는 부족하다. 따라서 본 연구는 토끼의 두개골에서 SB와 DMOG를 함께 황산칼슘 (CaS)에 탑재하여 수직적인 골형성을 유도하였을 때, 두 소분자의 상호 상승 효과가 나타나는지를 방사선학적, 조직학 분석 및 조직계측학적 분석을 통해 확인하고자 한다. 총 10 마리의 토끼 (male New Zealand white rabbit)가 실험에 사용되었고, 각 개체의 두개골에 4 개의 플라스틱 실린더 (지름: 7mm, 높이: 5mm)를 고정 후, 실린더 내부에 군배정에 따라 무작위로 CaS 단독 (대조군), SB 를 탑재한 CaS (CaS/SB), DMOG 를 탑재한 CaS (CaS/DMOG), 그리고 SB 와 DMOG 를 함께 탑재한 CaS (CaS/DMOG/SB)를 각 실린더마다 적용하였다. 사용된 소분자의 농도는 각각 100 μmol/L 로 CaS 와 교반 후 체에 걸러 500 and 1,000 μm 의 균일한 입자를 만들어 사용하였다. 수술 후 2 주와 8 주에 각 5 마리의 실험동물을 희생한 후 채득한 시편을 염색하였다. 시편 염색은 실린더가 강산과 유기용매에 녹는 이유로 탈회과정없이 염색한 후, 방사선적, 조직학적 그리고 조직계측학적 분석을 시행하였다. 동일 군내에서의 시간대별 통계적 비교를 위해서는 독립표본 t 검정을 시행하였고, 동일한 시기에서의 군간 통계적 비교를 위해서는 일원배치분산분석을 시행하였다 (p < 0.05). 방사선적 계측과 조직학적 분석에서, 4개의 군 모두 2주차에는 신생골층, 분해된 CaS층, 그리고 분해되지 않은 원형의 CaS층이 특징적으로 구분되어 관찰된 반면, 8주차에는 증가한 신생골량과 더 많이 흡수 또는 분해된 CaS 양상을 관찰할 수 있었다. 조직계측학적 분석에서는, 4개 군 모두 2주차에 비해 8주차에서 더 많은 신생골이 관찰되었고, 통계적 유의성은 CaS/DMOG군에서 8주차와 2주차 사이에서만 관찰되었다 (p < 0.05). 8주차에서 4개 군 사이의 통계적 유의차가 나타나지 않은 반면, 2주차에서는 CaS/DMOG군이 대조군에 비해 유의하게 더 적은 잔존 이식재 면적이 확인되었다. 본 실험의 결과, 수직적 신생골형성 측면에서 SB와 DMOG의 상호 상승효과를 확인하기는 어려웠으나, 각각의 소분자의 역할은 확인할 수 있었고, 특히 DMOG가 골이식 후 치유 초기에 혈관형성을 촉진하는 것을 알 수 있었다. 이번 실험에 사용된 실험모델은 플라스틱 실린더를 이용한 onlay graft model로 골형성에 있어서 외부 영향과 우연적 요소를 차단하는 극한의 조건을 제공하여 소분자들의 효과를 단독으로 확인하기에는 적합하였으나, 실제 구강환경처럼 투과성과 친수성을 띄는 실린더를 사용한다면 혈액과 체액이 공급하는 여러 성장인자의 영향으로 좀 더 많은 양의 신생골을 얻을 수 있었을 것으로 생각된다. 또한 소분자의 전달 매개체 역할을 한 CaS가 너무 빨리 분해된 점을 극복하고, 조골세포의 분화 유도와 혈관형성을 촉진하는 데에 적합한 소분자의 농도를 규명하는 것이 후속 연구를 위해 필요할 것이다. Purpose: To determine the synergistic effects of SB and DMOG loaded on calcium sulfate (CaS) on bone regeneration in the challenging vertical augmentation model in the rabbit calvarium. Methods: Four plastic cylinders screwed on the calvarium of each of 10 rabbits were randomly grafted with CaS, CaS/SB, CaS/DMOG, or CaS/DMOG/SB. All specimens were assessed by radiographic, histologic, and histomorphometric analyses. In the radiographic analysis, three different layers (new bone, degraded CaS, and pristine CaS layers) could be distinguished within the cylinder in all groups at 2 weeks. Results: Newly formed bone grew up from basal bone, and CaS in contact with newly formed bone was degraded into small particles to form a different layer. At 8 weeks, most of the pristine CaS had been absorbed and hardly seen within the cylinder. In the histomorphometric analysis, all groups showed comparable new bone areas and heights at 2 and 8 weeks. The DMOG group showed a significant increase in new bone area at 8 weeks compared with 2 weeks, but there was no significant difference among the groups at 8 weeks. The DMOG group showed significantly lower values for the residual material area than the control group at 2 weeks. Conclusion: Within the limitations of this study, SB and DMOG seem to exert smaller synergistic effects on bone regeneration compared to CaS alone in vertical bone augmentation.-
