Role of Activated Leukocyte Cell Adhesion Molecule (ALCAM/CD166) in Pulmonary Fibrosis related to Interstitial Lung Disease

Abstract

Background Idiopathic pulmonary fibrosis (IPF) is a chronic, progressive lung disease of unknown cause, and is the most aggressive, irreversible, and usually lethal disease among interstitial lung diseases. Activated leukocyte cell-adhesion molecule (ALCAM:CD166) is a transmembrane glycoprotein belonging to the immunoglobulin superfamily. Objective To investigate the role of ALCAM on the development of IPF. Methods C57BL/6 mice were used to establish a murine model of pulmonary fibrosis. ALCAM deficient (ALCAM-/-) and wild type (WT) mice were used for the bleomycin-induced fibrosis model. WT TGF-β1 transgenic (Tg) mice and ALCAM-/- TGF-β1 Tg mice were used for the TGF-β1-induced fibrosis model. ALCAM expression was examined by immunohisto-chemical staining for ALCAM in lung tissues of IPF patients and controls. Levels of ALCAM were measured in lung tissues and bronchial alveolar lavage fluid. Histopathologic investigation of pulmonary fibrosis was accessed by hematoxylin and eosin staining and Ashcroft score. A Masson’s trichrome (MT) stain and sircol assay were performed to assess the collagen deposition. A TUNEL evaluation was performed to assess apoptosis in a bleomycin-induced model. Western blots were carried out using antibodies against ALCAM, cleaved caspase 3, ERK1/2, p-ERK1/2, JNK, p-JNK, p38, p-p38, Akt, p-Akt, and β-actin. Microarray gene expression analysis was also performed. Results ALCAM protein concentrations in BALFs and mRNA and protein levels of ALCAM in lung tissues were decreased in bleomycin-treated mice compared to control mice. ALCAM expression was reduced in lung tissues of IPF patients. In ALCAM-/- mice treated with bleomycin, compared to WT mice treated with bleomycin, inflammatory cells and the amount of total protein and soluble collagen in BALF were more significantly increased. The body weights were more significantly reduced. The degree of lung fibrosis and collagen deposition were more significant in ALCAM-/- mice. Reduced ALCAM levels were observed in BALFs and lung tissues of doxycycline-administered TGF-β1 Tg mice. Moreover, fibrotic responses were exacerbated in ALCAM-/- TGF-β1 Tg mice. Lung tissues in ALCAM-/- mice had a higher number of apoptotic cells in the bleomycin-induced model. In ALCAM-/- mice, compared to WT mice, bleomycin treatment accelerated the phosphorylation of ERK1/2 and Akt. The expression of genes related to focal adhesion, extracellular matrix receptor interaction, and PI3K-Akt signaling pathway were significantly differently expressed between bleomycin-treated WT and ALCAM-/- mice. Conclusions These data suggest that ALCAM contribute to the prevention of pulmonary fibrosis, and that the role of ALCAM may be associated with TGF-β responses via the PI3K-Akt and ERK signaling pathways. 특발성 폐섬유증은 원인이 알려져 있지 않으며, 만성적으로 진행하여 사망에 이르는 간질성 폐질환들 중에서 가장 예후가 좋지 않은 질환이다. Activated leukocyte cell-adhesion molecule (ALCAM)은 면역글로불린 상과에 속하는 막관통 당단백질이며, 면역반응, 종양의 진행과정, 여러 종류의 세포와 조직의 발생과 유지에 관여하는 것으로 알려져 있으나 정확한 기전은 불분명한 상태이다. 본 연구에서는 마우스 모델을 통하여 특발성 폐섬유증의 발생 과정에서 ALCAM의 역할을 규명해 보고자 하였다. Bleomycin을 투여하여 폐섬유화를 유도한 마우스의 폐세척액과 폐조직에서 ALCAM의 발현양을 대조군 마우스와 비교하였다. 특발성 폐섬유증 환자의 폐조직과 정상 폐조직에서 ALCAM의 발현양을 비교하였다. 야생형 마우스와 ALCAM 결핍 마우스에 bleomycin을 투여하여 폐섬유화를 유도한 후에 폐세척액과 폐조직에서 염증의 정도, 폐섬유화, 콜라겐 침착 정도를 비교하였다. Doxycycline을 투여하였을 때, TGF-β1의 발현이 증가하여 폐섬유화가 발생하는 TGF-β1 과발현 마우스를 이용하여 ALCAM 발현/TGF-β1 과발현 마우스와 ALCAM 결핍/TGF-β1 과발현 마우스에 doxycycline을 투여하여 폐섬유화를 유도한 후, ALCAM 발현 정도, 폐섬유화, 콜라겐 침착 정도를 비교하였다. ALCAM결핍 마우스와 야생형 마우스를 이용한 bleomycin 유도 폐섬유화 모델에서 TUNEL 염색을 통해 세포자연사 정도를 비교하고, ALCAM, cleaved caspase 3, ERK1/2, p-ERK1/2, JNK, p-JNK, p38, p-p38, Akt, p-Akt, β-actin에 대한 항체를 이용하여 웨스턴 블롯을 실시하였으며, 미세 배열 유전자 발현 분석을 실시하였다. 그 결과 야생형 마우스에서 bleomycin을 투여하여 폐섬유화가 유도된 군의 폐세척액과 폐조직에서 ALCAM의 발현양이 감소되어 있는 것을 확인할 수 있었다. 또한, 특발성 폐섬유화를 앓는 환자의 폐조직에서 마찬가지로 ALCAM의 발현양이 감소되어 있는 것을 확인할 수 있었다. ALCAM 결핍 마우스와 야생형 마우스에 bleomycin을 투여하여 폐섬유화를 유도하였을 때, ALCAM 결핍 마우스의 폐세척액에서 염증세포 수, 단백질과 콜라겐의 양이 야생형 마우스와 비교하여 더 증가되어 있는 것을 확인할 수 있었고, 체중의 감소도 더 심하게 발생하였다. TGF-β1 과발현 마우스를 이용하여 폐섬유화를 유도한 경우에도 ALCAM의 발현이 감소하는 것을 확인할 수 있었고, ALCAM 결핍 마우스에서는 폐섬유화의 정도와 콜라겐의 침착이 더 심하게 발생하는 것을 확인할 수 있었다. Bleomycin를 이용한 폐섬유화 모델에서 ALCAM 결핍 마우스의 폐조직에서 세포자연사 세포의 비율이 더 높은 것을 확인할 수 있었다. 또한, 폐섬유화 발생시 ALCAM 결핍 마우스에서는 ERK 1/2와 Akt의 인산화가 야생형 마우스보다 더 증가하는 것을 확인할 수 있었다. 미세 배열 유전자 발현 분석 결과 초점 접착, 세포외 매트릭스 수용체의 상호작용, PI3K-Akt 신호전달체계 등이 ALCAM 결핍 마우스의 폐섬유화에 중요하게 작용하는 것을 확인할 수 있었다. 이러한 결과를 바탕으로 ALCAM이 특발성 폐섬유화의 진행 예방에 관여하는 것을 확인하였다. 이 과정은 PI3K-Akt 신호전달체계와 ERK 신호전달체계를 통한 TGF-β 반응과 연관이 있는 것으로 보인다.
