골격성 III급 비대칭 환자의 악교정 수술 전후 비중격 휘어짐 비교

Abstract
골격성 Ⅲ급 비대칭 환자는 상악골의 후퇴 및 하악골의 전돌과 더불어, 경조직과 연조직의 편측 편위를 보인다. 또한 비대칭 환자에서 얼굴의 비대칭과 비중격 휘어짐(nasal septal deviation : NSD)은 높은 상관관계를 보인다. 본 연구에서는 골격성 Ⅲ급 비대칭 환자의 비중격 휘어짐의 정도를 평가하고, 악교정 수술 전후, 그리고 유지 1년 시점의 비중격 휘어짐의 변화를 측정하고자 하였다. 부가적으로 비중격 휘어짐 정도가 하악골의 비대칭과 상관관계가 있는지, 악교정 수술 과정에서 상악골의 anterior impaction 양, canting correction 양에 따라 비중격 휘어짐의 각도 변화에 차이가 있는지 등을 알아보고자 하였다. 골격성 Ⅲ급 비대칭으로 진단받고, 악교정 수술을 받은 만 18세 이상의 27명의 성인환자의 의무기록 및 3 dimension computed tomography 기록을 이용하여 수술 전후 및 유지기간의 골격적 변화를 분석하고 수술 시에 비중격 휘어짐에 영향을 줄 수 있을 것이라고 생각되는 요소와의 상관관계 분석을 실시하여 다음과 같은 결론을 얻었다. 1. 비중격 휘어짐이 있는 환자에서 악교정 수술 직후 비중격 휘어짐은 약간 감소하였다. 악교정 수술 1년 후 비중격 휘어짐은 수술 전에 비해 감소된 값을 유지하였다. 2. 골격성 Ⅲ급 비대칭 환자에서 Menton deviation 값과 비중격 휘어짐의 값은 상관관계가 없었다. 3. 골격성 Ⅲ급 비대칭 환자에서 하악의 비대칭 방향은 비중격 휘어짐의 방향과 유의한 상관관계가 있었다. 4. 악교정 수술 시 상악골의 impaction 양과 canting correction 등의 이동량과 비중격 휘어짐 변화량은 상관관계가 없었다. 이상을 종합하여 골격성 Ⅲ급 비대칭 환자의 악교정 수술 시 비중격 휘어짐이 개선되지만 완전히 교정된다고 볼 수 없다. 따라서 수술 전 비중격 휘어짐이 있는지 확인하고, 휘어짐이 관찰되는 환자에서는 증상여부를 확인 후 필요 시 추후에 비중격교정술(septoplasty)을 고려할 수 있을 것으로 생각된다.

In skeletal Class Ⅲ patients with facial asymmetry, with retrusion of maxilla and protrusion of mandible, both hard tissue and soft tissue show deviation to one side. And there is high correlation between facial asymmetry and nasal septal deviation(NSD). In this study, we aimed to evaluate the degree and direction of NSD of skeletal Class Ⅲ patients with facial asymmetry, and compare the degree of NSD after bimaxillary orthognathic surgery. Also, we aimed to study the correlation between the anterior impaction of maxilla, canting correction and NSD change.The study included 27 patients whose dental records are retrospectively reviewed at the Department of Oral and Maxillofacial Surgery and Orthodontics, College of Dentistry, Yonsei University, Seoul, Korea. They were diagnosed with mandibular prognathism and facial asymmetry. All patients underwent bimaxillary orthognathic surgery from July 2013 to December 2016. Pre-(T0) and post-treatment(T1) and retention 1 year(T2) records and 3D-CT were retrospectively collected. We measured horizontal, vertical angle of NSD and the results were as follows. 1. The vertical, horizontal NSD has significantly reduced after surgery(T1) , and maintained the reduced angle at retention 1 yr(T2) (p < 0.05). 2. No significant correlation between the degree of menton deviation and NSD at T0(p > 0.05). 3. The direction of deviated mandible and nasal septum at T0 showed statistically significant correlation (p < 0.05). 4. No significant correlation between the anterior impaction of maxilla, and canting correction during surgery and the NSD change (p > 0.05).The results indicate that the NSD is slightly corrected after the bimaxillary orthognathic surgery, but not completely. Thus in patientswith skeletal Class Ⅲ and facial asymmetry, a careful observation is needed before the orthognathic surgery. If significant NSD is observed, an additional septoplasty might be considered after the orthognthic surgery with an otolaryngologist.
