침샘관암에서 PD-L1의 발현율 분석과 종양 표지자로서의 효용성에 대한 고찰

Abstract

침샘관암은 두경부에서 발생하는 비교적 드문 질환으로 다양한 치료가 시행되고 있음에도 그 예후가 매우 불량한 암종으로 알려져 있다. 전통적으로 외과적 절제술을 비롯한 항암화학요법, 방사선 요법 등이 시행되어 왔으나 보다 성공적인 치료 방법에 대한 요구가 있었다. 이에 면역요법 등을 통한 새로운 치료방법이 시도되고 있으며, 최근 programmed death 수용체를 표적으로 하는 치료법이 다양한 암에서 우수한 결과를 보인점을 토대로 침샘관암에서도 programmed death 수용체를 표적으로 하는 치료에 대해 연구가 진행되고 있다. 이에 본 연구는 PD-L1의 발현율을 조사하고 이것이 생존율에 미치는 영향을 분석하여 침샘관암에서 PD-L1을 통한 면역치료의 가능성에 대해서 고찰해보고자 하였다. 본 연구의 결과와 다양한 논문 고찰을 통해서 침샘관암에서 PD-L1의 발현률이 다른 암종에 비해 낮음을 발견하였고 또한 생존률에 미치는 영향에 대해서도 일관된 결과를 보이지 않는다는 점을 발견하였다. 이에 대해 침샘관암에서 PD-L1을 이용하여 환자를 선택하고 면역치료를 시행하는 데에는 제한이 있을 것이라는 결론을 도출하였다.

Salivary duct carcinoma is a rare tumor in head and neck which has one of the poorest prognosis among other types of cancers, even though multiple approaches of treatments are performed.The conventional treatments include surgical resection, chemotherapy, as well as radiation therapy. However, there has been a need for more successful treatment methods for salivary duct carcinoma. Therefore, treatment options using new technologies are attempted like immunotherapy. Based on recent study results indicating that targeting programmed death receptors are effective in treating various cancers, this study aimed to identify the frequency of PD-L1 expression and its impact on survival rate in salivary duct carcinoma. According to this study and review of similar studies, we discovered that the expression of PD-L1 in salivary duct carcinoma was lower than other types of cancers. The impact of PD-L1 on survival rate also showed inconsistency in salivary duct carcinoma. Thus, a conclusion was made that using PD-L1 as patient selection marker has limitation and immunotherapy using PD-1/PD-L1 in salivary duct carcinoma needs further evaluation for clinical application
