측두하악장애와 경추 후만증의 연관성

Abstract

측두하악장애(Temporomandibular disorders, TMD)와 경추의 후만증은 현대인에게 발병율이 증가하고 있다. 두경부의 부적절한 자세와 TMD 사이에 관련이 있다는 의견이 제기되고 있고, 이에 대한 견해가 분분하다. 본 연구의 목적은 경추의 후만증과 TMD사이에 관련이 있는지 알아보는데에 있다. 강남세브란스병원 치과병원에 내원한 환자들 중 측모두부규격 방사선사진을 촬영한 환자들을 대상으로 측두하악장애 관련 증상을 호소한 221명을 실험군으로, 측두하악장애 관련 증상이 없는 188명을 대조군으로 설정하였다. 실험군과 대조군의 경추 후만곡 정도를 측정하고 분류하여 그 비율을 조사하였다. 또한 실험군 중에서 Helkimo index가 조사된 사람들을 통해 측두하악장애의 심한 정도와 경추의 후만곡 정도를 비교하여 양측간에 심한 정도가 비례하는지 조사하였다. 대조군에 비해 실험군에서 경추의 후만증이 더 높은 비율로 나타났다 (p < 0.001). 그러나 실험군 내에서 측두하악장애의 심한 정도와 경추 후만곡의 심한 정도는 비례하지 않았다 (p > 0.05). 따라서 비록 측두하악장애 환자군에서 경추 후만증이 높은 비율로 관찰되었지만 측두하악장애와 경추후만증이 상호간에 인과관계가 있다고 보기는 어렵다.

Temporomandibular disorders(TMD) and cervical kyphosis are increasing in modern people. There is an opinion that TMD is related to the inadequate attitude of the head and neck. The purpose of this study is to determine whether there is a relationship between TMD and cervical kyphosis. Among the patients who visited the dental clinic of Gangnam Severance Hospital, 221 patients who complained of TMD symptoms were selected as experimental group and 188 subjects without TMD symptoms as control group. The degree of curvature of the cervical vertebrae was measured and classified in the experimental group and the control group. We also compared the severity of TMD and the severity of cervical kyphosis with those of Helkimo index in the experimental group. There was a higher proportion of cervical kyphosis in the experimental group than in the control group (p < 0.001). However, the severity of TMD and the severity of cervical kyphosis were not correlated within the experimental group (p > 0.05). Thus, although cervical kyphosis were observed at a high proportion in patients with TMD, it is difficult to conclude that there is a mutual causal relationship between TMD and cervical kyphosis.
