Effects of transarterial chemoembolization on regulatory T cell and its subpopulations in patients with hepatocellular carcinoma

Abstract
조절 T 세포는 항암 면역기능을 조절하는 데 필수적인 역할을 하는 것으로 알려져 있다. 간암에서 조절 T 세포에 대한 몇 개의 연구가 있지만 그 수가 많지 않으며, 특히 간동맥항암색전술에서 조절 T 세포의 대한 연구는 제한적이다. 본 연구의 목적은 간암 환자에게서 간동맥항암색전술이 조절 T 세포와 세포 분획에 미치는 영향을 알아보고자 하였다. 본 연구에는 2010년부터 2012년동안 연세대학교 의과대학 세브란스 병원에서 간암으로 간동맥항암색전술을 받은 27명의 환자와 23명의 건강대조군의 말초혈액을 추출하여 조절 T 세포의 빈도를 조사하였다. 환자는 간동맥항암색전술 후 종양반응에 따라 완전 반응군과 불완전 반응군으로 나누어 분석하였다. 조절 T 세포의 빈도와 여러 임상적 변수들의 관계를 분석하였고, 조절 T 세포가 예후를 예측할 수 있을지 조사하였다. 간암 환자에서의 조절 T 세포의 빈도는 건강 대조군에 비해 유의하게 높았다 (7.52±2.62 vs. 4.99±1.49 %, P < 0.001). 특히, 조절 T 세포를 세분화하여 조사해보니, 제 2 분획 조절 T 세포의 빈도가 간암과 건강 대조군 사이 유의한 차이를 보였다 (2.51±1.08 vs. 0.60±0.28 %, P < 0.001). 간동맥항암색전술 시술 전과 후의 조절 T 세포의 수를 비교한 연구에서는 불완전 반응군에서 시술 후 제 3 분획 조절 T 세포의 빈도가 완전 반응군 보다 유의하게 높았다 (3.97±0.67 vs. 3.07±0.96 %, P = 0.016). 또한 완전 반응군에서 제 3 분획 조절 T 세포의 변화가 시술 전 후 유의한 감소를 보였다 (63.8±42.2 → 53.2±31.9/mm3, P = 0.050). 시술 후 높은 제 3 분획 조절 T 세포의 빈도를 보인 환자군은 낮은 빈도를 보인 환자군에 비해 유의하게 짧은 무재발 생존 기간을 보였다 (3.8 vs. 11.6 개월, P = 0.038). 다변량 분석에서, 시술 전 저알부민혈증 (위험 비율 3.324; 95% 신뢰구간 1.098-10.063, P = 0.034)과 시술 후 높은 제 3 분획 조절 T 세포 (위험 비율 3.080; 95% 신뢰구간 1.091-8.696, P = 0.034)가 재발과 관련 있을 수 있는 인자로 확인되었다. 결론적으로, 간암 환자의 조절 T 세포의 빈도는 건강 대조군에서 보다 유의하게 높았다. 또한 간동맥항암색전술 시술 후 말초혈액에서 높은 제 3 분획 조절 T 세포의 빈도를 보인 환자는 낮은 빈도를 보인 환자에 비해 무재발 생존률이 유의하게 낮았다.

Regulatory T cell (Treg) plays an essential role in regulating antitumor immunity. Although the role of Treg in hepatocellular carcinoma (HCC) has been investigated, the effects of transarterial chemoembolization (TACE) on Tregs are unclear. The aim of this study was to investigate the effect of TACE on Treg in HCC patients. The frequency of peripheral blood Tregs in 23 healthy controls and 27 HCC patients underwent TACE between 2010 and 2012 at Severance Hospital of Yonsei University College of Medicine, Seoul, Korea were measured. The patients were divided into two groups according to tumor response; complete response group and incomplete response group. The correlations between the frequency of Treg and clinical factors including survival and progression were analyzed. The frequency of Treg in HCC patients was significantly higher than in healthy controls (7.52±2.62 % in HCC vs. 4.99±1.49 % in healthy controls, P < 0.001). Regarding Treg subpopulations, the frequency of Treg (II) was significantly higher in HCC patients than in healthy controls (2.51±1.08 % in HCC vs. 0.60±0.28 % in healthy controls, P < 0.001). In comparison of Treg numbers at baseline and post-TACE by tumor response, the frequency of post-TACE Treg (III) in patients with incomplete response was significantly higher than that in patients with complete response (3.97±0.67 vs. 3.07±0.96 %, P = 0.016). The change of Treg (III) in complete response group from baseline to post-TACE was statistically significant (63.8±42.2 → 53.2±31.9/mm3, P = 0.050). Patients with a high post-TACE Treg (III) exhibited a significantly shorter median time to progression than those with a low post-TACE Treg (III) (median time to progression; 3.8 vs. 11.6 months, P = 0.038). In multivariate analyses, hypoalbuminemia (hazard ratio 3.324; 95% CI 1.098-10.063, P = 0.034) and high post-TACE Treg (III) (hazard ratio 3.080; 95% CI 1.091-8.696, P = 0.034) were significant factors for associating with progression. In conclusion, the frequency of Tregs in HCC patients was significantly higher than in healthy controls. And patients with a high post-TACE Treg (III) exhibited a significantly lower progression-free survival rate than those with a low post-TACE Treg (III).
