Association between appendicular skeletal muscle mass and depressive symptoms

dc.description.abstractINTRODUCTION: The effects of skeletal muscle mass on depressive symptoms remain poorly understood, especially in the middle-aged population. I examined the relationship between skeletal muscle mass and depressive symptoms according to sex and menopausal status in the middle-aged Korean population. METHODS: Herein, 1,151 men and 2,176 women aged 30–64 years completed questionnaires and underwent health examinations in the Cardiovascular and Metabolic Disease Etiology Research Center study. Appendicular skeletal muscle mass (ASM) was measured via bioelectrical impedance analysis and adjusted for height squared (ASM/Ht2). Tertile groups of ASM/Ht2 were used for analysis. Depressive symptoms were assessed using the Beck Depressive Inventory-II (BDI), and the presence of depressive symptoms was defined as a BDI score ≥20 or participants with taking antidepressants. RESULTS: Multiple logistic regression analysis using a fully adjusted model showed that depressive symptoms was more frequently observed among men in the lower (OR: 3.91, 95% CI: 1.63–9.39) and middle (OR: 2.81, 95% CI: 1.31–6.01) ASM/Ht2 tertile than among those in the higher ASM/Ht2 tertile. In women, the risk for depressive symptoms was higher in the lower (OR: 1.38, CI: 0.90–2.01) and middle (OR: 1.31, CI: 0.91–1.88) ASM/Ht2 tertile than those in the higher ASM/Ht2 tertile; however, these associations did not demonstrate statistical significance. Stratification analysis according to the menopausal status showed that the risk for depressive symptoms was lower among premenopausal women in the lower and middle ASM/Ht2 tertile, and higher among postmenopausal women in the lower and middle ASM/Ht2 tertile than among those in the higher ASM/Ht2 tertile; however, both of their associations did not demonstrate statistical significance. CONCLUSION: I conclude that the independent association between low skeletal muscle mass and depressive symptoms was observed in men, but not in women. 사지근육양의 감소는 중년부터 시작되며, 여자에서는 폐경기 이후 우울감 유병이 증가된다고 보고되어 왔다. 그러나 사지근육양과 우울감과의 관련성에 대한 연구는 주로 노인층을 대상으로 이루어져 왔으며 그 결과도 일관적이지 않다. 따라서, 본 연구는 한국 중년 성인을 대상으로 성별과 폐경여부에 따라 사지근육양과 우울감 사이의 독립적인 관련성을 확인하고자 하였다. 연구 방법 본 연구는 Cardiovascular and Metabolic Disease Etiology Research Center Cohort Study에 참여한 30~64세의 남자 1,151명과 여자 2,176명을 대상으로 하였다. 사지근육양은 bioelectrical impedance analysis로 측정하였고, 키 제곱으로 보정하였다. 사지근육양은 삼분위수에 따른 범주형 자료로 분석에 이용하였다. 우울감은 한국판 Beck Depressive Inventory-II로 평가하여 20점 이상이거나 항우울제를 복용하는 경우 우울감이 있다고 정의하였다. 연구 결과 사지근육양 삼분위수에 따른 우울감의 오즈비는 낮은 정도의 사지근육양을 가진 남자에서는 3.91 (95% 신뢰구간: 1.63-9.39), 중간 정도의 사지근육양을 가진 남자에서는 2.81 (1.31-6.01) 이었다. 여자에서 낮은 정도의 사지근육양을 가진 그룹에서는 1.38 (0.90-2.01), 중간 정도의 사지근육양을 가진 그룹에서는 1.31 (0.91-1.88) 배 우울감에 대한 위험이 높았지만 통계적으로 유의한 차이는 없었다. 폐경여부로 층화하여 평가하였을 때, 폐경 전 여자에서는 사지근육양이 낮은 그룹에서 우울감의 위험이 낮았고, 폐경 후 여자에서는 사지근육양이 낮은 그룹에서 우울감의 위험이 높았지만, 모두 유의한 차이는 관찰하지 못했다. 결론 및 고찰 낮은 사지근육양과 우울감과의 독립적인 관련성은 남자에서만 관찰되었으며, 여자에서는 폐경여부로 층화 후에도 독립적인 관련성이 관찰되지 않았다.-
