입원환자의 병원이용 특성과 선택 동기가 환자경험에 미치는 영향

Abstract

As medical market competition is getting fierce and center of medical service is changing from provider-centered to patient-centered conditions, patient experience becomes an important factor to secure high competitiveness. Therefore, it is very important to manage various patients’ experience, as everything patient could experience should be patient-centered in order to improve quality of medical care and provide patient-centered care. As selecting discharged patients as subjects, this study is to analyze hospital selection motive and hospital use characteristics from hospitalization to discharging and research how the motive and hospital use characteristics influence patient experience evaluation and perception. This study surveyed 200 patients, who are going to be discharged after hospitalization in a general hospital in north Gyeonggi province. As main methods, this study conducted correlation analysis among difference analysis, hospital selection motive, and patient experience evaluation in order to identify experience evaluation factors of each field, depending on demographic characteristics of subject, hospital use characteristics, and hospital selection motive. It conducted multiple regression analysis to research how the independent variables such as patient’s demographic characteristic, hospital selection motive, and hospital use characteristic could affect the dependent variable that is the experience evaluation of each field. The results of analysis are as follows. First, this study found that the hospital use characteristic’s nursing care integrated service wards and the hospital selection motive’s continuous loyalty, hospital facilities, and convenience could influence nurse service experience. Second, this study found that the hospital use characteristic’s nursing care integrated service wards and the hospital selection motive’s facilities and convenience could influence doctor service experience. Third, this study found that the hospital use characteristic’s nursing care integrated service wards and the hospital selection motive’s continuous loyalty and the Internet hospital promotion could influence medication and therapy course experience. Fourth, this study found that the hospital use characteristic’s nursing care integrated service wards could influence hospital environment experience. Fifth, this study found that the hospital use characteristic’s nursing care integrated service wards and the hospital selection motive’s kindness of staff and patient care could influence patient right assurance experience. Sixth, this study found that the hospital use characteristic’s nursing care integrated service wards and the hospital selection motive’s kindness of staff, patient care, hospital facility, and convenience could influence overall evaluation. As synthesizing the results, compared to normal wards, the nursing care integrated service wards have significance in all dependent variables of patient’s experience evaluation, which are nurse service experience, doctor service experience, medication and therapy course experience, and they are important variables to influence the patient experience evaluation. It should be needed to try to promote patient experience, provide patient-centered care, and improve hospital’s competitiveness in company with effective operation of nursing care integrated service wards and continuous expanding methods as considering kindness of staff, patient care, hospital facility, and convenience of the hospital selection motive, which could influence overall evaluation.

환자경험은 의료시장의 경쟁이 점차 치열해지고, 의료 서비스의 중심이 공급자 중심에서 환자중심으로 변화함에 따라 의료기관의 높은 경쟁력을 보장하는 중요한 요소이다. 따라서 의료의 질을 향상시키고, 환자 중심의 의료서비스를 제공하기 위해서는 환자들이 경험하는 모든 것을 환자 중심에서 생각할 수 있는 다양한 환자들의 경험을 관리하는 것이 필요하다. 이 연구는 퇴원하는 환자들을 대상으로 입원 후 퇴원까지 병원서비스를 경험하면서 병원선택 동기와 이용특성을 분석하고, 그 동기나 병원이용 특성 요인이 환자경험평가나 인식에 어떠한 영향을 미치는지 파악하기 위해 진행되었다. 경기도 북부에 위치한 일개 대학병원에서 입원치료 후 퇴원예정인 환자들을 대상으로 200명의 설문조사를 실시하였다. 주요 방법으로는 연구대상자의 인구사회학적 특성, 병원이용 특성, 병원선택 동기에 따른 영역별 경험평가의 요인을 파악하기 위하여 차이분석과 병원선택 동기와 환자경험평가 간의 상관관계 분석을 실시하였다. 독립변수인 환자의 인구사회학적 특성, 병원선택 동기, 병원 이용 특성이 종속변수인 영역별 경험평가에 미치는 영향력을 파악하기 위하여 다중회귀분석을 실시하였다. 연구 결과는 다음과 같다. 첫째, 병원이용 특성의 간호간병통합서비스 병동과 병원선택 동기의 지속적인 충성도와 병원시설과 편리성이 간호사 서비스 경험에 영향을 미치는 요인으로 분석되었다. 둘째, 병원이용 특성의 간호간병통합서비스 병동과 병원선택 동기의 병원시설과 편리성이 의사 서비스 경험에 영향을 미치는 요인으로 분석되었다. 셋째, 병원이용 특성의 간호간병통합서비스 병동과 병원선택 동기의 지속적인 충성도와 인터넷 병원 홍보가 투약 및 치료과정 경험에 영향을 미치는 요인으로 분석되었다. 넷째, 병원이용 특성의 간호간병통합서비스 병동이 병원환경 경험에 영향을 미치는 요인으로 분석되었다. 다섯째, 병원이용 특성의 간호간병통합서비스 병동과 병원선택 동기의 직원의 친절성과 환자배려가 환자권리보장 경험에 영향을 미치는 요인으로 분석되었다. 여섯째, 병원이용 특성의 간호간병통합서비스 병동과 병원선택 동기의 직원의 친절성과 환자배려와 병원시설과 편리성이 전반적인 평가경험에 영향을 미치는 요인으로 분석되었다. 이 연구 결과를 종합해 보면, 간호간병통합서비스 병동이 일반병동보다 환자의 경험평가의 종속변수인 간호사 서비스 경험, 의사 서비스 경험, 투약 및 치료과정 경험, 환자권리보장 경험, 병원환경 경험, 전반적인 경험평가의 모든 종속변수에서 유의한 결과가 나왔으며, 환자경험평가에 영향을 미치는 중요한 요인임을 보여주고 있다. 간호간병통합서비스 병동의 효율적인 운영과 지속적으로 확대시키는 방안과 함께 입원경험에 전반적인 평가에 영향을 미치는 병원선택 동기인 직원의 친절성과 환자배려, 병원시설과 편리성을 고려하여 환자경험을 증진시키고, 환자 중심의 의료서비스를 제공하고 병원의 경쟁력을 높이기 위한 노력이 필요하다.
