청소년의 건강행위 이행정도와 교육 요구도 조사

dc.description.abstractThis study was a descriptive survey study done to investigate health behavior compliance and health related education needs of adolescents. The purpose of this study was to provide fundamental data for the development of health education programmes to promote a healthy life style in adolescents. The participants were 305 students from three areas (big city, small and medium-sized city, rural city). The students were selected from one class per grade and from one coeducational middle school and one high school per region. All participants voluntarily agreed to participate in the study and consent from legal guardians was also obtained. To examine the adolescents’ health behavior compliance, the Korean Adolescent Health Behavior Scale (KAHBS) was used; a tool developed by Shin Yun Hee (2010). The adolescents’ health education needs were measured through a tool with the same items as the KAHBS. Before using the tool the Content Validity Index was examined. Data were collected from April 2 to 12, 2018 and were processed with IBM SPSS version 23.0 to obtain descriptive statistics and independent t-test, chi-square test, one-way ANOVA, Scheffe post hoc analysis, and Pearson correlation coefficients. A summary of the study findings is as follows: 1) The highest level for health behavior compliance was for Substance abuse (3.97±0.16) and the lowest level, Health screening (2.30±0.72). Except for Substance abuse, rank order from the highest was as follows; Sexual behavior, Hygiene, Physical activity, Sleep habits, Stress and mental health, Computer and smartphone use, Weight control, Posture, Dietary habits, Safety. Alcohol consumption and Tobacco use expressed as percentages. The rates were for Alcohol consumption, 26.9%, Tobacco use, 3.3% and Sexual intercourse, 2.6%. 2) The highest level for health education needs was Weight control (4.83±1.20) and the lowest was Substance abuse (3.44±1.40). Following Weight control, the rank order from the highest was as follows; Physical activity and Posture with the same scores, Stress and mental health and Sleeping habits with same scores, Health screening, Oral health, Sexual behavior, Dietary habits, Computer and smartphone use, Safety, Alcohol consumption, Hygiene, and Tobacco use. 3) The health behavior compliance and health education needs of adolescents by general characteristics were as follows.According to the monthly allowance, dietary behavior compliance and health education needs differed among the groups. The survey results for Dietary education needs showed differences by residential area (F=4.11, p=.017) and by school (t=-2.31, p=.022). There were differences between gender for Physical activity behavior compliance (t=.14, p≤.000), Weight control behavior compliance (t=-4.12, p≤.000) and education needs (t=-3.22, p=.001). Stress and mental health behavior compliance differed by school marks (F=5.34, p=.005), household income (F=3.99, p=.020) and education needs had differences by gender (t=-3.01, p=.002). Hygiene behavior compliance (F=6.28, p=.002) and Safety compliance (F=6.28, p=.039) differed according to household income. Investigation of Safety education needs showed that there were differences by residential area (F=5.24, p=.006), number of siblings (F=3.29, p=.039), and household income (F=5.02, p=.007). The rates for Alcohol consumption differed by school marks (χ²=12.13, p=.016), monthly allowance (χ²=16.90, p≤.000), while education needs concerning alcohol consumption differed by school (t=-2.44, p=.016). The rates of Tobacco use showed statistical differences according to residential area (χ²=10.42, p=.034). There were statistical differences in the rates for Sexual intercourse by gender (χ²=7.04, p=.030), and by having had health education (χ²=7.47, p=.024). The health education needs of Substance abuse differed by school (t=-1.99, p=.048). Computer and smartphone use behavior compliance differed by school marks (F=7.47, p=.001). Sleep habits behavior compliance (t=2.92, p=.004) and education needs (t=-2.06, p=.040) differed by gender. The Health screening behavior compliance differed by gender (t=-3.46, p=.001), and education needs differed by residential area (F=3.20, p=.042) and type of school (t=-2.30, p=.022). Posture behavior compliance differed by gender (t=2.67, p=.008), and school marks (F=7.14, p=.001), and education needs differed by residential area (F=4.29, p=.015) and school (t=-3.16, p=.002). The results of this study show that each of the general characteristics is reflected in differences in the health behavior compliance and educational needs. Overall, health behavior compliance differed according to the general characteristics of gender, while health education needs showed significant differences for residential area and school. Based on the above results, a health promotion strategy should be developed to provide appropriate education and characteristics of the students should be considered when providing education to improve health behavior of youth. 본 연구는 청소년을 대상으로 건강행위별 이행정도를 파악하고, 청소년이 원하는 건강행위교육에 대한 요구도를 조사하여 추후 건강한 생활습관을 형성하기 위한 교육 프로그램을 개발하는데 기초자료를 제공하기 위한 서술적 조사연구이다. 연구대상은 대도시, 중소도시, 읍면지역 각각 한 지역씩 남녀공학 중학교 1개교, 고등학교 1개교를 선정하여 학년별로 한 학급을 대상으로 본인과 법적 대리인이 자발적으로 동의한 305명이었다. 연구도구로 건강행위 이행정도는 신윤희(2010)가 개발한 청소년 건강행위 이행정도 도구를 본 연구자가 연구목적에 맞게 보완한 도구를 사용하였고, 건강행위 교육 요구도는 위 도구와 동일한 항목을 활용하였으며 전문가 타당도 확인 후 사용하였다. 자료수집기간은 2018년 4월 2일부터 12일까지이며, 수집된 자료는 IBM SPSS 23.0 통계프로그램을 이용하여 기술통계, independent t-test, chi-square test, one-way ANOVA 및 Scheffe post hoc analysis로 분석하였다. 본 연구의 주요결과를 요약하면 다음과 같다. 1) 건강행위 이행정도가 가장 높은 항목은 물질사용(3.97±0.16)이었고 가장 낮은 항목은 건강검진(2.30±0.72)이었으며 물질사용을 제외하고 순위가 높은 순서대로 성행위, 위생습관, 운동, 수면습관, 스트레스 및 정신 건강, 컴퓨터와 스마트폰 사용, 체중조절, 자세, 식습관, 안전사고 예방으로 조사되었다. 음주와 흡연은 비율로 나타내었는데 음주율은 26.9%, 흡연율은 3.3%였고 성관계 경험이 있는 비율은 2.6%였다. 2) 건강행위 교육 요구도 중 가장 높은 항목은 체중 관리 교육이었으며(4.83±1.20) 가장 교육 요구도가 낮은 항목은 물질남용(3.44±1.40)이었다. 체중 관리 교육 다음으로 교육 요구도가 높은 순서는 신체활동과 자세가 같은 점수로 2순위였고, 3순위는 정신 건강과 수면습관이 같은 점수로 나타났으며, 다음으로 건강검진, 치아 건강, 성, 영양, 컴퓨터와 스마트폰 중독, 안전, 음주, 손위생, 흡연 순으로 조사되었다. 3) 일반적 특성에 따른 건강행위 이행정도와 교육 요구도의 차이를 요약하면 다음과 같다. 식습관은 한 달 용돈에 따라 이행정도(t=4.41, p≤.000)와 교육 요구도(t=2.23, p=.027)가 모두 차이를 보였다. 식습관 교육 요구도 조사 결과 거주 지역별(F=4.11, p=.017), 학교별(t=-2.31, p=.022) 차이도 나타내었다. 운동 이행정도(t=.14, p≤.000)와 체중조절 이행정도(t=-4.12, p≤.000) 및 교육 요구도(t=-3.22, p=.001)는 성별에 따른 차이가 있는 것으로 조사되었다. 스트레스 및 정신 건강 이행정도는 학교성적(F=5.34, p=.005), 가정 수입(F=3.99, p=.020)별로 차이를 보였으며 교육 요구도는 성별(t=-3.01, p=.002) 차이를 나타내었다. 위생습관 이행정도(F=6.28, p=.002)와 안전사고 예방 이행정도는(F=6.28, p=.039)는 가정 수입에 따른 차이가 있었다. 또한 안전사고 예방 교육 요구도는 거주 지역(F=5.24, p=.006), 형제자매 수(F=3.29, p=.039), 가정 수입(F=5.02, p=.007)에 따른 차이가 있는 것으로 조사되었다. 음주율은 학교성적(χ²=12.13, p=.016), 한 달 용돈(χ²=16.90, p≤.000)에 따라 차이가 있었고 음주 교육 요구도는 중·고등학교(t=-2.44, p=.016)에 따른 차이를 보였다. 흡연율은 거주 지역에 따른 통계적 차이를 나타내었고(χ²=10.42, p=.034), 성관계 경험율은 성별(χ²=7.04, p=.030), 최근 1년 이내 건강행위 교육 참여 경험 유무(χ²=7.47, p=.024)에 따른 차이를 보였다. 물질사용 교육 요구도는 중·고등학교(t=-1.99, p=.048)에 따른 차이가 있었고 컴퓨터와 스마트폰 사용 이행정도는 학교성적별(F=7.47, p=.001) 차이를 나타내는 결과를 보였다. 수면습관 이행정도(t=2.92, p=.004)와 교육 요구도(t=-2.06, p=.040)는 성별에 따른 차이가 있었고, 교육 요구도는 중·고등학교별(t=-2.89, p=.004) 차이도 나타내었다. 건강검진 이행정도는 성별(t=-3.46, p=.001) 차이를 보였으며 교육 요구도는 거주 지역별(F=3.20, p=.042), 중·고등학교별(t=-2.30, p=.022) 차이를 보였다. 마지막으로 자세 이행정도는 성별에 따라(t=2.67, p=.008), 학교성적에 따라(F=7.14, p=.001), 차이가 있었고 교육 요구도는 거주 지역별(F=4.29, p=.015), 중·고등학교별(t=-3.16, p=.002) 차이가 있는 것으로 나타났다. 이러한 연구결과를 통해 건강행위별 이행정도와 교육 요구도에 차이를 나타내는 각각의 일반적 특성이 다름을 알 수 있다. 종합적으로 살펴보면 건강행위 이행정도는 일반적 특성 중 성별에서 주로 차이를 나타내었고 건강행위 교육 요구도는 거주 지역과 중·고등학교에 따라 유의한 차이를 보였다. 위와 같은 결과를 반영하여 청소년을 대상으로 건강행위개선 교육을 실시할 때 대상자의 특성을 고려한 적절한 교육을 제공하는 건강증진 전략을 수립해야 할 것이다.-
