복합만성질환을 가진 자녀를 둔 부모의 소아청소년 완화의료 요구도

Abstract

본 연구는 복합만성질환을 가진 자녀를 둔 부모의 소아청소년 완화의료 요구도를 알아보고 대상자의 일반적 특성에 따른 소아청소년 완화의료 요구도의 차이를 파악하는 서술적 조사연구이다. 본 연구에서 복합만성질환을 가진 자녀를 둔 부모의 소아청소년 완화의료 요구도는 연구자가 소아청소년 완화의료에 관한 국내외 문헌들을 통합하여 개발한 32개의 문항으로 구성된 도구를 사용하여 측정하였다. 자료 수집은 2018년 3월 28일부터 5월 18일까지 서울특별시 소재 Y대학교 부속병원에서 복합만성질환으로 진료를 받고 있거나 복합만성질환의 온라인 자조모임에 가입되어 있는 18세 미만 소아청소년의 부모를 대상으로 구조화된 자가보고식 설문지를 사용하여 이루어졌다. 최종 분석에 이용된 설문지는 총 96부이었으며 SPSS IBM 23.0 프로그램을 사용하여 전산통계 처리하였고, 기술통계, Independent t-test, One-way ANOVA로 분석하였고, 사후 분석은 Sheffé test 방법으로 검정하였다. 본 연구의 주요 결과는 다음과 같다: 1. 복합만성질환을 가진 자녀를 둔 부모의 소아청소년 완화의료 요구도의 전체 평균은 4점 만점에 3.58±0.33점이었으며, 모든 문항의 평균이 3.00점 이상으로 전반적으로 요구도가 높았다. 2. 부모의 자녀에 대한 돌봄 요구는 신체적, 심리사회적, 영적 돌봄 영역 순으로 요구도가 높게 나타났다. 하위영역에서 요구도가 가장 높았던 것은 신체기능의 보존 요구이었으며, 문항 중 요구도가 가장 높았던 것은 경련을 조절하는 것이었다. 3. 부모의 자신에 대한 돌봄 요구는 심리사회적, 사별, 영적 돌봄 영역 순으로 요구도가 높게 나타났다. 하위영역에서는 의사소통 요구가 가장 높았고, 문항 중 요구도가 가장 높았던 것은 의료진과의 의사소통이었다. 4. 복합만성질환을 가진 자녀의 일반적 특성에 따른 소아청소년 완화의료 요구도의 차이는 통계적으로 유의하지 않았으나, 자녀가 질병을 3개 이상 가지고 있는 경우, 그룹Ⅱ(기대여명이 제한적인 질환군)의 경우, 진단 받은 지 10년 이상 경과한 경우, 입원횟수가 10회 이상인 경우에 요구도가 가장 높았다. 5. 복합만성질환을 가진 자녀를 둔 부모의 일반적 특성에 따른 소아청소년 완화의료 요구도의 차이는 통계적으로 유의하지 않았다. 이상의 연구 결과를 종합하여 볼 때, 복합만성질환을 가진 자녀와 그 부모에게 소아청소년 완화의료를 제공 시 자녀에게는 신체증상을 조절하고 신체기능을 향상시킬 수 있는 중재를 우선적으로 제공하고, 부모에게는 의료진과 원활한 의사소통을 할 수 있게 함으로써 대상자의 요구에 부합하는 소아청소년 완화의료를 제공할 수 있을 것으로 기대된다. 본 연구의 결과는 대상자의 요구를 우선순위에 따라 반영한 소아청소년 완화의료 프로그램의 내용을 구성하는데 기초자료로 활용될 수 있을 것이다.

The purpose of this study was to investigate the needs for pediatric palliative care (PPC) for parents having a child with complex chronic conditions (CCC) and to investigate the difference in the needs for PPC according to their general and clinical characteristics. In this study, the needs for PPC was measured using a 32-item instrument, developed by the researcher, integrating domestic and foreign literature on PPC. A cross-sectional survey with a structured questionnaire was conducted between March 28 and May 18, 2018. Parents (n=96) who had children under 18 years with CCC were recruited from the inpatient ward of Y University Hospital in Seoul or through the online self-help group. Data were analyzed using descriptive statistics, independent t-test, and One-way ANOVA, using SPSS IBM 23.0 program. The major findings were as follows: 1. The overall average needs for PPC for parents having a child with CCC was 3.58±0.33 out of 4. The average of all items was 3.00 or higher, which was generally high. 2. The needs for caring for a child was higher in order of physical, psychosocial, and spiritual care needs. In the sub-dimension, the preservation of physical function was the highest needs. Among the items, the highest needs was for seizure control. 3. The needs for caring for parents themselves was higher in order of psychosocial, bereavement, and spiritual care needs. In the sub-dimension, communication was the highest needs. Among the items, good communication with medical staff was the highest. 4. The difference in the needs for PPC according to the general and clinical characteristics of children with CCC and their parents was not statistically significant. In conclusion, when medical staff provide PPC for children with CCC and their parents, intervention for controlling physical symptoms and enhancing physical function should be given priority. In addition, it is expected to provide PPC that meets the needs of the parents, by facilitating communication with medical staff.
