Abstract
Background: Physiologic lipid mixtures composed of cholesterol, ceramide and fatty acid have a better effect on mainatining the epidermal homeostasis by barrier recovery than vehicles containing common moisturizing materials, and a have been recently used for dermatoses induced by skin barrier damage such as atopic dermatits and xerotic skin.

Purpose: We performed this study to identify the clinical efficacy of a lipid mixture which substituted pseudoceramide(PC104) for natural ceramides.

Methods: Forty two aged persons over 60 years old with xerotic skin were divided into 2 grouos, of which one group(active group) applied the moisturizer containing the lipid mixture with pseudoceramide(PC 104) and the other group(vehicle group), the cehicle containing common moisturizing materials. The application site for both groups was the shin of both legs and the application was done on the shin twice a day for 4 weeks. Visual score, capacitance, transepidermal water loss and D-squame analysis were measured at 0, 2 and 4 weeks after treatmet, and 2 weeks after the end of treatment to assess clinical efficacy.

Results: In both groups skin t which the applications were made showed a decrease in scales and an increase of water content in stratum corneum over skin which did not receive the application. The active group had a better effect by increasing the water content in the stratum conreum and decreasing the shedding of corneocytes as compared to vehicle group. However transpidermal water loss was rather increased after treatment in both group.

Conclusion: In conclusion, the moisturizer containg lipid mixture with pseudocermide(PC104) has better clinical efficacy on the xerotic skin of aged person in comparision to the vehicelcontaining common moisturizing materials.
