관상동맥성 심장질환의 역학적 연구

관상동맥성 심장질환의 위험요인으로서 현재까지 알려져 있는 혈압, 혈청지질, 흡연, 당뇨유무, 비만, 가족력등이 우리나라에서도 위험요인 여부인지를 알아보기 위해 환자-대조군 연구를 시행하였다. 환자군은 서울시 소재 K병원에서 1989년8월부터 1990년7월 사이에 관상동맥조영술과 심근조영술 진단방법으로 진단된 관상동맥성 심장질환 환자 100명으로 하였고 대조군은 K병원에 입원한 환자중 혈관계, 심맥관계 질환을 가지지 않은 사람 200명으로 하였다.

자료분석은 multiple logistic model의 설정에 들어갈 변수를 선정하기 위해서는 κ**'-검정을, 비차비(odds ratio)의 계산을 위해서는 multiple logistic model 분석방법을 이용하였다.

연구결과는 아래와 같다.

1. 관상동맥성 심장질환의 위험요인으로 비만도, 혈청지질 등의 통계학적으로 유의한 변수로 나왔으며, 비차비는 각각으로 2.2, 2.0으로 계산되었다.

2. 흡연, 고혈압등은 많은 다른 연구와는 달리 관상동맥성 심장질환에 별 관계가 없는 것으로 관찰되었다.

3. 이 연구결과 서구 여러나라와 문화적인 특이성을 갖고 있는 우리나라에서 관상동맥성 심장질환의 위험요인들이 특이한 양상을 보이고 있어 앞으로 이 방면의 지속적인 역학적인 연구가 필요하다고 생각된다.

A case control study was conducted to identify risk factors for Coronary Heart Disease. The case group consisted of patients with Coronary Heart Disease diagnosed by Coronary angiogram and ventrioulogram from August 1989 to July 1990 in K General Hospital in Seoul.

A total of 100 Coronary Heart Disease Cases and 200 patients without evidence of Cardiovascular Disease made case and control groups respectively.

Statistically significant variables were screened by X**2-test, which were entered to the multiple logistic model to calculate the odds ratios for risk factors.

The results were as follows:

1. Odd ratios of two main effects were obesity 2.2 and serum cholesterol 2.0 for Coronary Heart Disease.

2. Hypertension and cigarette smoking was found not to be associated with Coronary Heart Disease in this study contrast to many other reports.

This results implicate further epidemiological studies to investigate not only differences in composition of but also diverse effects of various risk factors of Coronary Heart Disease in distinct cultures.
