본 연구는 보건소의 무료건강검진이 노인의 건강증진행위에 미치는 정도를 파악하고 효율적인 노인건강검진사업 추진 및 비수검자의 검진 참여를 유도하기 위한 전략수립 시 자료로 활용하고자 시도된 연구이다.

대상자는 경기도 B시에 거주하는 만65세 이상 노인을 대상으로 하였다. 설문지를 이용하여 2003년 2월 5일부터 4월 23일까지 자료를 수집하였으며, 수집된 자료는 총 224부가 분석에 사용되었다.

연구도구는 일반적 특성 11문항, 주관적 건강지각 3문항, 노인건강검진 인식정도 및 만족도 12문항, 건강증진행위 측정 35문항 총61문항으로 이루어진 설문지를 사용하였다.

자료 분석은 SAS 통계프로그램을 이용하여 실수와 백분율, 산술평균과 표준편차, t-test, ANOVA, paired t-test 통계방법을 사용하였다.

본 연구의 결과는 다음과 같다.

1) 대상자들의 건강검진 전 건강증진행위 실천정도는 전체평균 2.58점(4점 척도)이었고, 실천정도가 가장 높은 영역은 영양(평균 2.83점)이었고, 가장 낮은 영역은 스트레스 관리(평균 2.27점)이었다.

2) 대상자들의 건강검진 후 건강증진행위 실천정도는 전체평균 2.73점(4점 척도)이었고, 실천정도가 가장 높은 영역은 영양(평균 2.93점)이었고, 가장 낮은 영역은 스트레스 관리(평균 2.36점)이었다.

3) 대상자들의 건강검진전후 건강증진행위 변화량은 5.03점 증가 하였으며 운동과 건강책임에 점수가 높아진 반면 스트레스 관리는 가장 낮은 점수 차이를 보였다.

4) 대상자의 일반적 특성과 건강증진행위 차이에서는 전체적으로 유의하지 않았다. 그러나 건강증진행위의 하위영역중 ꡐ운동ꡑ과 관련하여 성별(t=2.12, p<0.0349)과 연령(t=3.05, p<0.0493)에서 유의한 차이를 보였다.

현재 노인의 건강유지 및 증진을 위해 실시되고 있는 예방적 조치는 무료건강검진제도 이외에는 거의 전무한 실정이므로 건강검진후의 사후관리를 강화하고 형식적인 노인건강검진을 지양하여 상담과 보건교육이 적절히 이루어지는 건강검진이 건강관련행위 변화에 중요한 역할을 하고 있음을 보여주는 연구결과이다.

This study was done to evaluate the effectiveness of appropriate consultations and continuing health education after free health screening in promotion and maintenance of health in elderly people.

Data on 224 elderly people over 65 years, who visited B public health center in B city, Gyeonggi Province, Korea, were obtained via individual interviews.

Each person was interviewed using a questionnaire that consisted of 61 questions. Of these, 11 questions were on “general characteristics”- sex, age, marriage, and SES, 3 were on “whether they were aware of the importance of health”, 12 were on whether they were satisfied with free health screening and 35 questions were related to measuring the degree of self management for health promotion.

The data were analyzed using the SAS (Statistical Analysis System) program with percentiles, means, t-test, ANOVA and paired t-test.

The results of the study are as follows.

First, before the health screening, the overall average score on actual health promotion behavior was 2.58 on a scale of 1-4. The highest score was obtained in the area of ‘nutrition’ (average 2.83) and the lowest in the area of ‘stress management’ (average 2.27).

Second, after health screening, the actual health promotion behavior score was 2.73 on a scale of 1-4. The highest score was in the area of ‘nutrition’ (average 2.93) and the lowest in the area of ‘stress management’ (average 2.36).

Third, health promotion behavior variables increased 5.03 points after the health screening, with exercise and responsibility for health showing the biggest differences and stress management the lowest.

Fourth, general characteristics of elderly people did not have any significant correlation with health promotion behaviors. However, the subscale ‘exercise’ in health promotion behavior, showed a significant differences according to sex (t=2.12, p<0.0349) and age (t=3,05, p<0.0493).

Free health screening has been the only preventive action offered by the public health care program for health promotion and maintenance for elderly people, but it is not effective in promoting their health. The results of this study suggest that appropriate consultations and continuing health education after health screening can be effective complements to free health screening. In conclusion, it is necessary to provide appropriate consultations and continuing health education, as well as free health screening if the health promotion behaviors of elderly people are to be changed.
