본 연구는 16개 시,도 교육청에서 시달되는 학교보건기본방향을 분석하고, 전국 초등학교 보건교사를 대상으로 현재 수행하고 있는 학교보건업무를 분석하여 보건교사 업무개선 및 업무표준을 위한 기초 자료를 제공하고자 한 기술적 연구이다.

연구대상은 전국 초등학교 보건교사 중 임시교사와 신규발령자를 제외한 4016명을 대상으로 2003년 4월 14일부터 4월 26일까지 인터넷을 통해서 자료를 수집하였고, 설문에 응한 766명중 부적절하다고 판단한 17명을 제외한 749명을 대상으로 SPSS 통계 프로그램으로 분석하였다. 연구결과를 요약하면 다음과 같다.

1. 16개 시,도 2002 학년도 학교보건기본방향을 분석한 결과 지역별로 내용과 업무수행 방법에 차이가 있었다.

가. 내용면에서 학교보건업무 및 교육청 보고서식만 간략히 기술한 서울, 경남, 강원, 전남, 대구 등이 있는 반면, 학교보건업무 수행방법 및 근거자료, 보건업무 관련 서식, 학생질병관리요령 등 상세한 내용을 수록한 제주, 대전, 광주 지역이 있었다.

나. 업무수행 방법에서는 보건교사 배치율은 2002년 4월 1일 기준 서울이 98.15%로 가장 높고, 제주가 43.27%로 가장 낮아 지역별 차이가 많았다. 순회근무를 겸임으로 발령하는 지역으로 대전, 제주, 충북이 있었으며 순회근무를 명시한 지역 중 가장 많이 하는 곳으로 충북 주2회, 울산 주 1-2회였다. 신체검사 방법 중 제주에서는 4학년을 대상으로 종합검진을 실시하고 있었으며, 보건교사가 실시하는 보건수업시수를 정해서 시달한 지역은 서울, 대구, 경북이었다. 학교환경관리업무 중 학교환경위생정화구역 및 학교주변 유해환경정비는 부산, 대구, 대전, 울산 지역에서는 별도 공문으로 분리되어 시달되었다.

2. 전국 초등학교 보건교사가 현재 수행하고 있는 학교보건업무를 분석한 결과 대부분의 영역에서 학교당 학생수에 따라 업무내용의 차이가 있었다.

가. 체격검사방법으로 학생수가 적을수록 체격검사 항목의 일부 또는 전 항목을 보건교사가 실시하는 경우가 높았고, 학생수가 많을수록 담임교사가 실시하고 보건교사가 관리하는 경우가 높았다.

나. 체질검사 방법으로 학생수가 적을수록 건강기록부에 제시된 체질검사 항목의 일부 또는 전 항목을 교의가 실시하는 경우가 높았고, 학생수가 많을수록 보건교사가 1차 스크린 후 교의가 실시하는 경우가 높았다.

다. 학교임의 단체예방 접종, 불소용액양치사업, 학급당 연간 총 보건교육시간은 학생수가 적을수록 실시율이 높았다.

라. 학교환경관리, 학교에서 응급사고 발생시 환자후송담당, 건강기록부 전산입력, 안전공제업무, 화장실관리 등은 학생수가 적을수록 보건교사가 수행하는 경우가 높았다.

마. 상담 및 처치를 위한 1일 보건실 방문자수, 개인교육 및 상담인원수 등은 학생수가 많을수록 높았다.

바. 보건실 면적 및 시설 등 보건실 환경은 학생수가 적을수록 열악하였다.

이로써 학생수가 적을수록 학교보건의 대상자로서는 학교보건서비스 특히, 건강검진, 보건교육, 상담 등에서 질적․양적으로 더 많이 받는 것으로 나왔으나, 제공자로서 보건교사는 고유의 보건업무 이외의 시설관리, 행정적인 업무, 환경정화 관련업무 등 타 부서 업무를 하는 경우가 많았고, 보건실 환경 또한 열악함을 알 수 있었다. 학생수가 많은 경우에는 대상자가 많아져 보건실 관리업무 등 보건업무 증가로 인하여, 학교보건대상자가 받는 보건교육 및 상담의 횟수가 줄었으며, 건강검진방법 등이 미흡함을 알 수 있다.

이상의 연구 결과를 고려할 때, 학생수별 학교보건업무 표준화 지침을 마련하여야 하며, 보건교사가 보건업무 이외 시설관리, 행정적인 업무, 환경정화관리 등 타 부서 업무를 하는 것을 배제하여 보건교사업무에 충실할 수 있도록 하여야겠다. 또한 전국 모든 학교에 보건교사 배치 및 초,중등 교육법시행령에 근거하여 43학급이상의 초,중,고등학교에 교감 2인을 두는 것과 같이 학생수가 1501명 이상인 경우 1교 2인의 보건교사를 배치하여야겠다. 보건교사가 학교보건업무를 효율적이고 체계적으로 계획,수행 및 평가를 하기 위해 장학지도 및 감독을 할 학교보건전문가로 보건교사 출신의 보건전문직(장학사)이 16개 시,도 및 각 지역교육청에 1인 이상 배치되어야겠다.

The purpose of the research was to analyze the direction of school heath systems and school health services. The directions, which were used in this research, were instructions from Departments of Education in major cities and provinces (n=16), and school health services are current job descriptions for elementary school health teachers. Basic data has been obtained to support the need for a standard for health teachers. Suggestions are also made for improvements in the way services are structured. For this study, a sample of 749 teachers from 4016 elementary school health teachers was selected through the internet from April 14, 2003 to April 26, 2003.. Temporary teachers and new teachers were not included. Of the 749, 17 were dropped, as data were not inappropriate. Analysis was done with the SPSS program. A summary of the research is as following.

1. There were differences in the instructions and working methods for the 2002 school year directions for the school health system.

A. Instruction Method: The Departments of Education in Seoul, Kyungnam, Kangwon, Junnam, Taegu and a few other provinces gave a brief description for school health services and for reporting to the Department of Education but in Jeju, Taejun, and Kwangju they gave specific instructions including methods for school health services, documentation forms for health services, and methods of case management for students.

B. Working method: The rate for assignment of school health teachers was different by region. The highest assignment rate for school health teachers was 98.15 % for Seoul and the lowest was 43.27 % for Jeju. The regions that had rotation work to additional posts were Taejun, Jeju, and Choogpook. In some regions rotation work was the rule. The regions that most frequently had additional positions were Choongpook for twice a week and Woolsan for once or twice a week.

C. Physical examinations: The Jeju Department of Education gave fourth graders comprehensive medical testing. The Departments of Education in Seoul, Taegu, and Kyungpook stated that the number of classes for health education by school health teachers was set. For school environment management, Pusan, Taegu, Taejun, and Woolsan had instructions given separately as official documents for sanitary clean areas in schools and consolidation of toxic environments near schools.

2. Analysis of school health services managed by current elementary school health teachers showed that there were differences in the types of work according to the number of students in the school.

A. For examination of physique, school health teachers generally performed all or parts of the examination in schools with smaller numbers of students. For schools with large numbers of students, the class teacher performed the examination and the health teacher managed it.

B. For health examinations, school health teachers usually did each item in the examination in schools where enrollment was small. For schools with a large enrollment, the school health teacher screened the students and then the school doctor examined the test.

C. When the number of students was small, rates for preventive innoculation, fluorine tooth brushing and annual health classes for each class were high.

D. When the number of students was small, health teachers often worked on school environment control, emergency patient transport, documentation of student health, safety control, and management of cleaning rest rooms.

E. The number of students visiting the dispensary per day for consultation and primary care, or personal education, and the number of consultants were high when there was a large enrollment.

F. Environmental conditions for dispensaries, such as size and facilities, were inferior in schools where the number of students was small.

Therefore, if the enrollment of a school is small, each student in the school receives better health services, such as health examination, health education and consultation compared to students in larger schools. However, health teachers usually have extra work besides health care and also have to work in the dispensary that is inferior to thse ones in larger schools. In larger schools, the number of service recipients is larger. Therefore the health management job is increased, and health services for students, such as physical examinations. are insufficient.

Considering the results of this research, there is a need for a standard for school health according to the number of students. It is also necessary for health teachers to concentrate on school health and be excluded from duties other than school health. Each school in the country must have a health teacher and if there are more than 1501 students, two health teachers are needed. This is similar to the requirements of the educational law for schools with more than 43 classes to have two acting principles. More than one expert in health education, should be appointed to each Department of Education as a school commissioner to support health teachers in planning, performing and evaluating health care effectively.
