한국표준의료행위 분류체계 개발

최근 대한의사협회의 주관으로 32개 의학 분과학회가 참가하여 한국표준의료행위용어를 개발하고 있다. 그러나 작업이 분과학회 별로 진행되었기 때문에 한국표준의료행위용어는 아직 뚜렷한 분류체계를 갖추지 뭇하고 있다. 따라서 본 논문에서는 의료행위 분류체계가 1) 과학성 2) 유용성 3) 규범성의 세 가지 조건을 갖추어야 할 것을 제시하고 이를바탕으로 한국표준의료행위분류체계를 개발하였다.

연구의 방법은 다음과 같다. 첫째, 화인스타인(Feinstein, 1988)이 제시한 과학적 분류체계의 조건을 이용하여 한국, 미국, 대만, 일본, WHO의 의료행위 분류체계를 비교하고그 구성원리와 틀을 도출하였다. 둘째, 위의 틀과 구성원리에 의료행위의 규범적 원칙을적용하여 분류체계의 방향을 설정하였다. 셋째, 위의 분류체계의 방향을 바탕으로 한국표준의료행위 분류체계를 개발하였다. 분류체계가 갖추어야 할 조건 중 유용성은 그 구체적인 기준을 설정하기 어려워 본 논문에서는 분석의 대상으로 삼지 못하였다.

연구결과는 다음과 같다. 한국표준의료행위 분류체재의 전체적인 틀은 '예방-치료-재활'이라는 포괄적 보건의료(comprehensive health care)의 모습이 드러날 수 있도록 구성하였다. 대분류체계는 Ⅰ. 예방 및 건강증진, Ⅱ. 진찰 및 관리, Ⅲ. 검사, Ⅳ. 내시경, Ⅴ. 치료 및 처치, Ⅵ. 재활 등 여섯 장으로 구성되어 있다. 중분류체계는 Ⅰ. 예방 및 건강증진은 예방상담 및 위험요인중재, 예방접종/예방적 화학요법, 선별검사, 지역사회 예방의료 등으로 세분하고 Ⅲ. 검사 부분은 기능 검사, 영상 검사, 병리 검사, 생검 및 검체채취 등으로 세분하고 Ⅴ. 치료 및 처치부분온 정신치료, 비관혈적 치료, 관혈적 치료, 마취, 치료방사선 등으로 세분하였다. 위의 분류 중에서 기능검사, 생검 및 검체채취,내시경, 관혈적 치료 부분은 해부학적 체계를 이용하여 분류체계를 구성하였으며 영상검사, 병리검사, 마취, 치료방사선 등 다른 분과와의 중복 의료행위가 적은 분과는 되도록

그 분과의 분류체계를 그대로 수용하였다.

본 논문에서 제안한 분류체계는 앞으로 군집분석 등의 방법을 이용하여 전문가의 합의를 모아 보완할 수 있을 것으로 생각한다.

In recent years, the Korean Medical Association has undertaken the feat of establishing the Korean Standard Terminology of Medical Procedures with the dedicated help of 32 medical academic societies. However, because the project is being conducted by several different circles, it has yet to see a clear system of classification. This thesis, therefore, proposes the three principles of scientific properties, usefulness and ideology as the basis for classification system and has developed the Classification System of Medical Procedures in Korea upon their

foundation.

The methodology and organization of this thesis as follows. First, by adopting scientific classification system of Feinstein(1988), an analysis of the classification systems of the medical procedures in the United States, Japan, Taiwan, WHO was carried out to reveal the framework and the basic principles in

each system. Second, the direction of classification system has been constructed by applying the normative principle of medical field in order to show the future direction of the medical field and realize its ideology. Third, a finalized framework for the classification system will be presented as based on the direction

of classification system. Of the three basis principles mentioned above, the analysis on the principles of usefulness was left out of this thesis due to the difficulty of establishing specific standards of analysis.

The results of the study are as follows. The overall structure of the thesis is aimed at showing the "Prevention-Therapy-Rehabilitation" quality of compehensive health care and consists of six chapters;

Ⅰ. Prevention and Health Promotion,

Ⅱ. Evaluation and Management,

Ⅲ. Diagnostic Procedures,

Ⅳ. Endoscopy,

Ⅴ. Therapeutic Procedures,

Ⅵ. Rehabilitation.

Chapter three Diagnostic Procedures is divided into four parts ; Functional Diagnosis, Visual Diagnosis, Pathological Diagnosis, Biopsy and Sampling. Chapter five Therapeutic Procedures is divided into Psychiatry, Non Invasive Therapy, Invasive Therapy, Anaesthesia and Radiation Oncology, Of these sub-divisions, Functional Diagnosis, Biopsy and Sampling, Endoscopy and Invasive Therapy employs the anatomical system of classification. On the other hand, Visual Diagnosis, Pathological Diagnosis, Anesthesia and Diagnostic Radiology, namely those 야visions in which there is little or no overlapping in services with other divisions, used the classification system of its own division.

The classification system introduced in this thesis can be further supplemented through the use of the cluster analysis by incorporating the advice and assistance of other specialists.
