델파이기법을 이용한 병원정보시스템 관리에서의 주요 이슈에 관한 연구

우리 나라와 같이 전산화가 아직 초보적인 단계에 있고, 최고 경영자의 컴퓨터에 대한 지식이 거의 없는 경우에는 전략적 의사결정을 담당하는 전산책임자의 역할은 성공적인 전산화에 중요한 역할을 하고 있으며 병원정보 시스템 개발 및 운영의 과정에서 어려운

결정에 자주 직면하게 된다.

이에 본 연구에서는 델파이기법을 이용하여 서울 및 수도권 지역의 500병상 이상의 계장직급 이상의 전산운영자를 대상으로 2회의 우편설문과 전화면담을 실시하여 병원 정보시스템을 개발 및 관리, 운영에 있어서의 주요 관리이슈를 조사하였으며, 주요 관리이슈의 발생원인과 순위결정요인을 분석하여 향후 3-5년의 병원정보시스템의 관리모형을 제시하였다.

델파이기법의 설문 중단시점의 근거로 제시되는 안정도는 변이계수와 T-test를 이용하여 조사하였고 그 결과 변이계수는 모든 항목에서 0.5이하로 안정적으로 분석되었으며 1차 설문에 비해 많이 감소한 것으로 나타났다. T-test는 5개 문항에서 유의적 차이가 있다고 분석되었으나 평균에 근접하는 특성과 원래의 점수를 유지하려는 두 가지의 특성을 가진 델파이기법의 특성을 고려하여 안정도에 만족한다고 분석되었다.

향후 3-5년의 성공적인 병원정보시스템의 주요 관리이슈는 '최고 경영자의 지원'으로 조사되었고, 정보통신망 운영, 관리정보자원의 보안 통제, 사용자 부서와의 유기적 협조체제, 인적자원의 고용 및 개발, 전자의무기록의 법제화, 시스템 운영을 위한 사용자 교육, 데이터자원의 효율적 운영, 환자어보 시스템의 구축, 정보시스템 조직의 병원조직의 연계 등의 순으로 병원정보시스템의 주요 관리이슈가 도출되었다.

조사 대상의 특성별분류에 의한 분석은 직급별, 지역적 특성, 설립형태별, 설립시기별 등으로 이루어 졌으며 그 겨로가 상위 직급일수록 '최고 경영자의 지원'에 더욱 높은 관심을 나타냈으며, 이와 함께 의사결정에 영향을 미치는 '테이터자원의 효율적 운영'도 높게 나타났다. 지역적 특성은 서울 지역이 수도권 지역의 병원에 비해 정보시스템의 시행율이 낮게 나타낫으며 이러한 이유로 '사용자부서와 유기적 협조' 및 '통신망의 운영 및 관리'가 높게 나타났다. 설립주체에 따라서는 비대학병원이 대학병원에 비하여 병원정보시스템의 시스템의 시행율이 낮게 나타났으며. '사용자 부서와의 유기적 협조체제'가 높게 나타났다. 병원설립시기에 따라서는 전자와 같은 양상으로 1990년 이전에 설립한 병원이 병원정보시스템의 시행율이 낮게 나타났고, '사용자 부서와의 유기적 협조체제'가 높게 나타났다.

이상에서와 같이 성공적인 병원 정보시스템의 개발을 위해서는 최고 경영자의 이해 도모가 급선무임을 알 수 있었고 이와 함께 계획성 있는 정보망의 확충과 재정비를 통해, 의료 데이터를 디지털화하여 효율적으로 활용하며 이러한 데이터의 보안에 역점을 두어야 함을 알 수 있었다.

본 논문의 기대효과로는 본 연구를 통해 주요 관리이슈의 변화를 파악하고 분석하여 향후 3-5년의 병원정보시스템의 발정방향을 제시하며, 이를 통해 중장기적 시점에서 병원정

보시스템을 예측하여 성공적인 병원정보시스템을 개발하고운영하는데 이용할 수 있을 것이다.

Informations System(IS) manager assumes a big role in developing a successful hospital management information system(HMIS), and often faces with the complex decision making in the process of managing HMIS since HMIS is still an early stage and many of hospital administrators don't have sufficient understanding about the system.

This study was conducted to identify the managerial issues of development of HMIS using Delpya technique. The survey data were collected from IS managers who manage more than 500-bed hospital in Seoul metropolitan area y interviewing twice through phone and mail. Stability of the survey based on the Delpya method was tested by the coefficient of variations(C.V.) and t-test. The survey was found to be stable and was stopped after the second round because C.V. fell below 0.5 and significantly lower mean scores were found among five items in the second survey.

Top 10 Major issues for the management of HMIS in three to five years were:

top management support, network management, security and control, close relationship with users, human resource management, legalizing the computer-based Patient Record(CPR), facilitation of organizational learning, efficient management

of educational data resource, development of patient-orient information system(IS), integration of IS organization and other hospital organizations.

These issues were also categorized according to the hospital characteristics in terms of rank of respondents, geographic regions, types of establishment, and year of establishment. Respondents in the higher ranks gave more weight on 'top

management support' and 'efficient management of data resources. Use of HMIS was lower for the hospitals in Seoul than those for the hospitals in the Metropolitan area excluding Seoul. And 'close relationshop with users' was selected as the most inportant issue for the hospitals in Seoul and non-teaching hospitals.

Similar results were also found from the non-teaching hospitals and older hospitals (established bofore 1990).

Expectation of this thesis paper can be on analyzing and understanding main management issue, presenting ways of development of HMIS, and estimating management of successful HMIS.
