노인의료복지시설에서 발생할 수 있는 윤리적 딜레마 상황에 처했을 때 간호사들은 윤리적 의사결정을 내리는 훈련과 준비가 부족하여 어려움을 겪게 되곤 한다.그럼에도 불구하고 우리나라 노인 간호 관련 교육과정은 법적·윤리적 문제 영역에 대해서 제한적이며 개괄적 언급에 그치고 있는 상황이다. 게다가 윤리적 딜레마를 대처하기 위해서는 창의적이고 비판적인 사고력을 함양할 수 있는 계속적인 교육방법이 요구되는데, 그 가장 효과적인 학습방법 중 하나가 문제중심학습법(PBL)이라 할 수 있다. 따라서, 노인의료복지시설 임상간호사들을 중심으로 한 노인대상 간호 수행시 윤리적 딜레마 상황을 중심으로 한 실제적인 PBL 문제개발과 평가를 통하여 PBL학습의 궁극적인 목적이라 할 수 있는 실무대처능력 향상을 도모하기 위한 PBL모듈 개발의 기초를 마련하고자 시도하였다.본 연구는 문제개발 과정은 조연순 등(2003)의 PBL 문제개발 절차를 일부 수정 및 적용하여 2007년 9-10월에 걸쳐 서울, 경기 3개의 노인요양시설에서 근무 중인 간호사 6인과 두 차례 이상 아이디어 회의를 통해 실제임상사례를 수집하였으며, 국내외 문헌고찰 중에 제시된 사례와 판례 검색을 통해 알려진 사례를 통한 총 9건의 문제 중 6건을 추출하였고, 2007년 10월 31일부터 11월 20일까지 서울, 경기지역 10개의 노인의료복지시설 간호사를 대상으로 개발된 PBL문제의 평가를 위한 구조화된 설문지를 본 연구 참여에 동의한 자만을 상대로 이루어졌다. 수집된 51부의 설문자료는 부호화하고 SPSS 12.0 for Window program을 이용하여 빈도분석, T-test로 분석하였다.본 연구 결과는 다음과 같다.1. 노인의료복지시설 간호사와 관련된 간호관리학과 간호윤리학, 노인간호학의 학습목표 및 교육과정을 고려하여 개발될 문제가 윤리적 기본개념과 이론, 간호사의 법적책임과 윤리적 역할 및 입소노인의 권리와 연관되어야 함을 확인하였다.2. 질문지 방법을 이용하여 간호사들이 윤리적 딜레마상황에 대해 가지는 사전지식과 흥미를 파악한 결과 간호사의 법적책임으로 비밀유지의 의무, 주의의 의무 등의 간호사의 법적 의무와 윤리의 원칙인 자율성의 원리, 악행금지의 원리, 선행의 원리, 분배적 정의와 환자의 권리인 자기결정권과 생명권, 자유권에 관해 이해하고 있으며, 권리간의 갈등 및 법적책임, 상황에 대한 구체적인 대처방법에 관심이 높음을 발견하였다.3. 최근 기사, 논문 및 도서, 국내외 판례 검색, 노인의료복지시설 간호실무자와 아이디어회의를 통해 문제가능성을 검토하여 국외문제 1개, 임상문제 5개를 선정하였다.4. 선정한 문제에 대한 문제지도와 교육과정 지도를 그렸으며, 각 문제의 역할과 상황을 현재형으로 설정하였고, 시나리오 형태로 유인물을 통해 학습자인 간호사에게 제시하기로 하였다.5. 작성된 문제를 전문가 집단에서 내용 타당도를 검증하여 최종 6개의 문제를 개발하였다.6. 본 연구자는 김기경(2007)이 개발한 학습자용 PBL 문제평가도구를 6개의 문제에 적용하여 설문조사 한 결과 문제1의 평균점수는 4.02(SD=.473), 문제2는 3.94(SD=.560), 문제3은 4.19(.535), 문제4는 4.13(SD=.522), 문제5는 3.62(SD=.651), 문제6은 3.96(.554)로 나타났다.7. 윤리적 딜레마 상황에 대한 경험유무에 따른 대상자의 평균 평가 점수 간에는 유의한 차이가 나타나지 않았다.8. 윤리적 딜레마와 관련한 교육이 있다면 ‘참여하겠다’는 응답이 86.3%(44명)로 노인의료복지시설 간호사들이 윤리적 딜레마 관련 교육프로그램의 개발과 보급에 대한 요구도가 높은 것으로 나타났다.결론적으로 윤리적 딜레마 상황이 주어졌을 때 간호사는 자신이 실제 경험하였다고 하여도 그 대처방법이 지식, 태도, 기술적으로 모색이 되지 않았다면 반복적으로 유사 상황에 처한다고 해도 또 다시 딜레마를 경험하게 된다. 따라서 윤리적 딜레마 상황은 경험을 통해서 대처방법이 마련되는 것이 아님을 알 수 있다. 따라서 노인의료복지시설 종사자의 실질적인 업무환경에 대한 충분한 이해를 바탕으로 한 구체적인 윤리 교육 및 재교육을 통해 윤리적 딜레마 상황에서의 올바른 의사결정과 윤리적 딜레마 감소를 도모해야 하는 것이다. 이를 위해서 윤리교육 프로그램 개발 및 평가에 관한 향후 지속적인 연구가 이루어져야 할 것이며 노인의료복지시설 간호사들 간의 정기적 사례관리 집담회를 이용한 정보교류의 계기 활용하여 정보 공유의 계기를 마련하는 것이 고려되어야 할 것이다.

When nurses meet with dilemmatic situations in terms of ethics which may take place in medical welfare facilities for the aged, they are prone to experiencing difficulties, since they do not get ready for making ethical decisions because of no prior training provided.Nonetheless, the education programs related to the nursing of the old people supplied in most of the universities in our country have some fundamental restrictions in the area of legal and ethical matters, only referring to the general aspects. Moreover, it is required to supply a series of continuous educations which can help the participants cultivate the abilities to think in a creative and critical manner, in order fro them to cope with such ethical dilemmas. One of the most effective learning methods is to focus on Problem Based Learning (PBL). Accordingly, in this study, an attempt has been made for putting up a basis of developing a PBL module to enhance a capability relevantly reacting to the real situations, which is the ultimate objective of PBL, by way of development of actual issues in PBL and their assessments which are centered in the dilemmatic situations in terms of ethics, arising in the course of nursing the old, and facing the clinical nurses working in the medical welfare facilities for such people.In this study, the process of issue development has been established by partly modifying the procedure of issue development in PBL suggested by Cho Yeon-Soon et al (2003), in which more than 2 times meetings were arranged for gathering ideas including the actual clinical instances, with 6 nurses currently working in 3 nursing homes located in Seoul and Kyounggi province over the period of September and October, 2007. In addition, 6 were selected out of total 9 cases which had been gathered through the search of judicial precedents and the examination of both domestic and overseas literatures, and a structured questionnaire, which had been specially developed for assessing PBL issues, was distributed to and gathered from the nurse working in 10 of such medical welfare facilities situated in Seoul and Kyounggi province and having accepted such a survey, from October 31, 2007 to November 20. 51 sheets of the questionnaires collected were encoded and analyzed the frequency of answers as well as T-test by using SPSS 12.0 for Window program.The results of this study are as follows.1. It was identified that, in consideration of the learning objectives and school curriculums in such academic fields as nursing administration, nursing ethics and care for the aged in relation with the nurses in medical welfare facilities for the aged, the issues developed should be properly associated with the basic concept and theory of ethics, legal responsibilities and ethical roles of a nurse, and the rights of the old people utilizing such facilities.2. It has been found that, as a result of surveying the nurses’ preliminary knowledge and interests about the situation of ethical dilemma, they have a very good understanding about the legal obligations of a nurse like the duty of confidentiality, the duty of attention, and the ethical principles such as the principle of self-regulation, the principle of prohibiting from doing evil behaviors, the principle of good conduct, the distributive justice, and the rights of the patient such as the right of self-determination, the right of life and the right of freedom, and they also have high interest in conflicts between rights and legal responsibilities, and how to react to each situation.3. Out of both the research results for the latest news articles, papers and books, and local and foreign judicial cases respectively and the meetings with the nurses in medical welfare facilities for the old, careful reviews were made for potentiality each as an issue, and as a result, 1 was selected as a domestic issue and 5, as clinical ones.4. A issue map and a curriculum road-map were made for each issue chosen, of which the role and situation have been settled to be currently progressive. These were presented to the nurses, so to speak, the learners by printed matters in the shape of scenario.5. Such established issues were reviewed again by an expert group for the purpose of verifying the validity of its contents and developing 6 kinds of issue finally.6. The author made his own survey by applying the PBL issue assessing tool to those 6 issues, which was developed by Kim Kee-Kyoung (2007). As a result, the average point earned for Issue No. 1 recorded 4.02(SD=.473) No. 2, 3.94(SD=.560) No. 3, 4.19(.535) No. 4, 4.13(SD=.522) No. 5, 3.62(SD=.651) No. 6, 3.96(.554), respectively.7. There was no significant difference found among the average points of assessment obtained according to whether the research objects had experienced the situation of ethical dilemma or not.8. The survey result showed 86.3% (44 persons) of the nurses surveyed would join when a training program is given in reference to ethical dilemma, which proves the development and proliferation of a training program related to ethical dilemma is highly requested by the nurses working in medical welfare facilities for the old.In conclusion, when a situation of ethical dilemma is given, even though a nurse has already experienced such a situation before, he/she is subject to repeat experiencing another similar dilemma, unless he/she is well equipped with the knowledge, attitude and technological background to cope with. Accordingly, we found such ethical dilemma could not be resolved by once experiencing it only. Therefore, it is required to make a right decision in such a situation and thus reduce the burden of a possible ethical dilemma by way of providing concrete instruction of ethics and repeating it regularly, based on the full understanding of the substantial working environment facing the attendant in medical welfare facilities for the old. In order to do this, further continuous study must be made for developing and assessing educational program of ethics, and it should be considered providing an opportunity to share the information of case management by holding a regular conference among the nurses in medical welfare facilities for the old.
