Abstract
[한글]B형간염 바이러스(HBV) 감염은 전 세계적으로 발생빈도가 높을 뿐만 아니라 간경변, 간경화, 간암과 같은 만성 간 질환을 초래할 수 있다. B형간염의 감염은 임신 및 출산 중에 감염되는 주산기 수직감염이 주요 요인으로 밝혀졌다. 본 연구는 2002년 7월부터 2005년 12월까지 B형간염 표면항원(HBsAg) 양성 산모에게서 태어난 신생아들로 B형간염 수직감염 예방사업에 참여한 신생아들을 대상으로 하였다. 출생 후 B형간염 면역글로불린(HBIG)과 B형간염 백신을 접종하고, 생후 9~15개월에 사이에 HBsAg과 anti-HBs 검사를 완료하였다. 검사결과를 토대로 이들의 예방처치 효과에 영향을 미치는 요인을 분석하였다. 산모 연령, 국적, 거주지, HBeAg 유무, 신생아의 성별, 출생체중, 출생일, 접종일, 접종백신에 대한 정보를 이용할 수 있었다.예방처치에 성공한 경우(7,964명)와 실패한 경우(356명)인 8,320명의 정보를 가지고, propensity score matching을 통해 예방처치에 성공한 경우(1,587명)와 실패한 경우(356명)인 1,943명을 분석하였다.단변량 로지스틱 분석 결과 산모 HBeAg 가 양성인 경우 산모의 HBeAg 가 음성인 경우보다 예방처치에 실패할 상대위험도가 3.2(95%CI:2.5-4.1)로 나타났다. 혈장유래백신을 접종한 경우 유전자재조합백신을 접종한 경우 보다 실패할 상대위험도는 3.2(95%CI:2.3-4.6)로 나타났다. 이른시기에 3차접종을 한 경우 권장시기에 접종 한 경우 보다 예방처치에 실패할 상대위험도는 1.5(95%CI:1.1-1.9)였다.다변량 로지스틱 분석 결과는 백신종류와 접종시기의 변수에 대한 영향력을 통제한 상태에서 산모의 HBeAg 가 양성인 경우 산모의 HBeAg 가 음성인 경우 보다 예방처치에 실패할 상대위험도가 3.3(95%CI:2.6-4.2)으로 나타났다. 산모의 HBeAg 유무, 접종시기 변수에 대한 영향력을 통제한 상태에서 혈장유래백신으로 접종을 완료한 경우 유전자재조합백신으로 접종을 완료한 경우보다 실패할 상대위험도는 3.7(95%CI:2.5-5.3)로 나타났다. 산모의 HBeAg 유무, 접종시기 변수에 대한 영향력을 통제한 상태에서 교차접종으로 접종을 완료한 경우 유전자재조합백신으로 접종을 완료한 경우보다 실패할 상대위험도가 1.7(95%CI:1.1-2.7,p=0.0278)로 나타났다.본 연구 결과로부터 산모의 HBeAg 유무, 백신종류, 접종시기가 예방처치 효과에 영향을 미치는 요인으로 조사되었다. 이 중 산모의 HBeAg 유무와 백신종류가 예방처치 효과에 보다 큰 영향을 미쳤다.

[영문]Globally, hepatitis B virus(HBV) is a major cause of acute and chronic hepatitis. Age at the time of acute infection is the primary determinant of the risk of progressing to chronic HBV infection. More than 90% of infants infected perinatally are likely to develop chronic HBV infection. Between 25% and 50% of children infected between 1 and 5 years of age become chronically infected, whereas only 2% to 6% of acutely infected older become children and adults develop chronic HBV infections.This study had a aim to evaluate the efficacy of immunoprophylaxis in infants born to a HBsAg positive mother. The subjects were infants born to an HBsAg (hepatitis B surface antigen) positive mother from the registry the prevention program of HBV vertical transmission in Korea from July 2002 to December 2005. Infants were received hepatitis B immune globulin (HBIG) 0.5ml and 0.5ml hepatitis B vaccine at birth, followed by the remaining doses in the series and tested for HBsAg and anti-HBs at 9~15 month. This study matching had used propensity score matching analysis, the efficacy of immunoprophylaxis compared one preventing vertical transmission success group with the other preventing vertical transmission failure group.In univariate logistic regression analysis, a maternal HBeAg positive state, plasma-derived vaccines and a early in the third dose of vaccine were associated with increased risk of vertical hepatitis B viral infection. In multivariate logistic regression analysis, a maternal HBeAg positive state, plasma-derived vaccines and administration of different vaccine were associated with increased risk of vertical hepatitis B viral infection. From the findings, HBIG and a three-dose hepatitis B vaccination regimen were generally effective in preventing vertical transmission in infants born to HBsAg positive mothers. This study suggests that additional immunoprophylaxis are needed to prevent vertical transmission in infants born to HBeAg positive mothers.
