정신보건센터 서비스 효과성 분석

Abstract

[한글]최근 우리나라는 정신보건사업의 발전과 함께 이에 대한 효과성 평가가 중요한 시점으로 대두되고 있다. 보건사업에 대한 효과성 평가는 기존 사업 실행에 대한 타당성의 입증을 위해 필요할 뿐 아니라, 향후 정책 방향을 제안하는데 있어 매우 중요한 역할을 할 수 있다. 따라서 본 연구는 정신보건센터에서 제공되는 서비스의 효과성을 평가하고, 효과에 영향을 미치는 요인을 보다 자세히 분석함으로써 향후 보건사업의 실행 및 방향 설정에 도움이 되고자 하였다.본 연구는 성남시 정신보건센터에 등록하여 최종적으로 연구를 수행한 정신질환자 196명을 대상으로 하였다. 이들에 대해 정신보건서비스를 제공하기 전의 일상생활기능, 정신증상, 년간 입원력(기간, 횟수), 년간 노동수입을 조사하였고, 정신보건서비스를 제공 후 1년이 지난 시점에서 일상생활기능, 정신증상, 년간 입원력(기간, 횟수), 년간 노동수입을 조사하여 두가지 자료를 비교 분석하였다. 아울러 서비스 효과성에 대하여 어느 요인이 영향을 미쳤는가를 보기 위해 다중회귀분석을 시행하였다.연구 결과를 요약하면 다음과 같다.첫째, 성남시 정신보건센터에 등록된 정신질환자에 대한 정신보건서비스 제공 후 일상생활기능, 정신증상, 입원력, 노동수입에 있어 서비스 제공 전에 비하여 유의한 차이가 발생하였다.둘째, 정신보건서비스 제공의 질적 수준에 따라 효과성에 있어 유의한 차이가 발생하였다. 즉, 사례관리 서비스나 주간재활 서비스 혹은 직업 및 주거 서비스 제공 수준에 따라 일상생활기능, 정신증상, 입원력, 노동수입에 있어 유의한 차이를 보였다.셋째, 정신질환자의 개선효과에 영향을 주는 요인을 다중 회귀 분석하였다. 그 결과 사례관리 서비스는 일상생활기능과 노동수입의 개선효과에 영향을 미치는 요인으로, 직업 및 주거 서비스는 정신증상과 노동수입의 개선효과에 영향을 미치는 요인으로 나타났다. 한편, 연구시점의 일상생활기능, 정신증상, 입원 기왕력, 노동수입의 정도에 따라 각각 일상생활기능, 정신증상, 입원력, 노동수입의 개선효과에 있어 유의한 변화를 보였다.넷째, 정신보건서비스 제공에 따른 비용 편익 분석에서 연구에 참여한 196명의 정신질환자에 대한 순수 비용 절감액은 약 1억 8천 7백만원, 정신보건서비스 제공에 따른 비용 편익 비율(비용/편익)은 0.59로 산출되었다.이러한 결과를 종합할 때, 성남시 정신보건센터에서 제공된 정신보건서비스는 정신질환자의 삶에 긍정적 효과를 가져온 것으로 평가할 수 있다. 또한 정신보건서비스의 질적 수준은 정신질환자의 사회재활 효과에 영향을 미칠 수 있고, 정신보건서비스는 비용 경제적으로 효율적인 것으로 평가할 수 있다.본 연구는 현재 우리나라 정신보건센터에서 제공되는 서비스에 따른 효과성을 입증하였다는데 의의가 있다고 볼 수 있다. 특히 정신질환자가 정신보건센터에 등록한 시점으로 기반으로 하여 전향적 코호트 연구를 했다는 점에서 이전 연구들과 차별성이 있었다.본 연구 결과를 토대로 다음과 같은 제언을 하고자 한다. 첫째, 차기 연구에서는 보다 장기간에 걸친 정신보건서비스 효과성 연구가 진행되어야 할 것이다. 둘째, 본 연구에 포함되지 아니한 서비스(의료 서비스, 기타 보건복지 서비스 등)를 포괄적으로 고려한 연구가 필요하다. 셋째, 본 연구는 성남시 정신보건센터에 소속된 정신질환자만을 국한하여 연구한 결과로서 향후 연구에서는 다른 지역 정신보건센터에 소속된 정신질환자를 대상으로 한 연구가 필요하다. 넷째, 본 연구의 경제성 평가는 정신의료비용과 노동수익만을 고려한 결과를 토대로 이루어졌으므로, 향후 연구에서는 정신과 외 의료비용, 다양한 간접 비용, 심리사회적 비용과 같은 무형비용 등 보다 포괄적이며 세밀한 경제성 연구가 진행되어야 할 것이다.

[영문]Recently, there has been a development of care for the mentally ill people in the world. Now most developed countries changed their policy from hospital based care to community based care and rehabilitaion. In Korea, there has been a rapid development in community psychiatry. It is important to evaluate the effectiveness in community psychiatry for the evidence based rehabilitation and policy making. Therefore this study aims to evaulate effectiveness of mental health services and analyse the factors influencing on the effectivenss.196 service users that are suffering from mentally ill problems were participated in this study. They were all registered in Seongnam City Community Mental Health Center. And then the Center provided them with individualized services such as outreach services, vocational and residential services, day care programs for 1 year. Datas on service users' changes( general functioning, psychopathology, hospitalization, income) before and in 1 year after mental health services were collected. We compared 2 outcomes before and after community mental health services. And then we explored the factors affecting the effectivenss of this study using multiple regression analysis.The results of this study are as follows1) Participants improved over 1 year at all of effectiveness measures2) Participants that were provided by higher qualified services were more likely to be improved much3) General functioning improvement was influenced by case-mangement and baseline score of general fuctiongning. Psychopathology improvement was influenced by occupational and residential services and baseline severity of psychopathology. Hopsitalization were influenced by baseline history of prior hospitalizaion. Earned Income were influenced by occupational and residential services and baseline income.4) Community mental health services were proved to be cost-effective. Net benefit and cost-benefit ratio were calculated each as 187,000,000 won, 0.59.Consequently Seongnam community mental health services to mentally ill people were effective.In the future, the following sdudy plans are suggested.1) It is necessary to have a study on effectiveness related with longer duration of community mental health services.2) It is necessary to have a study considering more comprehensive services of community mental health center.3) It is necessary to have a study with larger sample size.4) In econonmic evalution, further study have to be more detail one considering indirect medical costs and intangible costs.
