전화상담 프로그램이 관상동맥 우회술환자의 불확실성, 자기 효능감, 자가간호행위에 미치는 효과

본 연구는 전화상담 프로그램이 관상동맥 우회술환자의 불확실성, 자기 효능감, 자가간호 행위에 미치는 효과를 분석하여 전화상담 프로그램이 퇴원 후 환자에게 효과적인 간호 중재인지 규명하기 위해 실시된 비동등성 대조군 전후 설계를 이용한 유사실험 연구이다. 연구기간은 2005년 3월 20일부터 2005년 6월 4일까지 경기도 소재의 B병원에서 관상동맥 우회술을 받고 퇴원하는 환자를 대상으로 실험군 10명, 대조군 11명으로 구성하였다.본 연구에 사용된 전화상담 프로그램은 경기도 B병원의 심장수술환자 교육책자에 실린 내용을 기본으로 구성하였으며 퇴원 1주 후부터 매주 1회 총 3회 연구자 1인이 각각 30분씩 시행되었다.불확실성은 Mishel(1988)이 개발한 질병에 대한 불확실성 측정도구로 이인혜(1985)가 번역하여 수정하고 연구자가 수정. 보완한 도구를 이용하였고, 자기 효능감은 sherer & Maddux(1982)가 개발한 일반적 자기 효능감과 송경자(2001)에 의해 수정. 보완된 구체적 자기 효능감 측정도구를 연구자가 수정. 보완한 도구를 사용하였으며, 자가간호 행위는 한애경(1998)이 개발하고 송경자(2001)에 의해 수정. 보완된 도구를 이용하여 측정하였다.자료분석은 SPSS/WIN 12.0 프로그램을 이용하여 비동등성 Chi-square test와 Mann-Whitney U test를 이용하여 분석하였다.연구결과는 다음과 같다.1) 일반적 특성과 질병관련 특성에서 실험군과 대조군의 통계적으로 유의한 차이가 없어 두 그룹간의 동질성이 확보되었다.2) 가설 검증 결과는 다음과 같다.① 제 1 가설; "전화상담프로그램을 제공 받은 관상동맥 우회술 환자는 대조군 보다 불확실성이 이 낮을 것이다" 는 실험군이 대조군 보다 불확실성 점수가 감소되어 가설 1이 지지되었다. (U=0.00, p=0.00).② 제 2 가설; " 전화상담 프로그램을 제공 받은 관상동맥 우회술 환자는 대조군 보다 자기 효능감점수가 더 높을 것이다" 는 투약 영역에서 실험군이 대조군 보다 자기 효능감이 증가하였으나 통계적으로 유의한 차이가 없었지만((U=37.0, p=0.17) 심리적 적응능력(U=4.0, p=0.00), 운동(U=6.0, p=0.00), 식이(U=4.0, p=0.00), 일상생활(U=6.0, p=0.00)의 영역과 총 자기 효능감 점수가 실험군이 대조군보다 통계적으로 유의한 점수의 증가를 보여 가설 2는 지지되었다. (U=1.0, p= 0.00)③ 제 3 가설; " 전화상담 프로그램을 제공 받은 관상동맥 우회술 환자는 대조군 보다 자가간호 행위 점수가 더 높을 것이다" 는 투약의 영역에서 실험군이 대조군보다 점수가 증가하였으나 통계적으로 유의한 차이가 없었지만(U=27.5, p=0.46), 추후관리(U=4.0, p=0.00), 식이(U=10.5, p=0.00), 운동(U=7.5, p=0.00)의 영역과 총 자가간호 행위점수가 실험군이 대조군보다 통계적으로 유의한 점수의 증가를 보여 가설 3이 지지되었다. (U=0.00, p= 0.00).이상과 같은 연구결과를 종합해 볼 때, 관상동맥 우회술 환자에게 전화상담 프로그램은 환자의 불확실성을 감소시키며 자기 효능감과 자가간호 행위를 증진시키는데 효과가 있으므로 관상동맥 우회술 환자의 간호중재로서 전화상담 프로그램을 적용할 수 있을 것으로 보이며 전문 간호사의 역할 확대에 기여할 것으로 보인다.

[영문]The purpose of the study was to determine the effectiveness of the post discharge telephone consulting program by a advanced practice nurse upon the uncertainty of the patients, the level of self efficiency and the ability of self care in the patients who have undergone coronary bypass graft surgery.The study design was based on the non-equivalent control group pretest- post test design.The subjects of the study consist of 21 patients who underwent CABG surgery at a university hospital in Geyonggi-Do, Korea from March 20 till June 4, 2005. the 21 patients were divided to the control group (11 patients) and the experimental group (10 patients).The telephone consulting program was to provide medical informations and instructions for the patients based on the hospital booklet, and psychologic support to assist the patients' recovery.The intervention began one week after the discharge of the patients and continued for 4 weeks with weekly telephone contacts.The data were analyzed with Chi-square test and Mann-Whitney U test using SPSS version 12.0 statistical program.The results are as follows;1. The fist hypothesis, " The experimental group which received the telephone consulting program will have a lower level of uncertain than the control group" was supported(U=0.00, p=0.00)2. The second hypothesis " The experimental group which received the telephone consulting program will have a higher level of self-efficiency than the control group" was supported(U=1.0, p=0.00)3. The third hypothesis " The experimental group which received the telephone consulting program will have a higher level of self-care behavior than the control group" was supported(U=0.00, p=0.00)In conclusion, it appears that the telephone consulting program by a advanced practice nurse improves uncertainty of the post operative patients, the level of self efficiency and the ability of self care of the coronary bypass graft patients during their recovery. This type of nursing intervention, i.e. post discharge telephone consulting program could be an important role of APN, expanding their future area.
