21세기는 ‘무한경쟁의 시대’로 급변하는 정치, 경제, 사회의 환경변화에 대해 적응하고 살아남기 위해서는 경쟁우위를 점할 수 있는 글로벌 경쟁력을 갖추어야 한다. 기업이 경쟁력을 확보하기 위해서는 상품 혹은 서비스의 질을 향상시키고, 비용을 줄이는 것뿐만 아니라, 고객만족을 위한 끊임없는 변화가 필요하다. 또한 기업 내에서도 최고경영자를 비롯한 전사원이 경쟁력을 키우며 기업의 모든 분야에서 혁신적인 개선활동을 수행하는 새로운 경영기법을 필요로 하고 있다. 이러한 경영기법이 6시그마 경영이다. 이에 이 연구는 6시그마기법을 적용하여 서울에 위치한 A전문종합병원에서 퇴원완료 후에 추가로 발생되는 병리조직검사관련 미수금 감소를 위하여 시행하였다. 이 연구에서 6시그마 기법의 여러 추진방법 중 제조분야에서 주로 사용하는 DMAIC(Define, Measure, Analyze, Improve, Control) 5단계방법을 적용하였으며, 2004년 1월부터 6월까지의 퇴원환자들을 대상으로 240부의 우편 및 방문 설문지 배부 후에 회수된 106부의 설문지를 대상으로 통계 분석한 결과 환자의 병세가 경미할수록, ‘치료 후 질병의 예후가 많이 호전될 것이다‘라고 응답한 경우일수록, 수납안내를 받은 경우일수록, 미수납을 모른사유가 입원진료비에 포함된 검사라고 생각해서와 추가조직검사를 했는지 몰라서라고 답한 경우일수록 퇴원 후 추가병리조직검사료에 대한 수납요구 시 병원에 대해 긍정적인 평가를 한 것으로 조사되었다. 반면 병원까지 소요되는 시간이 멀수록, 치료 후 예후가 ’호전되지 않을 것이다‘라고 대답한 경우일수록, 미수납을 몰랐던 사유가 추가검사결과를 알려주지 않아서라고 응답한 경우일수록 병원에 대해 부정적인 평가를 한 것으로 조사되었다.

다음 2004년 10월25일부터 11월 4일까지 파일럿 테스트를 시행하여 이 기간동안 발생한 총미수금액별, 건(명)별, 당일수술센터의 퇴원 후 추가병리조직검사 발생비용별, 건(명)별로 대조군과 각각 비교하였는데 접수지연은 대조군인 6월에 비해 13%가 감소된 것으로 조사되었고, 퇴원 후 추가병리조직검사 발생금액과 건은 1,429,844원(40건)으로 6월 대조군의 1,794,667원(94건)에 비해 20%의 발생금액 감소와 43%의 발생건수 감소를 보여 주었다. 특히, 당일수술센터의 발생금액과 건수에서 6월 대조군의 456,294원(42건)에 비해 140,526원(9건)으로 현격하게 감소된 것을 확인 할 수 있었다.

DMAIC의 마지막인 관리단계에서 해당되는 각 팀과 논의하여 현실적으로 적용가능한 부분과 2005년 3월 이후 차세대처방전달시스템에서 가능한 부분으로 나누어 퇴원 후 추가병리조직처방을 줄이고, 발생된 미수금에 대해서는 회수프로그램을 마련하여 손실비용을 줄이는 방안을 협의하였다. 추후 이상치가 발생할 경우 조치사항 등 관리지표를 주기적으로 피드백하고, 개선안 반영을 위해 관리도를 그려서 관리하도록 하였다.

이 연구는 제조업이나 기타 서비스업이 아닌 병원서비스업계에서도 6시그마기법을 적용하여 과학적이고 통계적인 시각에서 고객중심으로 고객불만족(결함)의 핵심요인을 파악한 뒤 비합리적인 프로세스개선을 통해 병원의 수익감소를 초래하는 요인을 제거 및 예방하고 효과적인 병원경영과 고객만족을 추구하려는 새로운 접근방법을 시도한 사례연구이다





[영문]With the rapidly changed hospital situation and compete with hospital, hospital need to develop and apply various management techniques. In this research, a case study was conducted on General Hospital A, based in Seoul. The research was aimed at reducing outstanding medical fees that patients incur by generating additional histopathological examinations after they are discharged from the hospital. The research used the six-sigma technique, an effective and useful tool, as well as a company-wide quality service innovation means designed to improve quality management. Specifically, the research used the five-step DMAIC (Define, Measure, Analyze, Improve, Control) method, mainly used in the manufacturing sector.

The research conducted a survey targeting 240 discharged patients by using mailed questionnaires and visiting them. Of these 240 questionnaires, 106 copies of answered questionnaire sheets, or 44%, were returned. An analysis of the responses found that the slighter the respondents'' diseases were, the more positive they were about the hospital''s requests for additional histopathological examinations done after they were discharged. This result showed a statistical significance. More of those who indicated that "the prognosis will improve much after treatment" were found to be positive about requests for fees on additional bioptic examinations after they were discharged.

The pilot test was conducted from October 25 to November 4, 2004. The corresponding results were compared with those of the control group according to the period''s total outstanding money and the number of corresponding cases, the day''s cost of the occurrence of additional histopathological examinations by the operation center after patients were discharged, and the number of corresponding cases. The delayed reception was reduced by about 13% in June, compared to the control group. Likewise, the cost amount for the occurrence of additional histopathological examinations after discharge, and the number of corresponding cases, were 1,429,844 won and 40 cases, respectively. These figures were down 20% and 43%, respectively, compared to the June control group''s 1,794,667 won and 94 cases. In particular, in terms of the cost amount for the day''s occurrence of examinations by the operation center, and the number of corresponding cases, the test group recorded 140,526 won and 9 cases, respectively, thus declining sharply compared to the June control group''s 456,294 won and 42 cases, respectively.

The control step, the last step, entailed discussing each related team and classifying the process into currently applicable portions and portions available with the next-generation prescription transmission system after March 2005. The step further endeavored to reduce additional histopathological prescriptions after patients are discharged, and discussed measures to prepare a program aimed at collecting medical fees incurred to reduce cost losses. Moreover, a management map was drawn up, reflecting improvement measures, to periodically provide feedback on necessary measures and management indices against the occurrence of abnormal cases.

The research applied the six-sigma quality improvement technique to hospital services, though the techniques normally employed in other sectors, such as manufacturing. Thus, from a scientific and statistical point of view, it sought to address faults or customer dissatisfaction based on customer views by identifying core cause factors and, consequently, improving the existing process. It thus attempted to remove and prevent factors that reduced the hospital''s revenues, and to ensure customer satisfaction.
