장기요양환자의 기능상태에 따른 재활치료 이용수준

우리나라의 노인인구는 세계 어떤 나라보다 빠른 속도로 증가하고 있으며, 그에 따라 일상생활에 장애를 갖는 노인 및 장기요양환자가 급증하고 있다. 이들에게 기능을 유지하고 회복시키는데 물리, 작업, 언어, 등 재활치료는 중요한 역할을 한다. 그리고 이러한 재활치료 프로그램은 노인의 기능상태에 적절하게 시행되어야 하며, 더불어 잘 시행되고 있는지를 파악하는 것도 매우 중요하다. 하지만 아직까지 국내에서 기능상태에 따라 적절한 재활치료가 시행되고 있는지에 대한 체계적인 연구들은 미비한 상태이다. 따라서 이 연구에서는 장기요양환자들의 기능상태와 재활치의 이용률 및 이용시간을 분석하여 장기요양환자의 기능상태에 따른 재활치료 이용수준의 차이를 분석하고자 하였다.

연구는 재활치료 서비스를 제공하고 있는 노인전문병원 7개 기관 환자 전수인 878명을 대상으로 하였으며, 조사기간은 2003년 1월 26일에서 3월 8일까지였다. 일상생활수행수준, 인지기능 등의 기능상태를 조사하여 그에 따른 재원일당 재활치료 이용률 및 이용시간을 개인단위로 분석하였다. 자료분석은 The SAS system for windows 버전 8.1을 이용하였고, 빈도분석, 분산분석, 로지스틱 회귀분석, 다중회귀분석을 시행하였다.

주요결과는 다음과 같다.

1. 연구에 참여한 7개 기관은 전국에 고루 분포하였고, 병상수는 76~256개의 규모였으며, 재활치료는 모두 물리치료사에 의해 제공되고 있었다. 대상자의 47.4%가 주 1회 이상 재활치료를 이용하고 있었고, 이용시간은 재원일당 9.4분이었다.

2. 낮은 일상생활수행수준/좋은 인지기능 군과 높은 일상생활수행수준/좋은 인지기능 군에서 재활치료 이용률 및 이용시간이 모두 높았다.

3. 일반적 특성과 기능상태에 따른 재활치료 이용률에 대한 분석 결과 다른 변수의 영향을 통제하였을 때 문제행동이 없는 군, 정신과 문제가 없는 군, 인지기능이 좋은 군에서 이용률이 높았으나, 일상생활수행수준은 이용률에 영향을 미치지 않았다.

4. 일반적 특성과 기능상태에 따른 재활치료 이용시간에 대한 분석 결과 다른 변수의 영향을 통제하였을 때, 일상생활수행능력과 인지기능이 좋은 군, 수의운동에 제한이 있는 군에서 치료이용시간이 많았다.

결론적으로 장기요양환자들의 재활치료 이용률 및 이용시간은 매우 낮았고, 일상생활수행능력 높고 인지기능이 좋은 군에서 오히려 재활치료이용수준이 높았다. 또한 재활치료에서 물리치료 외에 다른 치료 서비스는 찾아볼 수 없었다. 이러한 점들은 장기요양환자들에게 다양하고 체계적인 재활치료 서비스가 제공되어야 함에도 불구하고, 현실적으로 적절하게 운영되고 못하고 있음을 시사한다. 따라서 앞으로 장기요양환자에게 물리치료 뿐 만 아니라, 환자의 기능상태에 알맞은 다양한 재활치료 프로그램을 개발하고 활성화시켜야 한다. 동시에 재활치료의 다양한 효과를 증명하기 위해 많은 연구들이 필요하다고 생각한다.

[영문]Objective : The aim of this study was to investigate the utilization of rehabilitation therapy by functional status variables for patients in long-term care facilities.

Design and sampling: The design of the study is cross-sectional. The sample includes all residents in 7 long-term care facilities(n=878).

Methods : The functional status were examined by Resident Assessment Instrument for long-term Care Facility Version 2.0. The utilization of rehabilitation therapy was defined as total therapy minutes per diem for patients calculated using the data of self-reported task survey by physical therapists and rehabilitation utilization rate at least 1 time per day in the 7 days.

Results: Average total therapy minutes per diem was 9.1 minutes and rehabilitation utilization rate was 47.4%. The better the functions of activities of daily living(ADL) and cognitive performance function patients have, the more total therapy minutes per diem they need. And the better cognitive performance function, the more rehabilitation utilization rate they need.

Conclusions: The systemic and various rehabilitation therapy service by functional status in the long-term care facilities is very important. But the present result indicate that rehabilitation treatment wasn''t systemic and various because of the deficiency of expert, employee in rehabilitation area and effort developing rehabilitation treatment service by therapist. Therefore, it is necessary to supply and upbring the experts for the sufficient rehabilitation and to study the effectiveness of various rehabilitation treatment.
