소화기 내시경 간호표준 개발 및 간호수행도 조사

소화기계 질환 검사 중 소화기 내시경검사는 질환의 진단적 치료적 효용성이 광범위하게 적용되고 있고, 특히 소화기계 악성종양의 발견을 위한 전 세계적인 표준으로 소화기 내시경 검사가 제시됨으로써 발전된 내시경 검사에 대한 수요자의 요구가 높아지고 있다.

따라서 소화기 내시경 검사실 간호사는 모든 대상자가 최적의 간호를 받도록 하는데 책임이 있고, 이 목적을 달성하기 위해 간호사는 광범위한 지식을 적용하여 변화하는 의료계 내외의 요구에 효과적으로 부응하여야 하며 이를 위해 경험을 쌓고 적절한 간호능력을 유지하기 위한 계속교육들이 필요하다. 이 같은 간호활동은 전문 간호 실무를 위해 필요하며 이 간호실무의 준거로서 간호표준이 요구되는 것이다. 그러나 한국 소화기 내시경 간호의 경우 아직 지침이 될 만한 간호표준이 마련되어 있지 못한 실정이다.

따라서 본 연구에서는 소화기 내시경 간호활동을 기술하기 위해 문헌고찰을 기반으로 간호 활동을 추출하여 591개의 간호활동을 정리하고 대한 간호 협회의 간호표준을 기반으로 예비 소화기 내시경 간호표준을 작성하였다. 2004년 4월 19일 ~ 2004년 5월 3일까지 실시한 전문가 집단의 내용 타당도를 거쳐 최종 소화기 내시경 간호 표준을 개발하고 이를 바탕으로 6월 4일부터 7월 1일에 거쳐 50인의 내시경 간호경력 2년 이상의 간호사를 대상으로 소화기 내시경 간호 수행도를 조사하였다. 수집한 자료는 SPSS로 분석하였다. 연구결과를 요약하면 아래와 같다.

1. 예비 소화기 내시경 간호표준은 자료수집, 진단, 계획, 수행, 평가, 법/윤리 준수, 업무수행 평가, 교육, 연구, 협동, 자원 활용, 질 관리의 12개 표준과 업무표준에 대한 성취정도의 기대수준으로, 성취정도를 측정할 수 있는 타당하고 측정 가능한 요소인 기준 38개, 기준의 수행 성취 정도를 신뢰성이 있도록 정량적으로 나타낸 도구로, 간호의 질과 적정성을 감시하고 평가하기 위한 지침인 지표 145개, 구체적인 소화기 내시경 직․간접 간호 행위인 570개의 간호활동을 포함하여 개발하였다. 내용타당도 검증을 위한 내용타당도 지수는 각 기준 및 지표 문항에 대한 내용 타당도는 4점 척도를 이용하여, (1)전혀 타당하지 않음, (2)타당하지 않음, (3)타당함, (4)매우 타당함으로 표기하여 측정하였으며 그 결과를 가지고 각 문항에 대한 내용 타당도 지수(Index of Content Validity : CVI)를 산출하여 80% 이상의 CVI를 나타낸 문항을 선정되었는데, 예비소화기 내시경 간호표준 개발을 위한 전문가 내용 타당도 설문지의 경우 총 570개 항목 중 558개의 항목이 타당한 것으로 나타나서 타당도가 97.9%로 측정되었다.

최종 소화기 내시경 간호표준은 간호실무표준에 해당되는 5개 표준으로 자료수집, 진단, 계획, 수행, 평가와 전문직 수행표준에 해당하는 7개 표준으로 법윤리 준수, 업무수행 평가, 교육, 연구, 협동, 자원 활용, 질 향상의 총 12개 표준으로 구성되며 12개 표준, 38개 기준, 145개의 지표로 구성하고 지표의 하위수준으로 336개의 간호활동으로 구성되었다.



2. 개발된 소화기 내시경 간호표준을 이용하여 소화기 내시경 간호사의 간호표준별 수행도를 조사한 결과, 전체평균은 87.9%로 측정되었다. 표준별 평균 수행도를 보면 수행도가 높은 순으로 볼 때 ‘업무수행 평가’ 표준은 96.4%, ‘협동’표준에서 94.5%, ‘수행’표준의 경우 92.67%, ‘법/윤리 준수’표준에서 87.5%, ‘교육’표준에서 85.1%, ‘자원 활용’표준에서 84.5%, ‘자료수집’표준은 82.5%, ‘질 향상’표준에서 79.5%, ‘연구’표준에서 66.3%, ‘계획’표준에서 40.6%, ‘진단’표준에서 39.5%, ‘평가’표준에서 37.0%순으로 조사되었다.



3. 수행하지 못하는 경우 응답자의 대부분은 해당 업무가 병동간호사나 외래 간호사, 의사나 기타 타부서 직원이라고 응답하였으며, 그 외 병원내의 시설이나 지침 프로그램 미비를 원인으로 생각하였으나 실제 업무 수행에 있어 필요하다고 생각되나 수행하지 못하는 항목의 경우 시간부족을 원인으로 생각하는 경우가 많았다.

수행도 조사 결과 소화기 내시경 간호과정 적용부분의 간호수행도가 매우 낮게 나타났는데 이는 업무과중으로 인한 시간부족과 소화기 내시경 간호 분야의 체계적인 간호기록의 자료부족이 원인으로 생각된다.

그러므로 각 소화기 내시경관련 주된 간호 문제, 진단을 목록화하고 이에 따른 표준화된 간호 중재로 구성된 자료를 전산화할 경우 보다 체계적이고 표준화된 간호과정의 수행이 가능할 것으로 생각된다.

또한 소화기 내시경 간호행위의 주체가 불분명한 것으로 나타난 수행상의 장애는 각 기관의 운영상의 특성에 따라 다양하게 훈련된 소화기 내시경 간호사들의 문제로 생각되며, 소화기 내시경 간호학회를 중심으로 한 공론화된 업무규정이 있을 때 소화기 내시경 간호사의 업무와 책임을 효과적으로 수행 할 수 있게 하고 간호업무에 대한 책임 소재와 한계를 분명히 하여 간호사고로 인한 분쟁을 사전에 예방할 수 있을 것으로 생각된다.





[영문]Alimentary system endoscopy and its diagnostic and therapeutic efficiencies are widely applied to gastroenteric ailments. In particular, the evolvement of the alimentary system endoscopy as a global standard to find out malignant tumors in the alimentary system leads its consumers to take an increasing interest in it.

Thereupon, a nurse in charge of alimentary system endoscopy is responsible for those patients in question to receive appropriate nursing and to effectively meet the changing needs of the medical circles in order to realize that purpose. To this end, she necessitates to continuously receive education and training to acquire experience and maintain proper nursing capabilities. Such nursing activities are needed for specialized nursing practices, which requires a nursing standard as guidelines for nursing practices. However, such a nursing standard has not been thus far made as can serve as guidelines for nursing.

With this in view, this study focused on creating a preliminary alimentary system endoscopy nursing standard on based on the nursing standard of Korea Nursing Association by deriving and arranging 591 nursing activities from a review of the literature to describe alimentary system endoscopy nursing activities. And then a final alimentary system endoscopy nursing standard was made out through content validity of experts carried out from April 19, 2004 through May 3, 2004, on the basis of which a survey was conducted of alimentary system endoscopy nursing performance on fifty nurses with over two years of experience in alimentary system endoscopy. The data collected were analysed using SPSS. Results were obtained, as follows:

1. The preliminary alimentary system endoscopy nursing standard created for this study is a tool that can indicate quantitative values on nursing performance levels on 12 factors such as data collection, diagnosis, planning, performance, evaluation, abiding to the law or ethics, work performance appraisal, education, research, cooperation, resources utilization, and qualitative control, and 38 factors that can measure work performance levels. Furthermore, 145 indicators were developed to supervise nursing quality and aptitudes, along with 570 direct and indirect nursing activities for alimentary system endoscopy. Validity on each of the factors and indicator statements depended on a four-point scale: (1) ''not valid at all,'' (2) ''not valid,'' (3) ''valid,'' and (4) ''very valid.'' And then index of content validity (CVI) were calculated, of which statements showing DVIs above 80% were selected. Of the questionnaire poll of experts'' index of content validity, 558 items were found to be valid out of 570 in the aggregate, which shows a 97.9% validity.

The final alimentary system endoscopy nursing standard created for this study includes in itself 5 factors related to nursing practices and 7 factors related to data collection, diagnosis, planning, work performance, evaluation and profession work performance, and 12 factors related to abiding to the law and ethics, work performance appraisal, education, research, cooperation, resources utilization, and qualitative improvement. They consisted of 12 and 398 factors, and 45 index, and the subordinate level of the index consisted of 336 nursing activities.

2. As the result of having conducted a survey of alimentary system endoscopy nursing work performance by nursing factors using the alimentary system endoscopy nursing standard created for this study, the mean score showed 89.9%. The mean performance score by factors represented in order from the highest 94.5% in ''cooperation,'' 96.4% in ''work performance appraisal,'' 92.67% in ''work performance,'' 87.5% in ''abiding to the law and ethics,'' 85.1% in ''education,'' 84.5% in ''resources utilization,'' 82.5% in ''data collection,'' 79.5% in ''qualitative improvement,'' 66.3% in ''research,'' 40.6% in ''planning,'' 39.5% in ''diagnosis,'' and 37.0% in ''evaluation.''

3. Most of the respondents who did not respond gave such an answer as ''I am in charge of other duties than nursing because I am a doctor, a ward nurse, or an other department staff member.'' Furthermore, the reason why they were not able to perform the duties that were thought to be needed was attributed to ''insufficiency of hospital facilities'' or ''lack of programs'' at one time and at another to ''no time to do.''

The result of a survey of nursing work performance revealed a low level of performance in the application of alimentary system endoscopy to nursing practices, which may be ascribed to lack of time due to work overload as well as a shortage of data or lack of systematic nursing records in the alimentary system endoscopy field.

Therefore, it is expected that systematized and standardized records of nursing would be feasible when nursing problems related to alimentary system endoscopy are resolved and when computerization is realized of data obtained through standardized nursing intervention.

It seems that those obstacles arising from the obscurity of the alimentary system endoscopy nursing activity subjects are such problems as should be solved by those alimentary system endoscopy nurses who have been trained in varied ways in consideration of the characteristics of the institution they belong to. In conclusion, it is expected that complications arising from nursing accidents might be prevented only if a public work performance regulation is established centering around Alimentary System Endoscopy Nursing Association and when a clear limit is placed upon nursing work performance and liability along with leading alimentary system endoscopy nurses to perform their nursing duties and responsibilities more efficiently.
