자궁경부암 환자에서 유전자 염기 다형성의 유전-유전, 유전-환경 상호작용에 관한 연구

dc.description.abstract[한글] 자궁경부암은 우리나라 여성에서 위암에 이어 2번째로 발생률이 높은 암이다. 자궁경부암 발생은 인 유두종 바이러스 (human papilloma virus, HPV) 감염이 위험 요인으로 알려져 있다. 그러나, HPV에 감염된 사람의 5~10%만이 자궁경부암에 걸리므로 HPV 이외의 다른 요인들(cofactors)이 있을 것으로 생각된다. 이러한 요인들로는 흡연, 출산자녀수, 이른 첫 성교 연령 등의 생활습관요인과 유전요인을 들 수 있다. 이 연구의 목적은 한국 여성 자궁경부암과 관련있는 유전자 염기 다형성(SNP)을 찾고, 각 유전자들과 기존에 알려진 자궁경부암의 생활습관 요인과의 유전-환경상호작용, 기능적으로 관련이 있는 유전자간의 유전-유전 상호작용을 분석하는 것이다. 환자군은 2000 년 10 월부터 2001 년 12 월까지 강남 성모병원을 방문한 외래환자와 입원환자 중에서 Pap smear, punch biopsy, pathologic findings 등을 통해서 자궁경부암으로 환진된 749명이고, 대조군은 동일 기간에 가톨릭의과대학 건강검진에 참여한 대상자 중에서 Pap smear 결과 정상인 사람들 717명으로 총 연구대상자는 1,466명이다. 모든 연구 대상자에게는 임상 연구 참가 동의서를 사전에 받았으며, 생활습관요인을 위한 문진표 작성을 하였다. 또한, 혈액에서 추출한 DNA로 연쇄 중합 반응 (PCR)을 이용하여 p53 codon72, p21 codon31, FHIT codon98, RASSF1A codon 133, MTHFR C677T, GSTP1, IRF-1 intron 6, TNFA(-308, -238) 유전자의 다형성을 분석하였다. 통계분석방법은 Multiple Logistic Regression (MLR)을 이용하였으며, 연구대상자의 안정성을 평가하기 위하여 하디-와인버그 평형을 평가하였다. 모든 유전자 염기 다형성에서 하디-와인버그 평형을 만족하였다. 연구대상자의 평균 연령은 대조군이 46.1세, 자궁경부암(invasive cancer) 환자군이 50.3세로 환자군이 조금 높았다. 자궁경부암의 생활습관요인에 있어서 첫 성교 연령이 낮을수록, 교육수준이 낮을수록, 출산자녀수가 많을수록, 본인 흡연량이 많을수록, 남편의 흡연량이 많을수록 자궁경부암의 위험이 높았다. 유전요인분석에서 p53 codon72는 40세 이후에 진단된 자궁경부암 환자의 경우 비차비가 1.7(95% CI, 0.9-3.0)의 결과를 보여서 최근의 메타분석 결과와 일치하였으나 통계적으로 유의하지는 않았다. p21 serine allele 동형접합체의 경우에 선암과는 관련이 없었고, Cervical intraepithelial neoplasia(CIN) 2에서 비차비가 2.7(95% CI, 1.3-5.6)로 통계적으로 유의하게 높았다. FHIT codon98의 경우에는 진단연령이 40세 이하인 여성에서 비차비가 2.7(95% CI, 1.4-5.8), RASSF1A codon 133의 경우에는 3.0(95% CI, 1.5-5.9), MTHFR C677T의 경우에는 1.9(95% CI, 1.2-3.2)로 통계적으로도 유의한 결과를 보였다. 유전-환경 상호작용 분석에서는 진단연령이 40세 이하인 여성에서 유전자염기 다형성이 variant type이면서 첫 성교연령이 22세 이하인 여성이 wild type이면서 첫 성교연령이 23세 이상인 여성과 비교하여서 비차비가 6~8배의 분포를 보였다. 또한, MTHFR C677T와 출산력, GSTP1과 흡연간에도 복합효과(joint effect)를 보였다. 유전-유전 상호작용 분석에서는 FHIT와 MTHFR 간에는 두 유전자가 모두 variant type인 경우에 진단연령이 40세 이하인 편평상피 세포암에 대한 비차비가 2.4의 결과를 보였고 통계적으로도 유의하였다. 또한, multiplicative model에서의 interaction term도 p-value가 0.0530으로 marginally significant 결과를 보였다. P53과 MTHFR 간에는 두 유전자가 모두 variant type인 경우에 진단연령이 40세 이하인 편평상피 세포암에 대한 비차비가 3.4의 결과를 보였고 통계적으로도 유의하였다. 그러나, multiplicative model에서의 interaction term은 p-value가 0.1130으로 통계적으로 유의하지는 않았다. P53, MTHFR, 남편흡연의 3가지를 함께 분석한 결과에서는 진단연령이 40세 이하인 환자들에서 두 유전자 모두 variant homozygote이고 남편이 20개비 이상의 흡연자인 경우에 비차비는 5.1(95% CI, 1.8-14.3)의 결과를 보였다. MTHFR, RASSF1A, FHIT 등 주로 DNA methylation 관련 유전자들을 분석한 결과에서는 3가지 유전자 염기 다형성 모두 wild type homozygote인 여성들과 비교하여서 3가지 유전자 모두 varint type homozygote인 여성들이 40세 이전에 자궁경부암에 걸릴 위험은 4.3배 높았다. MTHFR C677T, FHIT, RASSF1A 등은 자궁경부암과 관련이 있었다. 이러한 관련성은 진단연령이 젊은 경우에 더욱 두드려졌다. 또한, 유전-유전 복합효과에 대한 결과에서는 MTHFR C677T와 종양억제 유전자, MTHFR C677T와 면역관련 유전자 간에 복합효과가 있었다. 본 연구결과 유전-유전, 유전-환경 복합효과를 보임으로서 Falconer의 polygenic threshold 이론과 일치하는 결과라고 할 수 있다. 앞으로 이 유전자 염기다형성에 대한 계속적인 역학적인 연구와 생물학적인 기전 연구가 필요하다. [영문]The risk factors for cervix cancer are early sexual exposure, multiple sexual partners, sexual contact with high risk males, and human papillomavirus (HPV). Even people exposed to the same environment exhibit varied susceptibility to a specific disease due to physiologic, environmental, and especially genetic characteristics; the latter known as one''s genetic susceptibility to a disease. The aim of this study was to examine the effect of p53, p21, FHIT, RASSF1A, MTHFR, GSTP1, IRF-1, TNFA on cervical carcinogenesis, and to identify gene-gene and gene-environmental factors affecting cervix carcinogenesis in Korean women. The study subjects were patients who were pathologically diagnosed with cervical neoplasia (n = 749) compared to normal healthy women as normal controls (n = 717). The single nucleotide polymorphisms (SNPs) of p53 codon 72, p21 codon 31, and interferon regulatory factor-1 (IRF-1) intron 6, FHIT codon98, RASSF1A codon133, MTHFR C677T, GSTP1, TNFA(-308, -238) were evaluated from extracted DNA of peripheral blood with an automatic DNA sequencer. The differences of each SNP, gene-gene and gene-environmental interactions between normal controls and patients were evaluated in the adjusted environmental background. The Hardy-Weinberg equilibrium calculation was performed for both controls and test cases, which all was consistent with the Hardy-Weinberg equilibrium. In the environmental aspect, the rate of cervix cancer increased in the women with a lower level of education, a younger age at first sexual intercourse and more childbearing. P53 codon 72 had a 1.7 (95% CI, 0.9-3.0) times highr risk of a late invasive cervical cancer when they were more than 40 years old at the time of diagnosis. P21 serine allele had a 2.7 (95% CI, 1.3-5.6) times higher risk of CIN2. In an early cervical cancer when they were less than 40 years at the time of diagnosis, MTHFR C677T had a 1.9 (95% CI, 1.2-3.2), FHIT codon98 had a 2.9 (95% CI, 1.4-5.8), and RASSF1A had a 3.0 (95% CI, 1.5-5.9) times higher risk. Women with two or more children and MTHFR TT had a 3.9 times (P=0.0078) higher risk of cervical cancer, compared to women with one or no children and MTHFR CC. The risk increased with GSTP1 AA among ever smokers compared with GSTP1 G allele positive among nonsmokers. The Women who had p53(Arg/Arg), MTHFR(TT), and a heavy smoker’s husband showed a 5.1-fold increased risk of cervix cancer compared to the women who had p53(~Pro), MTHFR(~CC), and non-smoker’s husband. The Women who had MTHFR(TT), FHIT(CC), and RASSF!A(GG) had a 4.3-fold increased risk of cervicx cancer compared to the women who had MTHFR(~CC), FHIT(TT), and RASSF1A(~TT). In conclusion, we found that the level of education, the age at first intercourse, and the number of children were independent risk factors in cervix carcinogenesis. Polymorphisms of MTHFR, FHIT, RASSF1A are associated with a higher risk of developing cervical cancer, and in particular for an early onset of cervical carcinogenesis. The specific combinations of p53, MTHFR, FHIT and RASSF1A gene-gene and gene-environmental interactions were significantly noted in the cervix carcinogenesis of Korean women.-
