모유수유와 소아비만과의 관련성

본 연구는 초등학교 학생들의 비만을 체질량 지수 분류별로 나누어 파악하고, 과거 모유수유 여부가 소아비만에 미치는 장기적인 영향에 대해 살펴보고자 하였다. 본 연구의 대상은 서울시 강동구 소재의 3개 공립 초등학교와 노원구 소재 1개 사립 초등학교 1학년을 대상으로 하였으며, 학생들의 인구, 사회학적인 변수와 생활습관 관련 변수, 수유방법, 수유 기간 등의 사항을 파악하기 위해 총 21문항의 설문지를 이용하여 조사하였다. 전체 연구대상자는 997명이며, 이중 학생의 신장과 체중을 이용할 수 없는 경우를 제외한 810명에 대해 분석을 실시하였다.

대상자들의 성별, 연령별 평균 신장(단위, centimeter)과 체중(단위, kilogram)은 남아 6세에서 120.6±5.2, 24.0±4.2, 7세에서 123.3±6.0, 24.8±4.1이고, 여아 6세에서 119.2±5.8, 22.6±3.7, 7세에서 122.0±6.3, 24.4±4.2이며, 평균 체질량 지수는 16.2±2.3이다. 1998년 대한 소아과학회에서 제시한 한국 소아의 체질량 지수 백분위수에 따르면 비만 위험군(85백분위수에서 94백분위수)는 남학생 53명(12.9%), 여학생 57명(14.3%)이며, 비만군(95백분위수 이상)은 남학생 48명(11.7%), 여학생 43명(10.8%)이다.

인구-사회학적인 특성 중 출생체중(단위, kilogram)은 정상군(3.24±0.43)에 비해 비만 위험군(3.37±0.49)에서 큰 것으로 나타났다(p=0.01). 부모의 체질량 지수는 정상군에 비해 비만 위험군, 비만군에서 높았으며(각각 p<0.001), 생활습관 관련요인에서는 정상군보다 비만 위험군, 비만군이 비활동적인 신체적 활동과 과식하는 식사습관이 상대적으로 많았다.

연구대상자중 인공수유(분유)를 한 경우는 295명(36.4%)이고, 모유수유를 한 경우가 515명(63.6%)이었다. 모유수유를 한 경우 모유수유기간이 1개월에서 6개월인 경우가 330명(64.1%), 7개월 이상인 경우가 185명(35.9%)이었다. 정상군, 비만 위험군, 비만군 발생에 있어 모유수유 여부(p=0.25)와 모유수유 기간(p=0.62)에 따른 관련은 의미가 없었다. 본 연구에서는 여학생에 있어 모유수유를 하지 않은 군에 비해 모유수유를 7개월 이상 한 경우 비만위험군의 교차비는 0.41(95% 신뢰구간 0.19-0.92)로 감소하였다. 그러나, 전체 학생들에 있어 인구, 사회학적 특성과 생활 관련 요인들을 통제하였을 때 모유수유를 하지 않은 군에 비해 모유수유를 한 군에서의 비만의 발생 감소는 통계적으로 유의하지는 않았다.

이상의 연구결과를 종합해 보면 과거 모유수유 유무와 기간에 따른 비만의 발생 감소를 확인할 수 없었다. 그러나, 모유수유의 소아비만에 대한 장기적 이점에 관한 연구는 보건학적으로 의료비 절감 등 파급 효과가 크기 때문에 필요하리라 본다. 앞으로 출생 코호트를 이용한 관찰연구를 통해 보다 정확한 연구가 필요하리라 본다.





[영문]The purpose of this study was to examine the prevalence of obesity based on body mass index data and to assess the long-term effect of breast-feeding on obesity among children at elementary school. A total of 997 children in the 1st grade were recruited from 3 public primary schools in Gangdong-gu and 1 private primary school in Nowon-gu, Seoul. A questionnaire was used for the measurement of demographic background and lifestyle-related information, infant feeding methods and duration. After excluding data with missing information, data from 810 subjects were analyzed for the study.

The mean height(unit, centimeter) and weight(unit, kilogram) were 120.6±5.2, 24.0±4.2 for 6 years-old boys, 123.3±6.0, 24.8±4.1 for 7 years-old boys, 119.2±5.8, 22.6±3.7 for 6 years-old girls and 122.0±6.3, 24.4±4.2 for 7 years-old girls. The mean body mass index was 16.2±2.3. According to the definition of BMI from the standard growth chart in the 1998 Korea Academy of Pediatrics, there were 53 boys (12.9%) and 57 girls(14.3%) identified as a high risk group for obese (BMI of 85 to 95 percentile) and 48 boys(11.7%) and 43 girls(10.8%) as a obese group (BMI of more than 95 percentile).    

The birth weight was higher in the high risk group than in the normal body mass index group. The parental body mass index was higher in the high risk group and the obese group than the normal group. In both the high risk group and the obese group, children were less active and ate more than the normal group.

Among subjects, 295 children (36.4%) were fed powdered formula milk and 515 subjects(63.6%) were breast-fed. Among 515 breast subjects, 330 subjects(64.1%) were breast-fed for 6 months or less while 185 subjects(35.9%) were breast-fed at least for 7 months. In all the study groups, the duration of breast-feeding, or whether the child was breast-fed or not had no association in this study. After adjusting for potential confounding factors, there seemed to be some risk reduction for obesity in the breast-fed group for at least 7 months, compared to the never breast-fed group, but had no statistical significance.

Overall, whether a child was breast-fed or not or the duration of breast-feeding did not seem to reduce the risk of developing obesity. But even if the effect of breast-feeding on future risk reduction of obesity is minimal, further researches on breast-feeding can have influences on the reduction of public health costs and further public health impacts. Future researches including birth cohort and investigational studies are demanded for better results on this subject.
