실험적 뇌지주막하 출혈에서 nimodipine이 대뇌피질 혈류에 미치는 영향

파열성 뇌동맥류는 자발성 뇌지주막하 출혈의 주된 원인이다. 파열성 뇌동맥류 환자에

서 과거에는 재출혈이 임상적으로 커다란 문제가 되어 왔으나 근자에는 뇌허혈성 결손이

사망 및 신경학적 장애의 주된 원인이다.

뇌지주막하 출혈 후 뇌혈액 순환장애는 뇌동맥 연축과 뇌부종 및 뇌척수액 순환장애로

인한 두개강내압상승등에 의하여 발생된다. 즉 뇌지주막하 출혈 환자에서 뇌혈류의 감소

에 따른 뇌허혈이 환자의 예후에 결정적 영향을 미치게 된다.

뇌혈관 연축의 기전은 아직 명백히 밝혀지지 않았으며 따라서 치료법도 온전히 정립되

지 않았다. 특히 뇌혈관 연축 시에 뇌혈액 순환을 호전시킬 수 있는 약물은 드물며 대부

분의 혈관확장제는 뇌혈관 보다 뇌 이외의 말초혈관에 더 강력하게 작용하는 것이 문제가

된다. 근자에 이르러 칼슘이온 생리가 밝혀짐에 따라 칼슘이온이 특히 생체내 평활근의

자극-수축 연계과정에 중요한 인자로 작용함이 밝혀짐에 따라 칼슘길항제가 임상적으로

여러 분야에 응용되고 있다.

본 연구에서는 실험동물 고양이 47마리를 투여약물의 종류, 투여량, 투여시기에 따라 9

군으로 나누고 각 군에서의 뇌혈류의 변화를 관찰하여 다음과 같은 결론을 얻었다.

1. 실험대조군에서 뇌지주막하 출혈 유발 전 평균 동맥압은 127.4±11.3mmHg, 심박동수

는 분당 145.8±11.4회, 뇌혈류는 49.2 ±9.3 ml/100 g/min이었으며, 뇌지주막하 출혈 유

발 후 평균 동맥압은 5.7-14.9% 상승하였고 심박동수는 7.0-19.7% 감소하였으며 뇌혈류는

47.3-73.2% 감소되었다.

2. Sodium nitroprusside의 투여 용량을 1.1-1 3μg/kg/min으로 하여 평균 동맥압을 뇌

지주막하출혈 유발 전의 평균동맥압보다 25∼30% 하강시킬 정도로 유지하였을 때, sodium

nitroprusside는 뇌혈류를 증가시키지 못하였다. 이는 sodium nitroprusside가 뇌혈관

보다 뇌 이외의 말초혈관에 더 큰 영향을 미친다는 것을 시사한다. 또한 sodium nitropru

sside를 뇌지주막하출혈 유발직후에 투여한 경우와 뇌혈류가 최저로 감소 된 후에 투여한

경우 양자간에 차이가 없는 것으로 보아 약물투여 시기는 뇌혈류 변화에 영향을 미치지

않는 것으로 생각되었다.

3. Nimodipine의 투여 용량을 증가시킬수록 평균동맥압과 심박동수는 감소되었으나 뇌

혈류는 약물의 용량을 1.0㎍/kg/min으로 했을 때 가장 많이 증가되었다. 그러나 약물의

용량을 2.0㎍/kg/min으로 했을 때 뇌혈류 증가효과는 없었으며 이는 뇌 이외의 말초혈관

에도 혈관 확장이 동시에 일어남으로서 평균동맥압이 하강되어 뇌혈류를 증가시키지 못한

것으로 생각된다. 따라서 nimodipine의 투여 용량을 1.0㎍/kg/min으로 하였을 때 nimodi

pine은 뇌 이외의 말초혈관보다 뇌혈관 화장에 더 큰 영향을 미치는 것으로 추정되었다.

Nimodipine의 투여시기는 뇌혈류에 영향을 미치지 않는 것으로 생각된다.

이상의 연구 결과로 보아 칼슘 길항제 nimodipin은 적정 용량을 투여할 때 뇌 이외의

말초혈관 보다 뇌혈관에 더 큰 영향을 미쳐 뇌혈류를 증가시킬 수 있는 것으로 생각된다.





The Effect of Nimodipine on Cerebral Cortical Blood Flow in Experimentally Induced

Subarachnoid Hemorrhage

A ruptured aneurysm is the major cause of spontaneous subarachnoid hemorrhage.

Rebleeding, which is the most dramatic and dreaded complication following aneurysm

rupture, was considered to be the major cause of morbidity and mortality in the

past. However, it has recently been demonstrated that the leading cause of death

and disability in patients with aneurysmal subarachnoid hemorrhage is cerebral

ischemia caused by cerebral vasospasm.

Reduced cerebral perfusion following decreased cerebral blood flow has been known

to be one of the major prognostic factors in the patients with subarachnoid

hemorrhage following ruptured aneurysm. A compound problem of cerebral ischemia due

to vasosprasm, cerebral edema and increased intracranial pressure has been widely

known to be a major cause of reduced cerebral perfusion secondary to subarachnoid

hemorrhage.

Clinical and experimental studies now available suggest that mechanical, chemical

and neurogenic factors are responsible for the occurrence and persistence of

cerebral vasospasm following subarachnoid hemorrhage.

Calcium ions play a major role in the regulation of cell functions. Especially,

final common pathways of vascular smooth muscle contraction depend on calcium ions.

In this report, the effect of the calcium antagonist nimodipine on feline

cerebral cortical blood flow was measured and compared with the effect of the

potent vascular smooth muscle relaxant, sodium nitroprusside, in experimentally

induced subarachnoid hemorrhage.

Cerebral cortical blood f]ole was measured in the middle cerebral arterial

territory by the hydrogen clearance method. Subarachnoid hemorrhage was induced in

47 cats by a slow injection of fresh autogenous arterial blood into the cisterna

manna. Foully-seven cats were divided into 9 groups of 5 or 6 cats each according

to the type of regimen, dosage and timing of administration.

The results of this study are summarized as follows:

1. Mean arterial blood pressure increased by 5.7% ∼14.9%, and cerebral blood

flow decreased by 47.3∼73.2% after induction of subarachnoid hemorrhage.

2. Intravenous infusion of sodium nitroprusside (1.1-1.3㎍/kg/min) decreased mean

arterial blood pressure by 25 ∼ 30%, and had no effect on the cerebral blood flow.

This result suggests that sodium nitroprusside has more effect on the peripheral

vessels than the cerebral vessels. The timing of administration of sodium

nitroprusside after induction of subarachnoid hemorrhage had no effect on the

cerbral blood flow.

3. Intravenous infusion of nimodipine had a greater decreasing effect on mean

arterial blood pressure and pulse rate according to the increase of dosage of

nimodipine. But cerebral blood flow increased most effectively with a dosage of

1.0㎍/kg/min in contrast to 0.5 or 2.0㎍/kg/min. Intravenous infusion of nimodipine

(1.0㎍/kg/min) decreased the mean arterial blood pressure by 12.6∼31.1% but

cerebral blood flow increased by 45.4-l04.7%. This suggests that nimodipine

(1.0㎍/kg/min) has a greater effect on the cerebral vessels than sodium

nitroprusside.

From the above results it is speculated that the effect of the calcium antagonist

nimodipine on feline cerebral vessels is greater than that on the extracranial

peripheral vessels, and nimodipine increases the feline cortical cerebral blood

flow in experimentally induced subarchnoid hemorrhage.
