관절연골손상후 능동적 운동이 치유과정에 미치는 영향에 관한 연구

관절연골은 sulfated점다당질과 교원질 섬유로 구성된 점착성의 기질에 관절연골세포가

분산되어 있는 특수한 결체조직으로 알려져 있다(Bennet등, 1932), 관절연골손상후 치유

과정에 관하여는 많은 논란과 더불어 연구의 대상이 되고 있으며 이에 대한 연구결과는

대략 다음의 3가지 가설로 요약할 수 있다. 즉 1) 손상된 관절연골의 치유는 일어나지 않

는다는 설(Hunter, 1743; Leidy, 1849; Kolliker, 1853; Paget, 1853; Maximow와 Bloom,

1957)과, 2) 손상의 치유는 일어나되 손상부 인접관절연골의 증식의 결과로 일어난다는

설(Redfern, 1851; Seggel, 1904; Fascioli, 1905)과, 3) 관절연골손상후의 치유는 손상

부의 기저부나 손상부주위의 연골하골부 및 활액막에서 유래된 결체조직이 초자연골로 화

생 (metaplasia)됨으로서 치유된다는 설(Fisher, 1923; Shands, 1931; Key, 1931; Bennet

t등, 1932; Carlson, 1957)등이다. 한편 관절연골손상후 치유과정에 대하여 여러 인자들

이 영향을 미치며 이중 손상의 깊이가 가장 중요하다고 하였고, 연골하골을 침해하지 않

는 표층손상은 치유되지 않으나 연골하골을 침해하는 심층손상은 골수에서 유래된 조직으

로 치유가 되며, 이는 이 조직이 설유연골이나 불완전한 초자연골로 변형됨으로서 치유가

된다고 인정되어 왔다(Meachim, 1963; DePalma등, 1966; Rosenberg, 1967; Campbell, 19

69: Ghadially 등, 1971).

한편 능동적 운동이 치유세포의 분화과정에 미치는 영향에 대하여도 많은 관심의 대상

이 되어왔으며, Smith-Petersen(1939)은 간헐적인 능동운동이 연골하골에시 유래된 세포

의 신생연골형상에 중요한 역할을 한다고 하였고, 그후 관절연골전층손상후 능동적 운동

이 손상부 치유과정을 자극한다는 여러 학자들의 보고가 있다(Hohl과 Luck, 1956; Kettun

en, 1963; Kettunen과 Rokkannen, 1973). 뿐만 아니라 Salter등(1980)도 관절연골전층손

상부의 치유에 있어서 계속적으로 수동적 운동을 시켰을 경우 관절의 고정을 시켰을 때나

간헐적인 능동운동을 시킨 경우보다 손상부의 치유가 조속하고 신생연골 형성이 좀 더

완전하다고 하였다. 한편 연골손상후 나타나는 퇴행성 변화에 대하여 Bennett과 Bauer(19

35) 및 Collins(1949)등은 관절연골손상이 퇴행성관절염의 시발점이 되며 그에 속발된 모

든 변화는 2차적이라 하여, 관절연골손상이 퇴행과정에 미치는 영향에 대하여 강조하였다

. Simmons와 Chrisman(1965)은 sodium salicylate가 관절연골세포의 기질인 chondromucop

rotein의 분해를 일으키는 이화효소, 특히 cathepsin의 작용을 억제하여 관절연골의 퇴행

과정을 방지한다고 최초로 보고하고 salicylate가 퇴행성 관절질환에 효과가 있음을 암시

했다. 그후 Chrisman과 Snook(1968)은 적당량의 salicylate가 관절연골의 퇴행성 변화에

대하여 절대적인 방지효과가 있다고 하였다. 이와 같은 보고를 종합하여, 본 연구에서는

성숙가토의 대퇴골 원위부 관절면에 전층손상을 만든후 능동적 운동이 손상부의 치유과정

에 미치는 영향과 salicylate처치가 주위관절연골부위의 퇴행과정에 미치는 영향을 규명

하고자 본 실험에 착수하였다.

실험재료 및 방법

실험동물은 체중 2∼4kg의 성숙가토 72마리를 사용하여 대퇴골 원위부 관절연골에 전층

손상을 만든후 다음과 같이 4군으로 구분하여 실험하였다.

제 1군: 능동적 운동군 18마리

제 2군: 능동적 운동 및 salicylate투여군 18마리

제 3군: 석고붕대고정군 18마리

제 4군: 석고붕대고정 및 salicylate투여군 18마리

수술후 각군은 1주, 2주, 3주, 4주, 8주 및 12주간격으로 도살하여 손상부의 육안적 및

광학현미경적 변화와 이염성의 출현정도에 대하여 관찰하고 손상부 주위관절표면의 이염

성의 변화를 관찰하였다.

실험성적 및 결론

1. 관절내 유착은 능동적 운동군에서는 거의 볼 수 없었으나, 석고붕대고정군에서는 3

주이후 부터 대부분의 실험동물에서 심하게 나타났으며, 석고붕대고정군의 salicylate투

여군은 비투여군에 비하여 그 정도가 경하였다.

2. 손상부 치유과정의 육안적 소견은 능동적 운동군이 석고붕대고정군에 비하여 조기에

치유되었으며, 치유조직의 육안소견은 정상연골에 가까왔고 salicylate투여군과 비투여

군을 비교하면 능동적 운동군은 별 차이가 없었으나, 석고붕대고정군에서는 salicylate투

여군이 치유가 빠르고 치유조직의 육안소견이 정상에 가까왔다.

3. 손상부 치유과정의 광학현미경적 소견은 능동적 운동군이 석고붕대 고정군에 비하여

치유조직내에서 초자연골분화가 잘되었으며, 석고붕대고정군은 관절내 유착으로 섬유조

직이 손상부치유조직과 주위연골을 덮는 경우가 많았다. Salicylate투여군과 비투여군을

비교하면, 능동적 운동군에서 뚜렷한 차이가 없으나 석고붕대고정군에서는 salicylate투

여군이 초자연골분화가 보다 잘 되었다.

4. 치유조직의 이염성소견은 능동적 운동군이 석고붕대고정군에 비하여 조기에 출현하

였으며 그 염색정도가 강하였다. Salicylate투여군과 비투여군을 비교하면 능동적 운동군

에서는 대동소이하나, 석고붕대고정군에서는 salicylate투여군이 이염성이 조기에 출현하

고 그 염색정도도 강하였다.

5. 주위관절연골의 이염성소견은 능동적 운동군에서는 표층을 제외하고는 거의 이염성

의 소실이 없었으나, 석고붕대고정군에시는 표층 및 전층에 걸쳐 이변성의 소실이 현저하

고 석고붕대고정군중 salicylate비투여군이 보다 현저한 이염성의 소실을 보였다.

이상과 같은 실험성적을 종합하면, 관절연골손상의 치유는 능동적운동과 밀접한 관계가

있으며 능동적 운동은 손상치유를 촉진시키고, 관절의 고정은 손상의 치유를 지연시키며

치유성분도 불완전하고 주위관절면의 퇴행을 초래하는 것으로 보인다. 또한 salicylate

는 관절의 고정에 의한 퇴행과정을 억제하고 손상부 치유과정을 촉진시키는 것으로 사료

되었다.





A Study for the Effects of Active Motion on the Repair of Articular Cartilage

Injury in Rabbits

Artifular cartilage is generally regarded as a specialized connective tissue

composed of chondrocytes which are disposed in a viscid ground substance composed

of heavily sulfated mucopolysaccharides, principally chondroitin sulfate, and

collagen fibers (Bennett et al., 1932).

The reparative processes which occur following injury to articular cartilage have

been controversial subject for many years. The subject has stimulated the curioisty

of many workers whose research has produced 3 main views on the nature of these

phenomena.

These views maintain: (1) that regeneration of damaged articural cartilage does

not cccur(Hunter, 1743: Leidy, 1849: Kolliker, 1853: Paget, 1853; Maximow & Bloom,

1957), (2) that healing occurs as a result of proliferation of adjacent cartilage

cells either from basilar or from the superficial cells at the joint margin

(Redfern, 1851; Seggel, 1904; Fascioli, 1905), and (3) that a cartilaginous defect

is repaired by metaplasia of connective tissue arising from the base and the sides

of the defect in the adjacent subchondral region (Fisher, 1923; Shands, 1931: Key,

1931; Bennett et al., 1932; Carlson, 1957).

And it would appear that many factors may affect the repair of wound in articular

cartilage, but none is more important than the depth of injury. So, it is believed

that superficial injuries which do not violate subchondral bone do not heal, while

deep injuries which penetrate subchondral bone heal by reparative tissue arising

from the marrow spaces with a greater or lesser transformation of undifferentiated

tissue into fibrocartilage or abnormal hyaline cartilage (Meachim, 1963; DePalma et

al., 1966; Rosenberg, 1967: Campbell, 1969: Ghadially et al., 1971).

There have been many observations for biological factors involved in the

differentiation of reparative cells. That intermittent active motion is a factor in

neochondrogenesis by cells from the subchondral bone was suggested by

Smith-Petersen's observation in 1939. After then, many observers published the

effect of active motion on heating process after articular cartilage injury (Hohl &

Luck, 1956: Kettunen, 1963; Kettunen & Rokkannen, 1973).

In addition, Salter et al. (1980) has proposed the concept that continuous

passive motion stimulates more rapid complete healing of full-thickness defects in

the articular cartilage of rabbits knees than does either immobilization or

intermittent active motion.

It has been emphasized that destruction of articular cartilage is the starting

point in the evolution of degenerative arthritis, and that all subsequent changes

in the various components of the joint are secondary (Bennett & Bauer, 1935;

Collins, 1949).

Simmons and Chrisman (1965) first demonstrated the ability of sodium salicylate

to inhibit the degradation of chondromucoprotein by catabolic enzymes, especially

cathepsin, and suggested a biochemical mechanism for one beneficial effect of

salicylate in degenerative joint disease.

After then, Chrisman & Snook (1968) showed that moderate levels of salicylates

had a definite protective effect against degenerative change of articular

cartilage.

However, it remains to be seen whether prophylatic use of aspirin in moderate

doses for a long period of time will prevent the development of degenerative

arthritis in recently damaged human joints.

The purpose of this study was to determine the effects of functional stimuli on

the healing of full-thickness defects and the effect of salicylate treatment on

degenerative changes of articular cartilage in rabbits as a test of the validity of

the aforementioned hypothesis.

In the experiment, full-thickness defects were created surgically in four areas

of the distal femoral condyles of mature rabbits.

A total of 72 mature rabbits, each weighing around 2∼4 kilograms, were used,

operated and divided into 4 groups. The first group was active motion, the second

active motion treated with salicylate, the third cast immobilization, and the

fourth cast immobilization treated with salicylate. All animals were sacrificed at

intervals up to 12 weeks and each defect was examined grossly and histologically.

The results are summarized follows :

1. Intra-articular adhesions were observed in the articular cartilage of rabbit's

knees that had been cast immobilized postoperatively for longer than 3 weeks. Such

adhesions were prevented by active motion. In cast immobilization groups, the

degree of intra-articular adhesion in salicylate treatment group was less than that

of no treatment group.

2. In gross findings on the healing of full-thickness defects, active motion

groups revealed more rapid and complete healing than did cast immobilization

groups. There was no significant difference between salicylate treatment and no

treatment group in active motion, however in cast immobilization groups, salicylate

treated group showed more or less rapid and complete healing of full-thickness

defects than did no treatment group.

3. In microscopic findings, neochondrogenesis in the healing of full thickness

defects seemed to occur through differentiation of the pleuripotential cells from

the subchondral tissues to hyaline cartilage. The degree of metachromasia in the

reparative tissue were comparable to the degree of differentiation to hyaline

cartilage. Active motion groups showed better healing than cast immobilization

groups. Although there were no significant differerce between salicylate treatment

and no treatment group, there was better differentiation to hyaline cartilage of

the salicylate treated rabbits in cast immobilization groups than no treatment

group.

4. Adjacent cartilages of the full-thickness defects in cast immobilization

without salicylate treatment revealed degenerative changes and marked loss of

metachromasia after three weeks, while active motion groups and cast immobilization

with salicylate treated group showed unremarkable findings on histology and

metachromasia.

In summary, the healing of full-thickness defects of articular cartilage has a

close relationship with active motion. As active motion group revealed more rapid

and complete healing, cast immobilization group showed delayed and incomplete

healing. In addition, degenerative changes in the adjacent articular cartilages in

cast immobilization without salicylate treatment group were more obvious than the

other groups. Therefore, the effect of salicylate on prevention of degenerative

changes seems to be insignificant between active motion groups, but it appears to

be effective on prevention of degenerative changes in cast immobilization and more

or less stimulated the healing of full-thickenss defects in cast immobilization

groups.
