항발한 국소도포제의 작용에 관한 전자현미경적 연구

땀분비는 체온 조절에 꼭 필요하나 피부과 영역에서 다한증, 한포진 및 염증성 피부질

환등의 치료를 위하여 땀분비를 억제하여야 할 경우가 많다

피부과 영역에서 많이 사용하고 있는 항발한 국소도포제증 glutaraldehyde(Papa 및 Kli

gman,1966)와 aluminum염(Lanadown, 1973)은 모두 단백질을 변성시키는 성질이 있어 이로

인하여 형성된 keratotic plug가 한공을 폐쇄하여 땀배설을 억제할 것이라 생각하고 있

으나 Juhlin 및 Hansson(1968)은 glutaraldehyde가 한공을 폐쇄하면 이차적으로 한선의

땀형성 기능이 저하되어 항발한 작용을 일으킨다고 하였고 PaPa 및 Kligman(1967)은 alum

inum염은 한공의 �쇄는 일으키지 않고 한관에서 땀의 재흡수를 일으킨다고 하였으며 She

1ley 및 Hurled(1975)는 한관을 기능적으로 폐쇄시켜 이차적으로 한선의 기능이 저하된다

고 하였다.

이와같이 glutaraldehyde나 aluminum염의 항발한 기전은 아직도 명확히 규명되있지 않

다.

본실험은 피부과영역에서 흔히 쓰이는 항발한 국소도포제인 glutaraldehyde, aluminum

hrdroxychloride 및 aluminum chloride를 택하여 주사전자현미경 및 투과전자현미경으로

한공 및 피부표면과 한선, 표피내한관의 형태학적 변화를 관찰하였다.

실험방법은 20∼30gm체중의 성숙된 마우스를 실험동물로 하여 발바닥에 실험약물을 매

회 20분씩 1일 2회 2∼5일간 국소도포한 후 땀분비 억제효과를 starch-iodine방법으로 확

인하고, 발바닥의 피부표면과 한공의 변화를 주사전자현미경으로 관찰하고 한선과 표피내

한관의 미세구조를 투과전자현미경으로 관찰하였으며 그 결과는 다음과 같다.

1. 대조군에 비하여 실험군에서 현저한 땀분비 억제가 있었다.

2. 주사전자현미경으로 한공 및 각질층을 관찰한 바 glutaraldehyde 및 aluminum chlor

ide처치군에서 한공이 현저하게 좁아지고 폐쇄되있었으며 한관의 나선도 관찰할 수 없었

고 각질층도 심한 변화가 있었다. 이에 반하여 aluminum hydroxrchtoride처치군에서는 한

공의 크기는 대조군과 거의 같았으며 한공의 일부만이 폐쇄되있었고 한관의 나선도 관찰

할 수 있었으며 각질층의 변화도 경미하였다.

3. 투과전자현미경으로 한관을 각질층, 과립층 및 유극충에서 관찰한 바 본 실험에서

사용한aluminum염은 전표피층의 한관에 변화를 일으켜 내강이 불규칙하게 좁아져 있었으

며 내강세포, 미세융모 및 주위세포의 변화도 있었으나 그 변화의 정도가 aluminum ch1or

ide에 비해 aluminum hydroxych1oride처치군에서 경미하였다. 이에 반하여 g1utaraldehyd

e처치군에서는 내강이 과립층 이상에서만 좁아져 폐쇄되 있었고 땀분비과립을 유극층에서

관찰할 수 있었다.

4. 투과전자현미경으로 한신을 관찰한 바 대조군이나 땀분비 억제약물을 도포한 실험군

에서 모두 땀분비 후에는 g1ycogen과립이 소실되고 intercellular canaliculi가 확장되고

그 수도 증가하였으며 mucous cell내에 공포가 증가함을 볼 수 있었다.

이상 본 실험약물들의 도포로 현저한 땀분비 억제가 있었으나 땀분비세포의 기능은 정

상이었고 한공과 표피층의 한관에 뚜렷한 변화가 있는 것으로 보아 본 실험약물들의 땀분

비 억제작용은 한공과 표피내한관의 변화에 기인하는 것으로 생각된다.

tion of Antiperspirants

Sweating is an essential physiologic phenomenon to regulate body temperature, but

in dermatologic field there are many skin diseases which require sweat inhibition

by topical antiperspirants.

Glutaraldehyde and aluminum salts are the oommonly uaed antiperspirants, but

their modes of action are stil unelear. Glutaraldehyde (Papa and Kligman, 1966) and

aluminum salts(Lanadown, 1973)have a Property of denaturing Protein to Produce

keratotic Plugs in sweat Pores.

This keratotic plug was held to be the common denominator for lack of sweating.

On the other hand, Shelley (1954), Juhlin and Hansson(1968) reported that sweat

glands avowed minimal activity in the presence of glutaraldehyde induced keratotic

plugging in the pored, and Papa and Kligman (1967) postulated that aluminum

anhidrosia is not due to formation of keratotic plugs, but due to increasing the

permeability of ducts resulting in complete reabsorption of thesweat.

According to Shelley and Hurley(1975), aluminum sales produce a functional

closure of the duct which is succeeded by shut-down of sweat gland secretion.

In the present work, both hind feet of a1bino mice weighing 20∼30 gm were

immersed into 10% glutaraldehyde, 10% aluminum hydroxychloride and 20% aluminum

chloride solution for 20 minutes twice daily for 2 to 5 lays and then

antiperspiratory effects were confirmed by the staroh-iodine method. Sweat pores,

intraepidermal sweat duets and secretory coils were observed by scanning and

transmission electron microscope.

The results are summerized as follows :

1) All of the antiperspirants used in this experiment showed marked

antiperspiratory effects.

2) In scanning electron microscopic findings, the sweat pores were markedly

narrowed aud occluded with non-cellular, amorphous materials and terminal spirals

(helices) of the ducts were not seen and surfaces of the foot pads were rough and

markedly superimposed in glutaraldehyde and aluminum chloride treated groups. The

sweat pores of aluminum hydroxychloride treated group were partially occluded and

terminal spirals of the suets were seen and akin surfaces were comparatively smootb

and slightly superimposed.

In contrast to antiperspirant treated groups, the control group showed that sweat

pores were patent and terminal spirals were clearly seen and skin surfaces were

smooth.

3) In transmission electron microscopic findings, the lumens of intraepidermal

ducta were irregularly narrowed without inclusions within the lumen in aluminum

hydroxychloride and aluminum chloride treated groups, but the narrowing of lumens

was more severe when using aluminum chloride than when using aluminum

hydroxychloride. In the glutaraldehyde treated group, the lumens were narrowed only

in stratum corneum and granulosum, and the lumens of stratum spinosum were filled

with secretory granulea as in the control greup.

The changes of luminal cell, microvilli and peripheral cell were definite in

glutaraldehyde and aluminum chloride treated groups, but those of aluminum

hydroxych1oride treated group were minimal.

4) After sweating induced by pi1ocarpine hydroch1oride injection, glycogen

granules of secretory coils were depleted and intercellular canaliculi were dilated

in both antiperspirant treated and control groups as shown by transmission electron

microscopy.

These otaervationa suggest that the production of sweat in secretory coils was

net affected by topical antiperspirants and the anhidrotic action of glutaraldehyde

may reaide within ducts in the stratum corneum and granulosum but the action of

aluminum aalts may reside within the entire duct.
