성인 편마비환자의 지팡이 높이에 관한 연구

연세대학교 의과대학 재활병원과 서울북부노인 종합복지관에서 입원 또는 외래치료중인

성인 편마비환자 중 지팡이 보행을 하는 25명을 대상으로 사용하고 있는 지팡이의 높이

를 알아보고, ink foot-print를 이용하여 각 환자의 지팡이의 높이에 따른 보행요인의 변

화등을 조사한 결과 다음과 같은 결론을 얻었다.

1. 연구대상자 25명 중, 23명(92.0%)이 건축의 대퇴골 대전자상단부보다 1인치 이상 높

은 지팡이로 보행하고 있었다.

2. 한발 지팡이를 사용하는 군이 네발 지팡이를 사용하는 군보다 평균보행속도가 16.48

cm/초 빨랐으며, 환측의 보폭은 16.56cm길었고 전체 체중지지면은 6.2cm 좁았다.

3. 지팡이 높이가 건축의 대퇴골 대전자상단부보다 1인치 높았을 때 보행속도는 건축의

대퇴골 대전자상단부 높이 때 보다 0.69cm/sec 빨라졌고, 걸음수는 1.42걸음/분 증가하

였다(p<0.05).

4. 지팡이 높이가 건축의 대퇴골 대전자상단부보다 1인치 높았을 때 두발사이의 체중지

지면은 건축의 대퇴골 대전자상단부 높이 때 보다 0.71cm 좁아졌고, 전체 체중지지면은 1

.14cm 좁아졌다(p<0.01).

이상의 경과로 볼 때 성인 편마비환자가 사용하는 지팡이의 높이는 이제까지 알려진 지

팡이 높이보다 높아야 보행에 도움을 줄 것으로 사료된다.





A Comparative Study of Two Heights of Cane in Adult Hemiplegia

The purpose of this study was to survey the height of canes that have been used

by adult hemiplegic patients and to determine if any significant difference exists

between two selected heights of cane, greater trochanter level(shorter) and one

inch higher than greater trochanter(longer) used by the study subjects.

The subjects of the study were 25 adult hemiplegic patients(17 male, 8 female)

with a mean age of 61.2 years, who used canes(9 mono, 16 quad) as an ambulatory

aid. All subjects received rehabilition treatment as in- and out-patients at Yonsei

University Medical College Rehabilitation Hospital and Seoul Buk-bu Welfare Center

for elderly. The study was carried out between April 1, 1993 and April 22, 1993.

All 25 subjects were tested for differences in 6 aspects of gait. Each subject

was tested twice, once with each height of cane. This study used the ink

foot-print, one of several temporal distance measures of gait performance.

The results of the study, analysed by Wilcoxon signed rank test, were as follows:

1. Of the total 25 subjects, 23 had been using the longer height prior to the

study.

2. The 3 variables studied all showed better scores(velocity: 16.48cm/sec faster

: stride in the affected leg: 16.56cm longer: base of support between the cane and

the affected leg: 6.2cm narrower) when using the mone rather than the quad cane.

3. When using the longer cane, the mean gait velocity and mean gait cadence were

0.69cm/sec faster and 1.42step/min more respectively.

4. When using the longer cane, the mean base of support between the sound and

affected legs and mean base of support between the cane and the affected leg were

narrower by 0.71cm and 1.14cm respectively.

These results show that the cane height of 1 inch higher than top of the greater

trochanter of the femur is more effective for adult hemiplegic patients who require

a cane as an ambulatory aid.
