매독의 혈청학적 검사 : VDRL, FTA-ABS 및 TPHA검사 결과의 비교

매독위 진단을 위해서는 환자의 병력, 임상소견 및 암시야검사법, 매독혈청검사등이 필

요하다. 많은 매독환자에 있어서 임상증세가 나타나지 않기 때문에 매독혈청검사가 매독

의 진단에 있어서 중요한 역활을 한다. 매독혈청검사는 사용되는 항원에 파라 non-trepon

emal test 와 treponemal test 로 나눌 수 있다. 이중 가장 널리 사용되는 검사법에는 VD

RL, FTA-ABS 및 TPHA검사가 있다.

대부분의 보고들에 의하면, TPHA검사가 FTA-ABS 검사와 비슷한 정도의 감수성 및 특이

성을 가진다고 한다. 반면에 PTA-ABS검사보다 검사방법이 쉽고 경제걱인 이점이 있다.

저자들은 1982년 5월부터 1983년 2월까지 연세의대 부속 세브란스병원에서 시행한 혈청

VDRL 및 FTA-ABS 검사 결과 어느 한가지 혹은 두 검사 모두가 양성반응을 보인 375예의

혈청으로 TPHA정성 및 정량검사를 시행하였다. 위의 세가지 검사 결과를 비교하고, TPHA

검사의 선별검사로서의 적합성을 조사하여 다음과 같은 결과를 얻었다.

1. VDRL 검사는 제2기매독을 제외하고는 TPHA 및 FTA-ABS검사에 비해 그 감수성이 낮다

.

2. FTA-ABS검사는 제1기매독에서 TPHA 검사보다 감수성이 높으나, 시간과 경비가 많이

소모되는 단점이 있다.

3. TPHA 역가는 제1기 매독에서는 비교적 낮아서 대개 320 이하이다.

4. 매독치료 후 1년이후에 TPHA 정량검사를 한 환자 50명 32명(64%) 에서 TPHA 역가가

320 이하인데 비해, 매독치료 후 1년이내에 검사를 한 환자는 32명중 13명(41%) 만이 TPH

A 역가 320 이하를 나타내었다.

5. TPHA검사는 VDRL검사와 77%의 양성반응 일치도를 보이는 반면 FTA-ABS 검사와는 89%

의 양성반응 일치도를 보였다.

6. 매독혈청의 80%만이 VDRL 검사의 의해 검출된 반면 93%가 TPHA검사에 의해 검출되었

다.

이상에서 보듯이 VDRL검사는 조작이 간편하고 경제적이긴 하나 매독혈청을 검출하는 능

력이 낮기때문에 선별검사로서는 부적당하다. 반면에 TPHA 검사는 매독혈청을 검출하는

능력이 높은 동시에 조작이 쉽고 경제적이어서 매독의 선별검사에 가장 유용한 검사로 생

각된다.

Serologic tests for syphilis are diagnostically useful when propery assessed in

combination with full historical, epidermiologic, and clinical knowledge of the

patient.

Serologic tests for syphilis are classified by the type of antigen used, namely

nontreponemal or treponemal.

The most widely used tests are the VDRL, FTA-ABS and TPHA tests.

The results of the majority of the authors show that the TPHA test has in general

a favourable specificity and sensitivity in comparison with FTA-ABS test.

The TPHA teat is easy to perform and it requires neither highly skilled

personnels nor expensive equipment.

On a total 375 sera with reactive results on VDRL and/or FTA-ABS test(s), derived

from Severance Hospital, qualitative and quantitative TPHA teats were carried out.

The objectives of the present study were to compare the result of VDRL, FTA-ABS and

TPHA tests in different syphilis atages, and to assess the suitability of the TPHA

test as a screening test for syphilis. The results are summarized as follows :

1. The sensitivity of VDRL test was poor compared with TPHA and FTA-ABS tests

except in secondary syphilis.

2. The FTA-ABS test(100%) was more sensitive than the TPHA test(86%) in primary

syphilis. but It is time consuming and costly.

3. The TPHA titers were relatively low in primary syphilis.

4. Below 1:320 in TPHA titer, the percentage of sera from patients tested over 1

year(64%) after the completion of treatment was higher than within 1 year(41%).

5. The TPHA test showed 77% agreement with VDRL test and 89% agreement with

FTA-ABS test.

6. 80% of known syphilitic sera was detected by VDRL tests and 93% of that by

TPHA test.

The VDRL test is easy to perform and economic, but it showed poor effectiveness

as screening test for the detection of syphilis. On the other hand the TPHA test

had a wade spectrum of reactivity in different stages of syphilis and was easy to

perform. So on the basis of the results presented, we concluded that the TPHA test

provides a very effective screen for syphilis.
