간흡충증에 있어 항체검출을 위한 Enzyme-linked immunosorbent assay와 thin layer immunoassay의 비교

간흡충증 진단을 위하여 환자 혈청에서 Sadun등(1959)은 피내 반응을, 任(1974)은 간접

형광항체 법을, 李등(1981)은 효소표식 면역법(enzyme-linked immunosorbent assay) 등의

방법을 이용하여 항체를 검출한 보고들이 있다. Nilsson등(1980a, 1980b)은 주혈흡충증

과 아메바증에서 thin layer immunoassay(TIA)를 이용하여 항체 를 검출하였다.

본 연구에서 간흡충증 환자 혈청에서 TIA에 의하여 항체를 정량적으로 측정하여 감수성

과 특이성을 알아보고, 효소표식면역법에 의한 성적과 비교하였다. 냉동건조한 간흡충 성

충의 생리식염수 추출물을 항원으로 사용하였고, TIA는 Elwing등(1976)의 방법으로, 효소

표식면역법은 Voller등(1974)의 방법에 의해 시행하였다.

간흡충증 환자 60명의 혈청에서 TIA zone 크기의 평균은 4.14mm였고 3mm이상을 양성으

로 판정하였을때 감수성은 100%, 특이성은 61%였다. 효소표식면역법에 의하면 ELISA치 0.

8이상을 양성으로 판정하였을때 감수성은 88.3%, 특이성은 87.3%였다. 혈청내 IgG농도는

TIA zone크기와 상관이 없었으나 효소표식면역법에 의한 ELISA치와는 상관이 있어 그 계

수는 0.69였다. TIA zone 크기는 효소표식면역법에 의한 ELISA치와 그 분포에 있어 상관

이 없었다.

Comparison of TIA with ELISA for circulation antibody detection in clonorchiasis

A comparison was made of a new serological method, thin layer immunoassay (TIA),

and an established method, enzyme linked immunosorbent assay (ELISA), in the

detection and quantification of antibodies in clonorchiasis.

Saline extract of lyophilized Clonorchis sinensis adult worm was used as antigen,

and TIA by the method of Elwing et al. (1976) and ELISA by Voller et al. (1974)

were performed. Using sera from known clonorchiasis cases, 100% of the sera tested

were positive by TIA and 88.3% by ELISA. TIA produced false positive results in the

14 cases out of 36 cases, which were 10 amoebiasis cases, 16 paragonimiasis cases

and 10 healthy control cases. ELISA however produced a small number of false

positive, 7 cases out of 55 cases.

There was correlation between Immunoglobulin G level in sera and ELISA value

(correlation coefficient, 0.69), whereas no correlation between Immunoglobulin G

level and TIA result. The performance of TIA and ELISA were not correlated in its

results using homologous antigen.
