병합요법에 의한 급성임파성백혈병의 생존율에 관한 연구

소아에서 가장 흔한 급성임파성백혈병은 다른 어떤 악성종양보다

장기 생존율이 높은 질환이다.

저자는 146예의 급성임파성백혈병 환아에 대한 임상적 연구와 특히 병합요법에 의하여

완해가 된 43예에 대한 생존율을 산정하여 그 결과를 보고 하는 바이다,

선진국과 비교하여 임상증상에서 의의있는 차이가 없으며, 최종진단은 골수 흡입을 시

행하였고 중추신경백혈병은 그 증상이 있는 경우 요추천자를 시행하여 척수액내의 아세포

를 확인하였다.

치료방법은 완해의 도입후 체내에 남아있는 백혈병세포를 완전히 근절시키기 위하여 적

극적인 집중적 화학요법으로 항백혈성 제제를 사용하고, 예방적으로 중추신경계통에 방사

선치료를 실시하였다.

이와같은 적극적 화학요법과 방사선치료를 실시한 즉 발전된 병합요법을 실시한 l975년

이후에서는 2년 생존율이 65%, 5년 생존율이 50%로 미국의 Children Cancer Study Group

(C.C.S.G)에서의 1972년도 생존율과 유사하였다.

Survial rate of acute lymphocytic leukemia with multimodality therapy

Acute lymphocytic leukemia apperars curable in over one third of all diagnosed

patients who received intensive combination chemotherapy and preventive CNS

radiotherapy.

Protocol for ALL has been used at Yonsei University during the last 7 years,

consist of - 1) Induction of remission; vincristine and prednisone have been the

standard combination. Drugs such as adriamycin or L-asparaginase have been added to

this combination for high risk patients.

2) Preventive CNS radiation; radiotherapy (2400 Rads) combined with intrathecal

methotrexate during the first month of complete remission. 3) Continuation

chemotherapy; the combination of methotrexate and 6-mercaptopurine was used for up

to 3 years.

Since Jan. 1975, 48 ALL patients were treated with above regimen with following

results; 5 year leukemia-free survival is estimated in 50% of patients by life

table method and 7 patients can be considered potentially cured. (Over 1 year from

end of therapy)
