침술의 역사는 상당히 길고 임상적으로 동통제어에 그 효과가 인정되고 있으나 그 작용

기전에 대해서는 아직 확실히 알려져 있지 않다. 현재 침술의 동통제어효과의 작용기전은

주로 뇌간과 척수에 존재하는 endogenous opiate system의 활성화와 광제가 있을 것으로

생각하고 있다 (Reynolds, 1969;Mayer등, 1971 :Mayer 및 Liebeskind, 1974 : Oliveras

등,1975 : Proudfit 및 Anderson,1975). 그러나 endogenous opiate system을 활성화시키

는 기전에 관하여는 아직 확실히 규명되어 있지 알다. 다만 침술이 그 진통효과를 나타내

는데는 침을 시행한 부위에 존재하는 ��駕키堧�자극되어 그 작용이 나타나는 것으로 알

려져 있다(levy 및 Matsumoto, 1975 : 백등, 1981).

한편 한방의학에서는 소위 침점 (meridian point)이란 것이 이ㅛ으며 동통부위에 따라

침술을 시행하는 침점이 달라질 수 있는데 반드시 동통이 있는 부위에서 침술을 시행하는

것은 아니다. 그러나 이와 같은 침점이나 경락system에 대한 신경해부학적 또는 생리학

적인 의의는 체계적으로 규명되지 못하고 있는 실정이다.

따라서 본 실험에서는 침술을 시행하는 부위와 진통효과가 나타나는 부위와의 상호관계

를 밝혀 보려는 실험의 일환으로 시행하였다.

실험동물로는 고양이를 사용하였으며, 굴근반사를 동통의 지표로 삼고, 굴근반사를 유

발한 하지의 대측 common peroneal nerve 및 대측 superficial radial nerve를 각각 20V

의 강도, 2msec의 duration 및 2Hz의 빈도로 60분간 전기 자극한 후 굴근반사의 변화를

기록하여 진통효과를 관찰하였고 또 굴근반사를 유발한 하지의 동측 common peroneal ner

ve를 전기 자극했을 때 나타나는 효과와 상호 비교하여 다음과 같은 결과를 얻었다.

1. 대측 common peroneal nerve 및 대측 superficial radial nerve를 전기 자극한 경우

굴근반사에 의의 있는 변화가 없으며, 두 경우 상호간에 의의 있는 차이를 볼 수 없었다

.

2. 굴근반사를 유발한 하지의 동측 common peroneal nerve전기자극시 굴근반사는 의의

있는 감소를 보였고, 특히 느린성분은 naloxone hrdrochloride를 투여함으로써 원상으로

회복되었다.

이상의 결과로 미루어 보아 고양이의 경우 통증을 유발한 하지의 동측 말초신경을 전기

자극시에는 진통효과를 볼 수 있었으나, 대측의 말초신경을 자극시에는 진통작용을 나타

내지 않을 가능성이 클 것으로 사료된다.

It is well known that the acupuncture has been used effectively for the relief of

certain types of pain. Although the precise mechanism of action of acupuncture

analgesia is unknown, it is generally accepted that their analgesic properties are

related to the activation of endogenous opiate system in central nervous system.

And it is suggested that pain-relieving properties of acupuncture may be related to

a stimulation of peripheral nerve underlying the acupuncture point on the son.

However, the efficacy of acupuncture has no relationship between the site of pawn

and the acupuncture point.

Consequently, the present study was undertaken to investigate electroacupuncture

analgesia in relation to the site of peripheral nerve stimulation. Cats were

decerebrated ischemically and the flexion reflex as an index of pain was elicited

by stimulating the sural nerve (20V, 0.5 msec duration) and recorded as a compound

action potential from the nerve innervated to the posterior biceps femoris muscle

in the ipsilateral hind-limb. Bilateral common peroneal nerve and contralateral

superficial radial nerve were selected as the site of peripheral nerve stimulation.

For the stimulation of peripheral nerve, a stimulus of 20 V intensity, 2

msec-duration and 2 Hz-frequency was applied for 60 min respectively.

The results obtained are summarized as follows :

1. Both atimulation of contralateral common peroneal nerve and contralateral

superficial radial nerve dud not change the flexion reflex and there were no

significant differences between them.

2. Stimulation of ipsilateral common peroneal nerve markedly depress the flexion

reflex the effect being reversed by naloxone application.

These results suggest that stimulation of ipsilateral common peroneal nerve has

the analgesic effect but both stimulation of contralateral common peroneal nerve

and contralateral superficial radial nerve to the pa
