가돈 피부의 표재성창상 치유에 관한 연구

dc.description.abstract[영문] [한글] 부분층 피부이식의 공여부, 찰과상, 박피술(dermabrasion) 부위 및 1∼2도 화상 등에서 임상적으로 흔히 접하게 되는 표재성 창상의 치유는 창상의 변연과 남아있는 모낭, 한선 (sweat gland) 및 피지선(sebaceous gland)등에 있는 상피세포의 재생에 의해서 이루어 진다(Grabb, 1979). 저자는 표재성창상에 있어서 개방적 처치와 폐쇄적 처치 및 연고체의 사용이 창상치유 에 어떠한 영향을 미치는가를 알기 위하여 사람의 피부와 가장 유사한 피부를 갖은 돼지 를 실험동물로 선택하여, 인위적으로 표재성창상을 만든후, 개방적 처치와 폐쇄적 처치 및 연고제 (Petrolatum, Gentamycin연고, Madecassol연고, Triamcinoone Acetonide 연고) 처치를 개방적 처치와 동반하여 시행하고 상피의 재생과정과 재생속도를 구분구적법(pla nimetry)에 의하여 매일 측정하였다. 실험결과, 창상은 Triamcinolone Acetonide 연고 처치군을 제외하고는 모두 7∼8일 경 에 치유되었으며, 창상의 초기 즉, 창상을 만든후 처음 3일 까지는 폐쇄적 처치를 한 군 이 상피세포의 재생속도가 가장 빨랐으며, 4∼5일 사이에는 개방적 처치를 한 군에서 재 생속도가 가장 빨랐다. 연고제의 사용은 창상의 치유초기에는 상피세포의 재생에 큰 영향을 못 미치며, 경우에 따라서는 오히려 상피세포의 재생을 지연시키는 경향도 있었다. 또한 이와 같은 결과로 서 현재 임상적으로 널리 사용하고 있는 피부이식의 공여부처치법, 즉 창상의 초기에는 폐쇄적 처치를 한 후, 4∼5일경에 이를 개방하는 방법이 상당히 의의가 있음을 입증 할 수 있었다. A study on the healing of superficial wounds of pig skin Yong-Bae Kim Department of Medical Science The Graduate School, Yonsei University (Directed by Professor Jae-Duk Lew, M.D.) The wound healing is one of the oldest concerns of the medical profession. In a superficial wound, the healing is accomplished by re-epithelization and migration of epithelial cells which originate from the intact epithelium that surround the wound and from the undestroyed portions of epithelium appendages(hair follicles, sebaceous glands and sweat glands) within the wound. This study was conducted to clarify the effect of various ways of management of the healing process of superifcial wound. A young domestic pig was used for this experiment because its skin structure resembles human skin in the relative thickness of epidermis to dermis, the presence of epidermal ridges, the distinct dermal papillary layer, and the relative spacity of hairs. A total of 60 superifcial wounds each measuring 2.0 cm x 1.5 cm were created by split thickness(0.3mm) skin graft using Stryker electric dermatome on the flank and back. There were 3 groups of wounds; group 1 occlusive dressing, group 2 open treated, group 3 ointment treated (subgroup 1 Petrolatum treated, subgroup 2 Gentamycin treated, subgroup 3 Madecassol ointment treated and subgroup 4 Triamcinolone ointment treated). Daily treatment and observation of each wounds were carried out with measurement of the area of re-epithelization by the planimetry. The results were comparatively analyzed and summarized as follows. 1. All group completed epithelization within 8 days except Triamcinolone ointment treated group. (11.5 days) 2. In an early period of wound healing, the occlusive dressing revealed the fastest rate of epithelization up to the third day after wounding. 3. After the third day, the open treatment showed the faster rate of epithelization than the other groups till the 6th day. 4. The Petrolatum and Gentamycin ointment were considered to be effective in the late period of wound healing. (5th to 7th days) In conclusion, it can be suggested that the rate of superifcial wound healing such as split thickness skin graft donor site can be promoted in re-epithelization by initial occlusive treatment followed by open treatment then the application of ointment.-
