항경련제 치료가 간질의 임상경과에 미치는 영향

간질은 가장 흔하면서도 치료가 쉽지 않은 만성 신경질환의 하나이지만 정확한 간질유

형의 진단 및 이에 따르는 선택적 항 경련제의 투여로 간질의 높은 진정율(remission rat

e)이 보고되고 있다. 그렇지만 우리나라의 경우 이러한 최신개념들이 아직 널리 보급되어

있지 않은 실정이며 간질치료에 관련된 임상요인들의 분석도 미비한 것으로 사료된다.

이에 본 연구자는 1989년 5월에 연세대학교 의과대학 세브란스병원에 간질진료소가 개

설됨에 따라 일정양식의 간질 등록지를 작성 이들에 원칙적으로 항 경련제의 단독요법을

적용하고 추적진료를 하여 1990년 4월까지 최소 6개월 이상 항 경련제치료와 추적진료를

한 96명을 대상으로 내원 전의 치료유무에 따라 유 치료군 (52명)과 무 치료군(44명)으로

나누었으며 내원 후 두 군에서의 임상경과와 이에 관련된 임상요인들을 분석하여 다음과

같은 결과들을 얻었다.

1. 간질의 종류는 유치료군의 경우 부분발작이 44예(84.6%)며 무치료군의 경우는 부분

발작이 31예(70.5%)로 두 군에서 모두 부분발작이 많았다.

2. 유치료군에 있어서 내원전 항경련제의사용은 병용요법이 29예(75%)로 대부분이었으

며 이중 phenytoin과 phenobarbital의 병용이 23예(79.3%)로 가장 많았다.

3. 유치료군에서 내원후 43예(82.7%)에서 항경련제의 단독요법이 적용되었으며 완전조

절이 20예(38.5%), 우수조절이 15예(28.8%)에서 이루어져 발작의 평균횟수도 통계적으로

의의있게 감소하였다.

3. 무치료군은 모든 환자에서 항경련제의 단독요법이 적용되었으며 이중 30예(68.2%)에

서 완전조절이 성취되었고 11예(25.0%)에서 우수조절이 관찰되었다.

5. 유치료군과 무치료군에서 간질의 완전조절율은 통계적으로 의의있는 차이를 (P<0.00

09) 보였다.

6. 불량한 간질조절과 관계 있는 환자의 임상요인들로서는 유치료군의 경우 부분발작,

다종의 발작형 (multiple type of seizure), 내원전 다수의 평균 한달 발작횟수 및 이상

뇌파검사소견 등이며 무치료군의 경우 다종의 발작형, 내원전 발작의 횟수와 유병기간 등

이었다.

이상을 종합하여 볼 때 현재 우리나라에서는 아직 항 경련제의 병용요법이 흔히 사용되

고 있다는 사실을 관찰할 수 있었으며 본 간질진료소로 내원후 이들 중의 대부분에서 단

독요법으로의 전환이 가능하였고 임상경과도 호전되었으므로 앞으로는 항경련제의 단독요

법이 널리 사용되어야 할 것으로 사료된다. 또한 무치료군과 유치료군의 간질의 완전조절

율은 통계학적으로 의의있는 차이가 있는 것으로 나타났으며 이는 간질의 중증의 원인적

요인에 의한 것으로 생각된다. 그렇지만 유치료군에 있어서도 20예(38.5%)가 선택적 항경

련제의 단독투여에 의해 완전조절이 가능하였음은 역시 적절한 항 경련제 치료가 간질의

임상경과에 중요한 영향을 미치는 것으로 설명될 수 있다. 따라서 본 연구는 간질발작의

유형에 따라 선택된 항 경련제의 단독 요법이 시행되어야 한다는 것을 시사하고 만일 간

질의 발작이 일년 이상 완전 조절되지 않는 경우는 간질전문진료소로의 조속한 의뢰가 필

요할 것으로 사료된다.





Attributions of Antiepileptic Drug Treatment in Clinical Course of Epilepsy

We conducted an investigation to evaluate the clinical outcome of epileptic

patients after the initiation or adjustment of antiepileptic treatment made at the

epilepsy clinic of Y.U.M.C. Total 96 epileptic patients had been followed for at

least 6 months at the epilepsy clinic until April 30, 1990. They were devided into

two groups; treated and untreated. Treated group consisted of 52 patients who had

been under active drug therapy by local physicians before their referral to the

epilepsy clinic. Untreated group was 44 patients who had never been treated with

any antiepilptic drugs previously.

All patients had through history taking, physical and neurological examinations.

CT scan of the head and one or more EEG recordings during the period of follow up

at the epilepsy clinic. Their seizure types were determined by two staff

neurologists according to the international classification of epileptic seizures.

Monotherapy, choice of a single drug considerd to be appropriate for the

patient's seizure types and use of maximal tolerable dosage until complete control

of seizures or emergence of side effects, was the principal method of drug therapy

at the epilepsy clinic.

Results are follows:

1. Among 52 patients of treated group, 29 cases were on polypharmacy before their

referral. At the end of follow up at the epilepsy clinic, 43 patients (82.7%) were

remained to take a single antiepileptic drug.

2. In treat group, complete seizure control was obtained in 20 cases and 15 cases

achieved more than 50% reduction of seizure frequency, all after adequete

monotherapy. In untreated group, 30 cases achieved complete control of seizures

during the follow up period, which was statistically significant compared to that

of the treated group.

3. The factors related with the poor outcome of epilepsy in treated group were

partial seizures, multiple types of seizures, higher frequency of seizures and

abnormal EEG findings. In untreated group, multiple types of seizures, more

frequent seizures and duration of epilepsy were correlated with a poor outcome.

Our results revealed that monotherapy is underutilized by local physicians in

Korea and a large proportion of refractory epileptic patients can be either

completely controled or better managed by structured antiepileptic therapy

conducted at the specialized epilepsy clinic.
