시신경관은 태생기 2개월에 형성되는 접형골의 소익 (lesser wing)으로 만들어지기 시

작하며 생후 3∼ 5년에 성인의 크기에 도달한다. 이는 안와와 두개강을 연결하는 관으로

길이는 평균 4∼9mm고 직경은 평균 4∼6mm다.

시신경공의 크기와 형태를 생체에서는 직접 측정할 수는 없으나 1911년 Rhese가 최초로

X선을 이용하여 시신경공을 관찰한 이래 여러 학자들의 연구에 의해 X선을 이용한 시신

경공의 크기와 형태에 관한 여러가지 검사가 가능해졌다. 시신경공을 X선사진을 촬영하

여 관찰할 때는 그 크기 뿐만 아니라 모양, 양안의 대칭성, 시신경관과 그 주위의 골절,

균열 및 가골형성등의 유무를 확인해야 한다.

1924년 Goalwin의 보고에 의하면 X선 필름에 나타나는 정상 시신경공의 모양은 원의 사

분의일 구역형이 대다수고 그 외에는 난형, 타원정, 원형 및 사각형이며, 크기는 시신경

공 둘레의 상내측정점에서 45° 하외측방향으로 측정한 A직경과 상외측정점에서 A직경과

수직으로 측정한 B직경으로 나누어서 표시했다.

1926년과 1927년 Goalwin은 정상성인의 A직경은 평균 4.3mm, B직경은 평균 4.4mm며 최

대직경은 5.6mm, 최소직경은 2.8 mm로 발표했고, 1910련 Harwood-Nash는 축성단층 X선 촬

영으로 4세이상의 연령층에서 나타난 정상 시신경골의 수평직경이 평균 5.5mmd며 7.0mm까

지의 크기의 것도 있다고 보고했다.

이와 같이 구미 각국에서는 시신경공에 말한 보고가 많이 있으나 우리나라에는 이에 관

한 보고가 아직 없는 실정이다.

저자는 이러한 점을 유의하여 1974년 1월부터 1979년 12월까지 연세대학교 의과대학부

속 세브란스병원을 내원한 한국인 156명( 312한)헤서 Goalwin의 촬영방법으로 X선 촬영을

하여 나타난 시신경공을 대상으로 하여 연령에 따른 시신경공의 크기와 변화를 관찰했고

성장이 완성되는 경향을 보인 5세를 경계로 하여 5세후 연령층 139명( 278안)의 정상시

신경공의 크기, 모양 및 양안대칭성에 관한 관찰을 통해 다음과 같은 결과를 얻었다.

1. 시신경공의 모양은 난형 (42.4%), 원형 (28.4%), 원의 사분의일 구역형(19.8%) , 타

원형 (9.4%)의 순으로 나타났고, 86%에서 양안대칭성을 보였다.

2. 시신경공의 각 직경의 평균크기는 A직경은 우안 4.73±0.642 mm, 좌안 4.76±0.691

mm, B직경은 우안 5.00 ±0.639 mm, 좌안 4.65±0.599 mm, 수평직경 은 우안 4.93±0.73

3 mm, 좌안 4.97±0.723 mm고 최대직경은 7.10 mm, 최소직경은 2.90 mm였다. Goalwin의

공식에 의해 계산된 시신경공의 각 직경의 평균크기는 X선 필름에 나타난 크기보다 0.03

∼0.04 mm 작았다.

3. 연령에 때른 시신경공의 크기는 각 직경에서 5세에 성장이 완성되는 경향을 보였고,

시신경공의 각 직경의 평균크기는 5세전 크기가 5세후 크기보다 0.44∼0.96 mm 작았다.

(P<0.006).

4. 성별에 따른 시신경공의 모양과 크기는 별 차이가 없었다.

5. 시신경공의 우완과 좌안의 크기의 차이는 각 직경에서 전혀 차이가 없는 경우는 평

균 13.8%, 5% 미만의 차이를 보인 경우는 평균 41.8%, 20%미만인 경우는 평균 89.1%였다.

6. 시신경공의 모양이 양안대칭인 경우에는 A 직경, B 직경 및 수직직경에서 좌우안의

차이는 비대칭인 경우보다 적었다. (P<0.04)





OBSERVATIONS OF NORMAL OPTIC FORAMEN IN KOREA

The optic canal begins to be formed by the fusion of two roots of t도 lesser wing

of sphenoid. It is a funnel shaped tube measuring about 4-9 mm long and 4-6 mm

broad. But the canal shows considerable variations in its caliber. The optic cana

lattains its full size at 3 and 5 years of age.

Since the radiograms of the optic canal was taken in the living human subject

first by Rhese (1911), various radiographic methods have been revised by many other

scholars. The development of the radiograhic technique made it possile not only to

evaluate the morphology (size, shape and its symmetry) of the canal but also to

find its pathology such as fracture, fissure, callus fromation and abnormal shadow.

In the radiogram of the optic foramen the most shows a quadrant of circle in

shape, being ore or less slliptical, circular or quadrangular. The tow principal

diameters were measured ; the A diameter, which is directed downward and outward at

an angle of 45 degress, and t도 B diameter, which is at tight angle to A (Golwin,

1924). The average A diameter of normal adult optic foramen was reported to be 4.3

mm and the B 4.4 mm by Goalwin (1926, 1927). The average transverse diameter of

normal optic foramen was said to be 5.5 mm by Harwood-Nash(1970).

In Korea, normal Korean optic canal symmetry of 312 normal optic foramen of 156

Krean were studied in the radiogram taken by Goalwin's method from January, 1974 to

December, 1979.

1. The shape of the optic foramen was oval (42.4%), circular (28.4%), a quadrant

of a circle (19.8%) and ellipitcal (9.4%). And 86% of the foramen showed

symmetrical in shape.

2. The size of the optic foramen was as follows ; A diameter is 4.73 mm(right),

4.76 mm (left) ; B diameter 5.00 mm (right), 5.06 mm (left) ; horizontal diameter

4.71 mm (right), 4.65 mm (left) ; vertical diameter 4.93 mm(right), 4.97 mm(left).

Difference between the diameters of the foramen measured by means of

roentgenography and the actual diameter calculated by Goalwin's formula was

0.03-0.04 mm.

3. The diameter of optic foramen seemed to attain its full size at 5 years of

age. The average size of each diameter before age 5 was shorter than that after age

5 by 0.44-0.96 mm (p<0.006).

4. There were no appreciable variations in size and shape depending on sex.

5. The optic foramen was same on the two sides (13.8%). Diferences of 5% or less

were noted 41.8% ; differences of 20% of less shown in 89.1%.

6. Differences between the diameters (A,B, vertical)of the foramen of the right

and that of the left were less in the symmetrical foramen than asymmetrical

(p<0.04).
