뇌하수체 종양환자의 수술 후 건강관련 삶의 질과 영향요인

Abstract

This study was designed with a retrospective correlational study for describing postoperative quality of life of the patients with pituitary macroadenomas and influencing factors. Medical records of sixty-two patients with pituitary macroadenomas who preoperatively experienced visual disturbance and thus underwent transsphenoidal surgery, were retrieved to this study. At the point of postoperative 3 months, the authors evaluated patients’ quality of life with Korean short form-36 (KSF-36), and measured patients’ major symptoms; visual field defect by Humphrey test, olfactory dysfunction by KVSS (Korean Version of the Sniffin’ Stick) test and headache with HIT-6 (Headache Impact Test-6). Physical component score of KSF-36 was 66.40 and mental component score was 66.81. The mean deviation of worse eye field was -4.33±5.14 on Humphrey test, and KVSS score was 32.29±4.36. For headache, the score of HIT-6 was 43.81±7.24. With correlation analysis, headache showed strong negative correlation with physical component score of KSF-36 (r=-.501, p<.001, Adj. R2=36%) and mental component score (r=-.448, p<.001, Adj. R2=26%), which implies headache is a major factor that decreases the quality of life after transsphenoidal surgery for pituitary macroadenomas. This study indicates that nursing intervention is essential for the evaluation and management of the postoperative headache in the patients who undergo transsphenoidal surgery for their pituitary adenomas. Other neurological symptoms including visual field defect and olfactory dysfunction should be also monitored after surgery, and further investigation is necessary for the development of practical remedies to enhance their health-related quality of life.
