심정지 환자에서 흉부 압박의 위치 변화에 따른 혈역학적 효과의 비교

dc.description.abstract[한글] 심폐소생술에 대한 많은 연구가 진행되었으나, 심폐소생술에 관한 관심은 심폐소생술의 보급과 전파에 집중되어 왔다. 따라서 현재 사용되고 있는 흉부압박법의 효율성에 대한 과학적 검증이 이루어진 적이 없으며, 특히 흉부압박이 심장 등 혈류 발생에 영향을 주는 장기에 어떠한 영향을 주는지에 대한 연구가 없었다. 현재의 흉부압박법에 대한 지침은 1960년 이 방법이 제시되었을 때에 비하여 단지 압박 속도만을 변경시켰을 뿐, 압박 방법은 당시와 동일한 위치와 방법을 사용하도록 규정하고 있다. 이 연구의 목적은 심정지 상태의 인체에서 현재의 지침에 따른 흉부압박법(표준압박)에 의한 혈역학적 효과와 흉골의 하단을 압박하는 새로운 흉부압박법(변경압박)의 혈역학적 효과를 비교함으로써, 현재의 방법보다 혈역학적으로 우수한 새로운 심폐소생술 방법을 찾아보는 것이다. 응급센터에 심정지 상태로 내원하여 30분이상의 전문 심폐소생술에도 자발적 순환이 회복되지 않았던 18세 이상의 비외상성 심정지 환자 17명을 대상으로 하였다. 표준 심폐소생술 및 전문 심장구조술을 30분 동안 시행한 후에도 자발적 순환이 회복되지 않으면, 표준압박으로 3분간 심폐소생술을 한 후, 변경압박으로 3분간 심폐소생술을 하였다. 심폐소생술이 진행되는 과정에서 각각의 압박 방법에 의한 심폐소생술이 시작된 후 2분 30초가 경과하면 30초 동안 호기말 이산화탄소압, 우심방압, 동맥압을 측정하였다. 대상예의 평균 연령은 57±2세였고, 성별은 남자 12명, 여자 5명이었다. 심정지의 발생으로부터 심폐소생술이 시작될 때까지의 평균 시간은 28±5분이었다. 심정지의 원인은 심인성이 7예(41%), 비심인성이 2예(12%), 그리고 원인 불명이 8예(47%)이었다. 자발 순환이 회복된 예는 없었다. 압박기 대동맥압(표준압박: 95.2±41.8mmHg, 변경압박: 114.1±51.0 mmHg, p=0.001)은 변경압박에서 높았고, 이완기 대동맥압(표준압박: 27.9±16.7 mmHg, 변경압박: 27.6±17.8 mmHg, p=0.883)과 평균 대동맥압(표준압박: 50.7±22.8 mmHg, 변경압박: 53.1±27.6 mmHg, p=0.387)은 차이가 없었다. 압박기 우심방압(표준압박: 76.8±30.0 mmHg, 변경압박: 78.8±26.9 mmHg, p=0.703), 이완기 우심방압(표준압박: 13.0±8.8 mmHg, 변경압박: 13.4±8.8 mmHg, p=0.090), 평균 우심방압(표준압박: 33.6±12.7 mmHg, 변경압박: 35.3±11.3 mmHg, p=0.351), 그리고 관상동맥관류압(표준압박: 15.7±11.7 mmHg, 변경압박: 14.2±13.9 mmHg, p=0.398)에서도 유의한 차이가 없었다. 호기말 이산화탄소분압(표준압박: 9.6±6.9mmHg, 변경압박: 11.2±6.7 mmHg, p=0.020)은 변경압박에서 높았다. 결론적으로 흉부압박의 위치를 흉골의 하단으로 변경하였을 때 표준 위치에서 압박 할 때 보다 압박기 대동맥압과 호기말 이산화탄소압이 상승하였다. 따라서 현재 권장되고 있는 흉부 압박의 위치보다 흉골 하단에서 흉부압박을 시행하는 것이 심폐소생술에 의한 혈류량을 증가시킨다 [영문]Background and purpose: Scientific evidences are insufficient for current position of external chest compression recommended by International Guidelines for cardiopulmonary resuscitation and emergency cardiac care. With transesophageal observation, we have previously reported standard external chest compression might cause a narrowing of left ventricular outflow tract or aortic root and a pressure gradient between the left ventricle and the aorta, so that it might increase an afterload of the left ventricle. This study was to test whether compression of the lower end of the sternum as an alternative position (alternative compression) results in superior hemodynamic effects compared with standard external chest compression (standard compression). Methods: Seventeen patients with non-traumatic cardiac arrest, who failed to gain return of spontaneous circulation within 30 minutes after initiation of CPR, were enrolled. During CPR, a catheter was inserted to the right atrium via internal jugular vein and the descending thoracic aorta via the femoral artery, respectively. External chest compression was done at the lower end of the sternum for 3 minutes if ROSC was not attained after 30 minutes of standard CPR. A pressure from the right atrium and the aorta was measured and end tidal carbon dioxide tension was monitored during standard compression and alternative compression. Results: Peak aortic pressure during compression systole was higher with alternative compression than standard compression (95.2±41.8 vs 114.1±51.0 mmHg, p=0.001). End tidal carbon dioxide tension was higher with alternative compression than standard compression (9.6±6.9 vs 11.2±6.7 mmHg, p=0.020). There was no different in aortic pressure during compression diastole, peak right atrial pressure, and coronary perfusion pressure with standard compression and alternative compression. Conclusion: External chest compression at the lower end of the sternum might result in better hemodynamic effect than external chest compression at currently recommended position.-
