전기화상에 대한 임상적 고찰

dc.description.abstract[한글] 현대사회가 발달함에 따라 전기사용의 증가로 전기사고의 발생이 늘고 있다. 이 때 전기는 스파크로 인한 화염화상과 전류가 신체에 직접 접촉되어 발생되는 전류화상을 일으킬 수 있어, 이들 간의 임상양상이 다를 것으로 생각된다. 이전 연구들에서는 사고 전압의 고저에 따른 임상양상을 주로 비교하였고, 이에 본 저자는 전기에 의한 화상을 고전압 전류화상(제1군), 고전압 스파크화상(제2군), 저전압 전류화상(제3군), 저전압 스파크화상(제4군)으로 나누고 이들 간의 임상적 차이를 알아보고자 하였다. 본 고찰은 2000년1월에서 2002년12월 까지 한강성심병원 화상센터를 퇴원한 전기화상 환자 208명에 대한 의무기록연람을 통하여 환자의 나이, 성별, 사고시간, 화상센터 도착 시간, 타 병원 경유 여부, 동반손상, 사고 전압, 사고 장소, 전류의 인체 접촉 여부, 전기 입 출력부, 화상의 면적과 심도, 혈중 혹은 소변 내 myoglobin, 혈중 CK-MB, troponin I 및 심전도 이상, 기타 동반손상과 합병증, 총 수술 횟수와 재활의학과 의뢰 여부, 절단 및 신경손상 정도에 대하여 조사하였다. 자료를 바탕으로 분석한 결과 전기화상 환자의 93.8%가 남자로 평균연령은 33.8세이었으며, 24시간 내 병원 도착율은 88.9%이었으나, 저전압 전류화상군(제3군)은 67.6%가 남자이고, 평균연령은 15.6세, 24시간 내 병원 도착율도 76.5%로 낮았으며, 화상 면적도 1.6%로 가장 적었다. 제 1군은 화상 면적이 넓고, 3도이상의 환자가 많았으며, 동반손상 및 합병증, 수술횟수 및 재활의학과 의뢰, 근육손상, 심장손상이 많았으며, 절단과 신경 손상도 가장 많았다. 절단은 수부절단이 15예로 가장 많았고, 입력부의 절단이 28예로 출력부의 절단 17예보다 좀더 빈번했으며, 말초신경손상은 정중신경이 22예, 척골신경이25예 발생하였고, 완전손상은 정중신경에서 더 많이 발생하였다. 입력부에서의 말초신경손상이 22예로 출력부의 13예보다 좀더 빈번히 발생하였다. 결론적으로 전기화상은 전압과 전류화상 여부에 따라 다른 임상양상을 보이며, 이에 대한 치료 시 각각에 대한 적절한 치료 및 합병증에 대한 대비가 필요할 것으로 생각된다. [영문]Electric injury, which has become progressively more common recently with the increasing use of electricity. Electricity may cause a flame burn by electric spark and deep tissue and organ injury by current flow, there are significant differences in clinical manifestation between the spark and current injury. But in the previous studies the spark injury was not mentioned, so we divided electric burn injuries into 4 groups as group 1: high voltage current burn, group 2: high voltage spark burn, group 3: low voltage current burn, group 4: low voltage spark burn. And we investigated differences of clinical manifestations among the 4 groups. We reviewed 208 charts of electric burn patients discharged from Jan. 2001 to Dec. 2002 at Hangang Sacred Hospital Burn Center. We recorded the age, sex, accident time, arrival time at hospital, visit to another hospital prior to admission at our hospital, voltage of current, the inlet and outlet of the current flow, concomitant trauma or complications, burned body surface area(BSA), degree of burn, Mb in blood or urine, CK-MB, torponin I, EKG findings, the number of operation, rehabilitation consultation, amputation and nerve injury. The results are as follows: They were mostly men(92.3%), their mean age was 33.8yrs, and they mostly arrived at this hospital within 24hours of the injury(88.9%). There was no seasonal variations in electric accident. However in group 3, there were fewer men, their mean age was lower, fewer arrived at hospital within 24hours, mean BSA was narrower(1.6%) than the other groups. Group 1 had deeper burn wounds, higher prevalence of amputation, nerve injury, rehabilitation consultation, rhabdomyolysis and cardiac injury and more operations and complications. We concluded that we have to diagnose the character of the burn at admission, treat the patients properly and prepare for all possible following complications.-
