Expression of matrix metalloproteinase induction/activation system in human carotid atheroma

동맥경화반의 생성과 재형성, 미란이나 파열에 matrix metalloproteinases (MMPs) 작용을 비롯한 세포외 간질의 변화가 중요한 역할을 하고 있음이 밝혀지고 있다. 최근의 연구에 의하면 심부전 환자에서 MMP의 발현에 중요한 역할을 하는 MMP 유도체인 EMMPRIN이 과발현됨이 밝혀진 바 있다. 동맥경화반에서 MMP에 의한 세포외 간질의 용해가 급성 혈관사고의 병태생리에 관여하게 되는데, 동맥경화반에서의 MMPs의 유도 활성 시스템의 발현에 관하여는 연구가 많지 않다. 이 연구에서는 인체의 진행된 동맥경화반 조직내에서 MMP family와 EMMPRIN의 발현 여부를 면혁 형태학적 검사와 실험실 검사를 이용하여 알아보고 MMP 활성도와의 상관성을 알아 보고자 하였다.

9개의 carotid endarterectomy specimen에 EMMPRIN 항체를 이용한 면역조직 염색을 시행하였고, MMP-2, -9에 대한 면역조직 염색을 실시하여 두가지를 비교하였다. 대조군 조직은 동맥경화가 없는 대동맥 조직을 이용하여 비교하였다. Gelatin zymography를 이용하여 MMP-2, MMP-9의 활성도를 분석하였으며, Western blotting을 실시하였다. 또한 인체의 골수 단핵구 세포를 동맥경화 유발성 염증 사이토카인을 처리하여 배양한 후 실험실 방법으로 EMMPRIN과 MMPs 발현의 차이를 알아보았다.

EMMPRIN 항체를 이용하여 9개의 동맥경화반 조직에 면역 조직염색을 시행한 결과 모든 검체에서 EMMPRIN이 발현되었으며, 대조군에서 EMMPRIN의 발현은 미미하였다. 이들과 MMP 항체를 이용한 면역조직염색을 비교한 결과 EMMPRIN과 MMP의 colocalization이 확인되었다. Zymography와 Western blotting 결과, 인체 동맥경화반 조직에서 MMPs의 활성도가 증가하였고 EMMPRIN의 활성도가 대조군에 비하여 증가되어 있었다. 골수 단핵구세포의 배양에서 사이토카인을 처리한 군에서 대조군에 비해 EMMPRIN의 활성도가 증가되어 있었으며 MMPs의 활성도도 증가하였다.

결론적으로 MMP 유도체인 EMMPRIN은 인체 동맥경화반에서도 발현됨이 확인되었으며 죽상반내에서 MMP-2, -9과 공존하였다. EMMPRIN은 동맥경화 과정에서 단핵세포의 분화 및 죽상반 내로의 유입에 관여하며, 따라서 죽상반내의 MMPs의 활성을 유도하여 플라크의 안정도에 영향을 미침을 시사하였다. 향후 동맥경화반 내의 EMMPRIN의 역할과 MMPs 활성에 관한 정확한 규명을 위한 연구가 더욱 진행되어야 할 것으로 생각된다.

MT-1 MMP system that controls thte expression of MMPs exists in human myocardium and plays a role in the ventricular remodeling in heart failure. However, it remained unclear whether MMP induction

activation system exists in human atherosclerotic plaques. Similarly to heart failure, EMMPRIN may be expressed in human atheroma and play a role in extracellular matrix (ECM) remodeling and atherogenic cell differentiation. This study was to investigate the presence, activity and possible biological role of EMMPRIN in human atheroma. Immunohistochemical analysis for MMP-2, -9, and EMMPRIN were performed on human carotid endarterectomy specimens and control aortas. EMMPRIN showed significant immunoreactivity in human atherosclerotic carotid lesions, and was colocalized with macrophage infiltrates in atherosclerotic intima, plaque itself and vascular smooth muscle cells (VSMCs). Zymography and Western blot analysis were performed with the same specimens, which revealed EMMPRIN expression in the carotid atheromas, but not in the control aortas. Human bone marrow monocytes, which were cultured with the atherogenic proinflammatory cytokines revealed increased EMMPRIN and MMPs expressions by Western blot analysis. In conclusion, ECM remodeling is under the control of induction and inhibition of matrix degrading protease in atherosclerotic lesion. EMMPRIN may play a role in influx and differentiation of monocytes and destabilizing atheroma. Further studies involving EMMPRIN-mediated MMPs upstream-regulation are necessary to fully elucidate the role of EMMPRIN in the progression of atherosclerosis and vascular remodeling.
