Abstract
[한글]

골다공증은 낮은 골량과 뼈의 구조적인 노후화에 따른 골절의 위험도를 증가시키는 대사성 골질환으로, 평균 수명의 증가 및 노령화로 인한 경제적인 부담은 물론 사회적으로도 많은 문제를 초래케 하는 중요 질환이다. 그러나 현재 사용되는 약제들 대부분은 에스트로젠, 비스포스포네이트, 칼시토닌 등과 같이 골 흡수만을 주로 억제하는 약제들이다. 이들은 약제의 특성상 골량의 괄목할 만한 증가 없이 골 전환률을 감소시켜 지속적인 치료에도 불구하고 불과 50% 정도의 골절 감소 효과만을 보일 뿐이다. 따라서 골량을 크게 증가시켜 효율적으로 골절을 억제할 수 있는 즉 동화작용의 효과를 나타내는 새로운 물질의 개발이 절실히 요구되는 실정이다. 최근 미국 식품의약품 안전청은 뼈에 대한 동화작용을 나타내는 대표적인 후보 물질인 부갑상선 호르몬을 골형성 촉진제로 승인한 바 있다.

부갑상선 호르몬은 칼슘과 포스페이트 항상성의 주요 조절인자로서, 투여방법에 따라 골에 대한 동화작용 또는 이화작용 효과를 나타낸다. 즉 저 용량을 간헐적으로 투여 할 경우 뼈에 대하여 동화작용을 나타내고, 고 용량을 지속적으로 투여 할 경우는 이화작용을 나타낸다. 그러나 현재 미국 식품의약품 안전청이 승인한 인간 재조합 부갑상선 호르몬 [rhPTH(1-34)]은 펩타이드 제제이어서 주사제로만 투여해야 하는 단점을 가지고 있고, 백서에 대한 독성시험에서 뼈에 악성 및 양성종양을 유발하는 등 임상적으로 널리 사용되기 위해서는 반드시 해결해야 하는 몇 가지 문제점들이 있다. 이에 본 연구에서는 부갑상선 호르몬의 활성도를 나타내는 최소 크기의 유도체인 PTH(1-11) 또는 PTH(1-12)를 기본으로 한 peptide array를 합성 제작하여 이들의 생물학적인 활성도를 측정하고, circular dichroidism 및 핵자기 공명 구조를 비교 검토함으로써, 향후 보다 약물 투여가 용이하고 환자의 적응도를 높일 수 있는 부갑상선 호르몬 유도체의 도출을 위한 기초 자료를 얻고자 하였다.

PTH/PTHrP 수용체를 LLC-PK1 세포주에 칼슘 포스페이트법으로 transfection 시킨 후 neomycin 투여에 의한 선택적 분리과정을 거쳐 stable cell lines을 만들었다. PTH 유도체의 경우, 기존의 연구들에서 아미노 터미널 잔기들이 수용체 결합에 결정적이어서 다른 아미노산기로 치환 시 활성을 잃는 것으로 알려져 본 연구에서는 PTH(1-11) 또는 PTH (1-12)의 아미노 터미널은 그대로 보존하면서 카복시 터미널 잔기, 특히 7, 8, 10, 그리고 11번째 잔기를 집중적으로 치환하였다. PTH(1-12)의 제 10번째 염기의 치환 결과 cAMP를 증가시키는 활성도 순서는 Ala Asp Gln > Glu순으로 나타났으며, 제 11번째 잔기는 Arg > Met Lys > Ile순으로 나타났다. PTH(1-11) 유도체는 첫번째, 7번째, 그리고 8번째 잔기를 다양하게 치환한 결과 잔기의 유형에 따라 다양한 cAMP 활성화 정도를 보였다. 특히, 제 10번째 Ala 잔기를 Glu로, 제 11번째 Arg 잔기를 Ile로 치환하였을 때 급격히 활성도가 떨어졌으며, 제 10번째와 11번째를 Glu10/Ile11로 치환하였을 때는 완전히 활성도를 잃어버렸다. 한편 PTH(1-12) 유도체의 경우, 제 7 번째 염기 Leu을 Phe으로 치환하면 전반적으로 활성도가 줄어들었지만 PTH(1-11) 유도체의 경우에는 상기 치환이 활성도의 차이를 보이지 않았다. 흥미롭게도 PTH(1-11) 유도체에서 첫 번째 잔기 Ala을 Ser으로 치환하였을 때 그 생물학적인 활성도를 잃어버렸다. 그러나 유도체들의 핵자기 공명 구조는 다양한 활성도와는 달리 흥미롭게도 거의 유사한 α-helical backbone 구조를 가지고 있었다. 특기할 만한 것은 [Ala3,10,12(Leu7/Phe7)Arg11]rPTH(1-12)NH2의 경우 안정된 α-helical 구조를 가지고 있는 반면, 10번째 염기가 Glu10으로 11번째 염기가 Ile11로 치환된 유도체의 경우 불안정한 α-helical 구조를 보였다.

요약컨데, 부갑상선 호르몬의 앞부분에 해당되는 PTH(1-11) 또는 PTH(1-12) 유도체들은 α-helical 구조를 보이며, α-helical 구조의 불안정성이 생물학적인 활성도를 저하한다는 것도 알 수 있었다. 그러나 같은 α-helical 구조를 갖더라도 각 염기 잔기에 따라 활성도가 크게 차이가 나는 점으로 미루어 backbone 구조 이외에도 각 잔기의 side chain orientation이 중요한 것을 알 수 있다. 따라서 향후 α-helical 구조를 유지하면서 PTH/ PTHrP 수용체에 결합할 수 있는 최적의 side chain orientation을 갖는 자연계에 존재하든 또는 존재하지 않는 잔기로의 치환을 통하여 강력한 부갑상선 호르몬 유도체의 개발이 가능할 것으로 사료된다.





[영문]Osteoporosis is characterized by low bone mass, structural deterioration of bone and increased risk of fracture. The prevalence and cost of osteoporosis is increasing dramatically with increasing of aging population. All currently approved therapies for osteoporosis are anti-resorptive agents that act on osteoclasts to prevent further bone loss: estrogen, bisphosphonates, calcitonin, and selective estrogen receptor modulators. Anti-resorptive agents stabilize the bone remodeling by reducing the number and activity of osteoclast, and thereby reducing the risk of fracture without increments of true bone mass. In contrast, anabolic agents are not only to increase bone mass remarkably but also reduce fracture risk extraordinary. Therefore, it is necessary to develop the anabolic agents acting on bone.

Parathyroid hormone (PTH) is vital for the regulation of calcium and phosphate homeostasis. PTH has both anabolic and catabolic effects on bone,depending on the route of administration. The anabolic effects of PTH on bone density have prompted considerable interest in the development of new PTH-1 receptor agonist analogues. Based on these understanding, PTH analogues have been regarded as one of most important candidates for treating osteoporosis.

In order to study the structure and function of short-length amino terminal PTH analogues, the cAMP generating activity of chemically synthesized PTH(1-11) and PTH(1-12) oligopeptides were analyzed in LLC-PK1 cell lines stably transfected with the wild-type human PTH-1 receptor, and performed NMR studies. Replacement of the 10th residue with Ala, Asp, Gln, or Glu, and the 11th residue with Arg, Met, Lys, or Ile in both [Ala3,10,12Leu7Arg11]rPTH(1-12)NH2 and [Ala3,10,12Phe7Arg11]rPTH(1-12) NH2 produced different magnitude of cAMP generating potency, dependent upon the residue type. Especially, the substitution of Ala10 of [Ala3,10,12(Leu7/Phe7)Arg11]rPTH(1-12)NH2 with Glu10 and/or the Arg11 with Ile11 markedly decreased cAMP generating activity. The substitution of Leu7 with Phe in most of the [Ala3,10,12Leu7Arg11]rPTH(1-12)NH2 analogues reduced the cAMP formation. The substitution of Leu7 with Phe in [Ala3,10Leu7Arg11] rPTH(1-11)NH2 did not. The substitution of Ala1 by Ser in [Ala3,10Leu7Arg11] rPTH(1-11)NH2 caused a near abrogation of cAMP formation. NMR analysis of both [(Ala1/Ser1)Ala3,10(Leu7/Phe7)Arg11]rPTH(1-11)NH2 and [Ala3,10,12(Leu7/Phe7)Arg11]rPTH(1-12)NH2 revealed a similar α-helical backbone structure and a more flexible conformation at the carboxyl-terminus. Data from circular dichroism and the NMR structural analysis of [Ala3,10,12(Leu7/Phe7) Arg11]rPTH(1-12)NH2 revealed stable α-helical structures, while the Glu10 and

or Ile11 substituted analogues showed unstable α-helical structures. The overall results suggest that short oligopeptides, based on the first 11 or 12 amino acids of PTH, tend to form α-helical structure, and that the different potency of these analogues is probably associated with residue specificity rather than secondary structure. In addition, 10th and 11th residues are important for stabilizing its helical conformation and destabilization of α-helical structure, induced by substituting the residues, remarkably affect its biological potency.
