척수손상 흰쥐의 방광에서 Nerve Growth Factor (NGF)의 변화와 capsaicin이 NGF에 미치는 영향

목적: Nerve growth factor (NGF)는 방광을 포함한 많은 기관의 자율신경지배에 중요한 역할을 하는 것으로 되어 있으나 민말이집신경섬유가 배뇨반사에 관여하는 척수절단에서 NGF가 방광과반사에 관여하는 기전에 관한 연구는 미비한 상태이다. 본 연구는 척수손상이라는 병적 방광 상태에서 NGF가 배뇨반사에 관여하는 신경세포 형성성에 대한 기전을 규명하고 capsaicin이 방광의 향신경인자에 미치는 영향을 살펴보고자 한다.

방법: 흰쥐에서 척수절단 4일째인 급성기와 6주째인 만성기에 방광의 NGF는 ELISA를 통해 측정하였고 NGF mRNA는 RT-PCR로 측정하였다. capsaicin이 향신경인자에 미치는 영향을 보기 위해 급성기군은 척수절단 전에 capsaicin을 전처치하였고 만성기군은 지속적인 민말이집신경섬유의 탈감작을 위해 반복적으로 capsaicin을 처치한 후 NGF와 NGF mRNA를 비교하였다. 각 군에서 표적장기의 생리평가를 위해 방광내압측정을 시행하였다.

결과: 척수절단 후 방광의 NGF는 척수손상 후 장기간 (6주) 경과한 후에 증가하였다. 방광의 NGF mRNA는 무반사상태인 척수절단 급성기에 증가하고 시간이 경과하면서 감소하였다. 척수절단 후 capsaicin의 반복적 투여는 척수절단 후 발생하는 배뇨근 비억제성 수축을 봉쇄할 뿐만 아니라 방광의 NGF를 감소시켰다. 척수절단 급성기에 capsaicin을 전처치하면 방광의 NGF mRNA 생성이 감소하였다.

결론: 척수절단 후 방광에서 NGF 단백과 NGF mRNA의 변화를 살펴봄으로써 NGF가 신경세포 형성성에 관여함을 확인할 수 있었고 표적기관인 방광에서 NGF가 역방향으로 축삭수송을 통해 배뇨반사에 관여함을 추정할 수 있었다. 또한 C 민말이집신경섬유의 탈감작을 통해 배뇨근과다반사를 봉쇄하는 것으로 알려진 capsaicin이 배뇨반사의 형성성에 관여하는 NGF mRNA 생성을 차단한다는 새로운 역할을 확인할 수 있었다.

INTRODUCTION: NGF is responsible for the growth and maintenance of sympathetic and sensory neurons. However, the past emphasis on NGF in mediating the neuronal plasticity was associated with urethral obstruction and cystitis. This study investigated the role of NGF in the mechanism underlying the neuronal plasticity and the effect of capsaicin on bladder NGF in the spinalized rats.

MATERIALS AND METHODS; Thirty Sprague-Dawley rats were divided into normal controls (n=5); capsaicin treated normal rats (n=5), spinalized rats (n=10), capsaicin treated spinalized rats (n=10). Spinalized rats were subdivided into rats with acute SCI (4 days, n=5) and chronic SCI (6 weeks, n=5). In chronic SCI, capsaicin injection was repeated for ablation of unmyelinated C fiber. The bladder NGF was measured by enzyme-linked immunosorbant assay (ELISA) and the NGF mRNA was determined by reverse transcription-polymerase chain reaction (RT-PCR).

RESULTS; The bladder NGF following acute SCI was significantly lower compared to the controls. However, the bladder NGF increased significantly following SCI for longer periods of time (6 weeks). Rats with acute SCI exhibited the largest increase in NGF mRNA levels. But, the NGF mRNA in chronic SCI was significantly lower compared to the rat with acute SCI. The repeated capsaicin injection decreased the amplitude of detrusor contraction and bladder NGF in rats with chronic SCI. The capsaicin injection before SCI also decreased the NGF mRNA in rats with acute SCI (4 days).

CONCLUSION; These study suggest that significant alterations of NGF mRNA and NGF following SCI are likely to mediate bladder afferent neuron plasticity. Discrepancies between NGF mRNA and NGF in bladder may reflect retrograde axonal transport of NGF to the dorsal root ganglia and retrogradely transported NGF may play a role in reorganization of the micturition reflex. Also decrease in NGF mRNA levels after capsaicin treatment suggest the possibility that capsaicin effect on detrusor is associated with alteration of bladder NGF.
