운동의 상상이 피질척수계의 흥분도와 상반억제에 미치는 영향

dc.description.abstract[한글] 운동피질의 경두개자극에 의해 발생된 운동유발전위가 수의적인 근수축에 의하여 촉진된다는 것은 잘 알려져 있다. 건강한 22명의 지원자를 대상으로 경두개자극기를 이용하여 운동의 상상이 피질척수계의 흥분도를 반영하는 운동유발전위의 잠복기와 내수지근에서의 상반억제에 미치는 영향을 알아보고자 하였다. 경두개자극은 먼저 휴식 상태에서 시행한 다음 피검자가 자신의 오른쪽 엄지손가락의 외전 또는 내전을 상상하는 동안 시행하였다. 각각 두개와 경부자극을 하였고 중추운동전달시간을 얻었다. 운동유발전위는 우측 단무지외전근과 무지내전근에서 동시에 기록하였다. 두개자극을 하는 동안, 외전의 상상은 단무지외전근에서는 잠복기의 단축을 무지내전근에서는 잠복기의 연장을 가져왔고, 반대로 내전의 상상은 무지내전근에서는 잠복기의 단축을 단무지외전근에서는 잠복기의 연장을 가져왔다. 그러나 경부자극에 의해서 잠복기의 변화는 관찰되지 않았다. 그러므로 중추운동전달시간의 변화에 의하여 이러한 잠복기의 변화가 나타남을 알 수 있었다. 외전의 상상에 의하여 단무지외전근의 중추운동전달시간은 10.8%가 감소하였고 무지내전근의 중추전달시간은 5.8%가 연장되었다. 반면에, 내전의 상상에 의하여 단무지외전근의 중추운동전달시간은 7.3%가 연장되었고 무지내전근의 중추운동전달시간은 5.9%가 감소하였다. 이러한 결과로 운동의 상상이 사람의 피질척수로의 흥분성을 증가시킴을 알 수 있었다. 그리고 상반억제로 길항근에서의 잠복기의 연장이 나타난다고 추측된다. [영문] Objective: It is well known that motor evoked potentials(MEP''s) elicited by transmagnetic stimulations(TMS''s) of the motor cortex are facilitated by voluntary muscle contraction. We evaluated the effects of the imagination of the movements on MEP latencies and reciprocal inhibition. In these studies, we investigated reciprocal inhibition during thinking about movement by using TMS. Methods: Twenty two healthy volunteers (eight men and fourteen women) were studied. TMS''s were delivered at rest and during imagining abducting or adducting right thumb. A stimulator with a round coil and a fixed intensity of 80% of maximum was used to evoke MEP''s. MEP''s were evoked by magnetic stimulations over the scalp and cervical spine(C7-T1), and central motor conduction times(CMCT''s) were calculated by subtracting the latency of compound muscle action potentials (CMAP''s) obtained by stimulation over the cervical spine from that obtained by stimulation over the scalp. MEP''s were recorded from right abductor pollicis brevis muscle(APB) and adductor pollicis muscle(AP) simultaneously. Results: Imagination of abduction resulted in a shortened latency of the CMAP''s in APB muscle, and a prolonged latency in AP muscle. Imagination of adduction resulted in a shortened latency in AP muscle, and a prolonged latency in APB muscle. But the imagination caused no significant change in the latency of CMAP''s elicited by stimulation over cervical spine. Therefore, the changes of the CMCT''s account for these latency changes with imagination of movement. With the imagination of abduction, there are significant reduction of the CMCT''s in APB muscle(10.8%) and prolongation in AP muscle(5.8%). On the other hand, with the imagination of adduction, prolongation of the CMCT''s in APB muscle(7.3%) and reduction in AP muscle(5.9%) were observed. Conclusion: These findings indicate that imagination of movement increases the excitability of the human corticospinal system. Imagination-induced increase in excitability within the central nervous system may account for facilitation, and reciprocal inhibition may be accountable for the prolonged latency in the antagonist muscle.-
