dc.description.abstract[한글] 교정치료에서 고정원의 조절은 교정 장치의 선택과 적용에서 가장 중요한 요소 중 하나이다. Head gear와 같은 구외 장치는 고정원의 보강에 매우 효과적이지만 환자의 협조를 구해야 한다는 문제를 갖고 있다. Gainsforth와 Higley(1945)가 vitallium screw를 개에 매식하여 교정학 영역에서 고정원의 목적으로 처음 시도하였으나 감염에 의한 탈락의 문제가 있었다. 이후 1960년대에 Brånemark에 의해 골유착(osseointegration)의 개념이 소개됨에 따라 Linkow(1969), Smith(1977), Sherman(1978), Gray 등(1983), Turley 등(1988) 선학들에 의해 교정학에서 고정원으로서의 endosseous implant가 연구되었다. 특히 Roberts등은 수년에 걸친 동물 실험(Roberts 등, 1984, 1989)과 임상 보고(Roberts 등, 1990)에 의해 implant가 고정원으로서 사용될 수 있고 가치가 있음을 입증하였다. 그러나 이러한 implant들은 크기가 큼으로 골조직의 양이 충분하여야 하는 시술 부위에 대한 제한과 골유착의 발생을 기다려야 하는 시간적인 제약이 있었다. Creekmore와 Eklund(1983)가 vitallium bone screw를 삽입한 10일 후 교정력을 적용하여 치료한 증례를 발표하였고 Block과 Hoffman(1995)이 onplant를 소개한 이후 고정원으로서 사용에 제약을 가지고 있던 endosseous implants에서 생적합적이고 비용이 비싸지 않으며 국소 마취 하에서 삽입과 제거가 쉽고 구강내 어느 부위에도 위치 시킬 만큼 크기가 작으면서도 시술 후 몇 일 만에 교정력을 적용 할 수 있으며 적용하는 교정력에 충분히 견딜 수 있는 순수한 고정원 목적으로의 implant에 관심을 가지게 됐다. Wehrbein 등(1996, 1998)은 상악 구개 전방부에 삽입하는 orthosystem을 통해 크기가 많이 줄어든 implant를 보고하였고 Kanomi(1997)는 삽입과 제거가 쉬우며 더 크기가 작아진 mini-implant를 소개했다(Ohmae 등, 2001). 그 후에 mini-plate(Umemori 등, 1998), miniscrew(Costa 등, 1998), micro-implants(Park 등, 2002) 등 공간적 그리고 시간적 제약을 줄이고자 교정용 미니스크류는 다양하게 연구(경 등, 2001)되고 있으며 매우 빠르게 발전하고 있다. 일반적인 교정력은 200-300g을 넘지 않으나 implants에 악정형력을 적용한 연구(De Pauw 등, 1999) 및 증례(Singer 등, 2000)와 400g 정도까지 교정력을 적용하여 치료한 경우(Higuchi와 Slack, 1991)도 보고되고 있어 교정용 미니스크류는 100-500g 정도의 수직 방향 보다는 수평 방향의 교정력에 저항할 수 있는 합당한 크기와 적절한 디자인이 유용하다. 그러나 미니스크류의 형태가 잘못 디자인 되었다면 과도한 교정력이 특정 부위에 집중되어 병적인 골흡수(pathologic bone resorption)같은 문제가 발생될 수도 있으며(Geng 등, 2001), 필요 이상으로 크기가 클 경우에는 미니스크류의 삽입 시 적절한 위치의 선택에 제한을 받거나 삽입시의 실수로 변연치조골(marginal alveolar bone)의 골절(fracture)과 같은 원치 않는 상황이 초래될 수도 있다. 그럼에도 불구하고 미니스크류 자체의 디자인에 대한 연구는 미미한 실정이며 미니스크류의 직경, 길이, 나사산 거리 등에 따라 유지력을 연구한 보고들이 있으나 수평 방향의 측방력 보다는 pull-out force에 의한 것(You 등, 1994)이고 기존의 시판되는 bone screw를 이용함으로써(Saka, 2000) screw의 디자인에 있어 매개 변수에 따른 변화를 동일 조건 속에서 비교할 수 없는 제한들이 있다. 본 연구는 미니스크류에 원하는 크기의 교정력을 적용시킬 때 디자인에 따라 주위 골조직에 응력이 어떻게 분산(distribution)되는 가를 알아보고자 시행되었다. 미니스크류의 디자인에 있어 매개 변수에 따른 변화를 동일 조건 속에서 비교하기 위해 유한요소분석(finite element analysis, FEA)을 사용하여 분석하였고 다소의 지견을 얻었기에 이에 보고하는 바이다 [영문] In this study, Stress distribution according to diameter, length, pitch , screw thread shape which were selected among many factors affecting primary stability of the miniscrew were compared by using the Finite Element Analysis(FEA) with the properties of stainless steel to investigate stress distribution in surrounding bone according to design, when applying the appropriate orthodontic force on the miniscrew. However secondary stability factors such as healing, bone formation, osseointegration was excluded. The screw thread shape did not show a spiral form, but rather a ring form and assuming that the cortical and cancellous bone were isotropic , the stress distribution was compared according to the design of the miniscrew and the optimum case(optimum geometry) was calculated by each parameter. The case that screw thread shape was spiral form and trabecular bone was reconstructed by assuming the cancellous bone was anisotropic was compared to the previous optimum geometry. There may be slight difference between the actual intraoral operation environment and this study environment due to the much presumption according to the various condition, however it was helpful in analyzing the pattern and the following results were gathered. 1. Stress change by the change in diameter showed the largest value. 2. Stress change by the change of length was not large enough. There was no difference over 8mm in length. 3. As pitch decreased, stress at the highest point of cortical bone decreased. However, in 0.4mm, the stress at the tip of the first screw thread increased remarkably, therefore it showed very unstable distribution. 4. In the case which screw thread shape was triangular, the smaller the point angle was the larger stress change was at the tip of the first screw thread. Dull screw thread had better stress distribution. In addition, according to the shape of thread, stress distribution varied widely. 5. In most cases, the largest stress value was marked at the tip of first screw thread, not at the top of cortical bone. However, there was no specific relationship between stress change and parameters at these two points. According to the fact that stress was concentrated on these two points, the bone quality at the highest point in cortical bone is most important. 6. Second screw thread had little effect on stress distribution. Reconstruction of experimental model has limitations and there are some differences between the actual application and the experimental model, because the stress in early phase of the orthodontic force applied was only observed. However, looking at the experimental results, it is good for the miniscrew that will be implanted in the maxillary posterior teeth to have a large diameter, but 1.4-2.0mm is appropriate diameter according to the applied orthodontic force and 6.5-7.0mm is sufficient length for anchorage. In addition, it is better for the stress distribution to position two screw threads, each being approximately 0.5-0.6mm in length in the cortical bone, the tip being dull. and the angle being large.-
