포르말린 유도 과민통 반응에서 척수 내 glutamate성 및 GABA성 체계의 역할

dc.description.abstract[한글] 말초 조직의 손상은 염증반응을 생성하며 이때 많은 경우는 염증성통증을 동반한다. 실험적으로 염증성통증을 유발하기 위해서 포르말린의 피하주사가 이용된다. 포르말린 주사는 특징적으로 두 차례에 걸친 통증반응기(약 5분간의 1차 반응과 35-40분간의 2차 반응)와 각 반응 사이에 10-20분간 통증이 억제되는 중간기로 구성됨이 행동학적 실험에서 밝혀졌다. 이러한 반응기 양상은 척수 내 통각정보전달 신경세포의 발화를 기록한 전기생리학적 실험에서도 유사하게 관찰되었다. 포르말린 주사 후 비교적 장시간 지속되는 2차 통증반응은 중추신경의 감작(sensitization) 때문이며 이러한 감작에는 글루타민산 수용체, 특히 NMDA 수용체의 활성화가 관여함은 잘 알려져 있다. 그러나 2차 반응에 다른 글루타민 수용체 아형의 작용이 관여할 수 있음도 제시되었으나 구체적으로 밝혀진 바 없다. 또한 중간기의 형성 요인에 관해서는 전혀 관심의 대상이 되지 않고 있다. 본 연구는 쥐를 이용한 척수동물(L1 절단)에서, 뒷발에 포르말린 피하주사 후 척수(L5) 내 통각정보전달 신경세포로부터 관찰되는 두 반응기 및 중간기의 생성에 대해 글루타민산 수용체 아형(NMDA, AMPA 및 mGlu 수용체)과 GABA 수용체 아형(GABA-A와 GABA-B 수용체)의 역할을 조사하여 다음의 결과를 얻었다. 1. 포르말린에 의해 척수후각 내 통각정보전달 신경세포에서 생성되는 1차 발화는 포르말린 주사 전에 척수에 가해준 AMPA 수용체 길항제에 의하여 억제되었다. 2. 포르말린에 의해 생성되는 1차 발화와 2차 발화 사이에 형성되는 중간기는 포르말린 주사 전에 척수에 가해준 GABA-A 및 GABA-B 수용체의 각 길항제 모두에 의해 그 형성이 억제되었으며, 이들 길항제의 중간기 동안 처치는 처치 시점에서 2차 발화의 생성을 촉진하였다. 3. 포르말린에 의해 생성되는 2차 발화는 포르말린 주사 전 가해준 NMDA 및 mGlu 수용체의 각 길항제 모두에 의해서는 그 유도가 억제되었고, AMPA 수용체 길항제에 의해서는 억제되지 않았으나, 2차 발화 초기에 척수에 가해준 GABA-A 및 GABA-B 수용체의 각 효현제 모두에 의하여서는 그 유도가 억제되는 경향을 보였다. 4. 포르말린에 의해 생성되는 2차 발화는 발화 기간 중에 척수에 가해준 NMDA, AMPA, 및 mGlu 수용체의 각 길항제 모두에 의하여 그 유지가 억제되었으며, GABA-A 및 GABA-B 수용체의 각 길항제 모두의 처리에 의해서는 그 유지됨이 영향 받지 않았다. 이상의 결과로부터 1) 포르말린 피하 주사에 의해 생성되는 요수 내 통각정보전달 신경세포의 1차 발화는 일차구심성 섬유 말단에서 분비된 글루타민산의 작용에 의한 AMPA 수용체의 빠른 활성을 통해 생성되며, 2) 뒤 이은 억제성 GABA 신경의 흥분과 그 결과 분비된 GABA 작용에 의한 GABA-A 및 GABA-B 수용체의 활성화를 통한 발화 억제가 중간기를 형성하고, 3) 2차 발화는 그 유도에 있어서 중간기 때 흥분되었던 GABA 신경의 흥분성 소멸과 더불어 글루타민산에 의한 NMDA 수용체 및 mGlu 수용체의 활성화가 관여하며 그 유지에 있어서는 글루타민산 작용에 의한 NMDA 수용체, AMPA 수용체 및 mGlu 수용체 모두의 활성화가 관여함을 알 수 있었다 [영문] Peripheral tissue injury often produces inflammatory response, which is frequently accompanied by the inflammatory pain. Subcutaneous injection of formalin is used experimentally to induce inflammatory pain. Behavioral study showed that formalin injection resulted in biphasic pain responses, in which an initial 5-min pain response was followed by the second phase lasting for 35-40 minutes in association with no pain of 10-20 minutes of interphase between two pain responses. The similar response pattern was observed in impulse firing of spinal nociceptive neurons in response to peripheral formalin application. It has been known that the formalin-produced long-lasting second response is due to sensitization of the central neuron by the action of glutamate on its receptors, particularly NMDA receptor subtype. However, a role of other glutamate receptor subtypes in the generation of the second response is not clearly understood yet. Furthermore, factors influencing the interphase following formalin injection have not received much attention. In the present study, we investigated the role of the glutamatergic (NMDA, AMPA, and mGlu) receptors and GABAergic (GABA-A and GABA-B) receptors in the generation of formalin-produced two firing phases and the interphase of spinal nociceptive neurons in rats spinalized at L1 level. 1. Formalin-produced first firing of nociceptive neurons in lumbar spinal cord dorsal horn is suppressed by AMPA receptor antagonist applied on lumbar spinal cord surface prior to formalin injection. 2. Antagonists for GABA-A and GABA-B prevented the interphase between the first and the second phase from forming when applied prior to formalin injection, and allowed the second firing to occur when applied at a certain time point during the interphase. 3. Induction of formalin-produced second firing was blocked by antagonists for NMDA and mGlu receptors, not by AMPA receptor antagonist, when applied before the formalin injection. This induction was suppressed by agonists for GABA-A and GABA-B receptor when applied at the beginning of the second firing, 4. Maintenance of formalin-produced second firing was blocked by antagonists for NMDA, AMPA, and mGlu receptors while being unaffected by antagonists for GABA-A and GABA-B receptors when applied during the second firing. The results indicate 1) that formalin-produced first firing of nociceptive neurons in the lumbar spinal cord is mediated by AMPA receptor activation via action of glutamate released from central terminals of primary afferents, 2) that following ceasing the first firing, excitation of inhibitory GABAergic neurons occurs and causes the GABA release to activate GABA-A and GABA-B receptors, which contributes to formation of the interphase by ceasing the firing, and 3) that formalin-produced second firing is mediated in its induction by diminution of GABAergic exitation as well as activation of NMDA and mGlu receptor while being mediated in its maintenance by activation of NMDA, AMPA, and mGlu receptors.-
