동맥경화증과 사구체경화증은 신장이식후 발생하는 만성이식신 기능부전의 주요 병리소견으로, 각각 혈관 평활근세포와 사구체 혈관간세포의 증식과 세포외기질의 침착에 의하여 초래되는 것으로 알려져 있다. Mycophenolic acid(MPA)와 rapamycin(RPM)은 혈관 평활근세포의 증식이나 세포외기질의 침착을 억제하는 것으로 알려진 반면 사구체 혈관간세포에서의 효과에 대하여서는 알려진 바가 적다. 이에 연구자는 MPA와 RPM이 사구체 혈관간세포의 증식과 세포외기질의 생성에 미치는 영향과 작용기전을 검색하고자 하였다.

흰쥐의 사구체에서 분리배양한 혈관간세포를 platelet-derived growth factor(PDGF)로 증식을 유도하였다. 혈관간세포에 대한 증식억제효과를 확인하고자 MPA(10 nM ~ 10 µM)와 RPM(0.1 nM ~ 1 µM)을 각각 단독 투여하였다. 혈관간세포의 증식정도와 collagen 생성정도는 각각 [3H]thymidine incorporation과 [3H]proline incorporation으로 측정하였으며, fibronectin의 생성과 ERK 및 p38 MAPK의 활성화는 Western blot분석으로 측정하였다.

MPA와 RPM은 모두 농도의존적으로 PDGF로 유도된 사구체 혈관간세포의 증식을 억제하였고, IC50은 각각 500 nM 과 100 nM이었다. PDGF에 의한 collagen 생성은 MPA와 RPM 모두에서 농도의존적으로 억제되었으나, fibronectin의 생성은 RPM에 의하여 억제되지는 않았다. MEK 억제제(20, 50 μM) 와 p38 MAPK 억제제(1, 10 μM)는 혈관간세포의 증식이나 세포외기질의 생성을 억제하여, 혈관간세포의 증식이나 세포외기질의 생성에는 ERK나 p38 MAPK의 활성화가 관련되어 있음을 제시하였다. MPA는 혈관간세포에 대한 증식억제효과를 보이는 농도(10 nM 이상)에서 농도의존적으로 ERK와 p38 MAPK의 활성화를 억제한 반면, RPM은 10 nM 이상의 농도에서도 ERK와 p38 MAPK의 활성화를 억제하지 못하였다. 이는 MPA의 증식억제효과가 ERK나 p38 MAPK의 활성화 억제와 관련이 있으나, RPM의 증식억제효과는 ERK나 p38 MAPK의 활성화와는 관련이 없었다는 것을 의미하고 있다.

결론적으로 MPA와 RPM은 PDGF로 유도된 사구체 혈관간세포의 증식을 억제함은 물론 collagen의 생성을 억제하므로, 이들 약제들이 면역억제제 뿐만 아니라 만성이식신 기능부전의 치료제로 사용될 수 있는 실험적인 근거를 제시하여 주고 있다. 아울러 MPA와 RPM의 작용기전이 서로 다를 수 있음을 시사해 주고 있다.



[영문]



Excess proliferation and extracellular matrix (ECM) accumulation of mesenchymal cells such as vascular smooth muscle cells (VSMC) and glomerular mesangial cells cause chronic allograft nephropathy including transplant vascular sclerosis and glomerulosclerosis. Mycophenolic acid (MPA) and rapamycin (RPM) are well known as strong inhibitors of VSMC proliferation, but not clearly understood in mesangial cells. This study examined the effects of MPA or RPM on PDGF-induced proliferation or ECM accumulation in rat glomerular mesangial cells.

Materials and methods: Mesangial cells isolated from the glomeruli of Sprague-Dawley rats were cultured with DMEM containing 20% fetal bovine serum. Growth arrested and synchronized cells were administered with test drugs (MPA 10 nM ~ 10 µM , RPM 0.1 nM ~ 1 µM) before the addition of PDGF 10ng/ml. Cell proliferation was assessed by [3H]thymidine incorporation, collagen by [3H]proline incorporation, and fibronectin, ERK, and p38 MAPK by Western blot analysis.

Results: PDGF stimulated the proliferation of mesangial cells effectively. PDGF increased mesangial cell proliferation by 4.64 fold. Compared to stimulated control, MPA above 500 nM and RPM above 10 nM showed a significant inhibitory effect in a dose-dependent manner. The IC50 of MPA and RPM against PDGF-stimulated mesangial cell proliferation were around 500 nM and 100 nM respectively. The collagen synthesis was also inhibited by MPA and RPM, but the fibronectin secretion was inhibited by MPA alone. The proliferation of mesangial cell correlated with activation of ERK and p38 MAPK. MPA, but not RPM showed inhibitory effect on the activity of ERK and p38 MAPK.

Conclusions: This study demonstrated that MPA and RPM significantly inhibited PDGF-induced proliferation and ECM production in rat glomerular mesangial cells. The inhibitory effects of MPA, but not RPM, are correlated with ERK and p38 MAPK.
