측두엽 간질에서 비침습적인 수술 전 평가로서 발작 양상 및 발작 중 두피 뇌파가 이용되고 있으며 그 정확도와 유용성은 이미 밝혀져 있다. 측두엽 간질 환자에서 발작 간 간질양 뇌파가 일측 측두엽에 국한된 경우(unitemporal interictal epileptiform discharges, UIED)가 양측 측두엽에 독립적으로 나타나는 경우(bitemporal interictal epileptiform discharges, BIED)보다 수술 부위의 측위화 및 수술 후 예후가 양호한 것으로 알려져 있으나 두 군에서의 발작 양상 및 발작 중 두피 뇌파로 측위화했을 때의 정확도를 비교한 연구는 드물다.
체계적인 약물치료를 시행하였음에도 불구하고 발작이 지속되는 난치성 측두엽 간질로 1999년 3월부터 2002년 2월까지 연세의료원 간질 크리닉에서 지속적인 비디오 및 뇌파감시를 시행하여 전측두엽 절제술을 시행한 48명의 환자에서 287회의 발작을 분석하여 UIED군
3.발작내내 발작파가 일측 측두엽이나 반구에 국한된 경우는 UIED가 있는 20명의 환자(62.5%)에서 나타났고 BIED가 있는 4명의 환자(25.0%)에서 나타나 UIED가 있는 환자에서 유의하게 많았다(P<0.05). 측위화의 전환은 UIED가 있는 경우 2명의 환자(6.3%)에서 나타났고 BIED가 있는 경우 3명의 환자(18.8%)에서 나타나 BIED가 있는 환자에서 유의하게 많았다(P<0.05).
4.양측 측두엽에서 독립적으로 발생하는 발작은 UIED가 있는 환자 1명(3.1%)과 BIED가 있는 환자 4명(25.0%)에서 나타났는 데, 두 군간에 통계적으로 유의한 차이를 보였다(p<0.05).
위의 결과로부터 UIED가 있는 환자와 BIED가 있는 환자에서 발작 중 두피 뇌파가 갖는 측위화의 정확도 및 발작 중 뇌파의 소견이 다르다는 것을 알 수 있었다. 그러나 발작 양상의 측위화에 있어 차이는 발견하지 못하였다.

Ictal semiology and scalp EEG are most often used and relied-on methods of noninvasive presurgical evaluation in TLE. Several studies have confirmed the accuracy and reliablility of two methods in the lateralization of intractable TLE. But there were few study to investigate the lateralizing value of ictal semiology and scalp EEG in patients with unitemporal (UIED) versus bitemporal (BIED) interictal epileptiform discharges.
We retrospectively analyzed semiology and ictal scalp EEG of 287 seizures in 48 patients who had anterior temporal lobectomy from March 1999 to February 2002.
Patients were classified as UIED(n=32) or BIED (n = 16) group. There were 201 seizures in UIED patients and 86 seizures in BIED patients. Clinical seizure lateralization was correct in 103 seizures (51.2%) and 19 patients (59.4%) in UIED group, but in 50 seizures (58.1%) and 12 patients (75.0%) in BIED group. It was not different significantly. Ictal scalp EEG patterns at seizure onset or later were correctly lateralized significantly more often in 188 seizures (93.5%) and 30 patients (93.8%) in UIED group compared with 49 seizures (57%) and 10 patients (62.5%) in BIED group. Switch of lateralization and bilateral independent onset were seen more frequently in BIED group, whereas the maintenance of ictal discharges in unilateral temporal or hemisphere through the whole tracing in UIED group. Our results show that interpretation of ictal scalp EEG must be considered
more cautiously in BIED group. BIED may be correlated with bitemporal excitability.
