고빈도 현미부수체 불안정형(high microsatellite instability, MSI-H) 종양은 DNA 복제오류 교정 유전자의 결함으로 발생하며, 이로 인해 종양억제유전자, 세포고사관련 유전자 및 DNA 수선 관련 유전자와 같은 종양관련 유전자의 단백질 지령부위내에 존재하는 현미부수체에 체이동 돌연변이가 발생하여, 암진행이 일어나는 유형의 종양이다. MSI-H 종양의 체이동 돌연변이 산물을 진단 및 면역치료에 이용하고자 하는 시도가 진행되고 있으나, 보다 실제적인 응용을 위해서는 단백질 항원으로 이용될 수 있는 다양한 표적유전자를 동정하고, 돌연변이형 유전자 산물의 발현을 분석하는 연구가 필수적이라 할 수 있다. 이

를 위하여 DNA 손상에 대한 감지기 역할을 하는 거대복합체로 알려진 BASC (BRCA1-associated genome surveillance complex)의 구성 유전자중 단백질 지령부위내 단반복 염기서열(coding mononucleotid repeat, cMNR)을 포함하는 유전자의 체이동 돌연변이를 조사하여 새로운 표적유전자를 동정하였다. 또한, 사람의 유전체 데이터베이스 탐색을 통해 cMNR을 포함하는 유전자를 발굴한 다음, 이들 유전자들의 MSI-H 대장암에서의 체이동 돌연변이율을 조사하였고, 동정된 표적유전자들의 돌연변이형 전사체 및 단백질의 발현 여부와 퇴화 기전을 조사하였다. 실험결과 BASC 복합체를 형성하는 유전자 중 hRAD50 (31%), BLM (21%), hMSH6 (21%)에서 빈번한 체이동 돌연변이가 나타났으나, BRCA1, ATM, NBS1에서는 돌연변이가 거의 나타나지 않았다. UniGene 데이터베이스 탐색을 통해 10개이상의 cMNR을 포함하는 33개의 유전자를 선별한 다음, MSI-H 대장암에서의 체이동 돌연변이를 조사하였는데, 가장 높은 돌연변이율을 나타낸 새로운 표적유전자는 MARCKS (72%), FLJ11383 (74%), TAF1B (82%)였으며, 이중 MARCKS와 FLJ11383은 동형(biallelic) 돌연변이 빈도가 각각 36%, 21%로서 높았다. 돌연변이형 전사체의 발현은 TGF-b RII, BAX, TCF-4 및 MARCKS 등 에서 관찰할 수 있었다. hMSH6, hRad50 유전자의 돌연변이형 전사체는 전부 또는 일부에서 발견할 수 없었으나, 단백질 합성 억제제인 puromycin을 처리하였을 때 발현하였다. 이는 hMSH6, hRad50의 돌연변이형 mRNA가 nonsense mediated mRNA decay (NMD) 기전을 통해 퇴화됨을 나타낸다. MARCKS 유전자의 동형 돌연변이를 포함하는 SNU-C4 세포주의 경우 MARCKS 단백질을 발현하지 않았지만, 특정한 프로테아좀 억제제에 의해 돌연변이형 단백질의 분해가 억제됨을 발견하였다. 결론적으로, 본연구를 통해 MSI-H 종양의 새로운 신규 표적유전자를 동정하였으며, 발생한 돌연변이는 RNA 혹은 단백질 수준에서 제거됨을 확인하였다. 발생한 체이동 돌연변이의 유전자 산물이 RNA 혹은 단백질 수준에서 제거되는 것은 암 면역 반응을 회피하는 하나의 기전으로 사료된다

Tumors with high microsatellite instability (MSI-H) are caused by defects in DNA mismatch repair genes and progress with by accumulating frameshift mutations in coding microsatellite sequences of various cancer-related genes including tumor suppressor genes, apoptosis-related genes and DNA repair genes. The mutant products from frameshift mutations in target genes containing nucleotide repeat sequences can be used for diagnosis or immune therapy in MSI-H tumors. So, it is essential to identify various target genes and validate the expression of mutant gene products in MSI-H tumors. Initially, frameshift mutations of genes which in a large complex named BASC (BRCA1-associated genome surveillance complex), that serves as a sensor

for DNA damage were examined. Furthermore, a number of genes containing cMNR were identified by genome-wide systematic database search and the frameshift mutations in these repetitive sequences in MSI-H colorectal carcinomas are investigated.

Expression of mutant transcripts and proteins were also examined. Frequent mutations in genes forming a BACS complex were found in hRAD50 (31%), BLM (21%), hMSH6 (21%), however rare mutations in BRCA1 (1%) and ATM (4%), and no mutation in NBS1 were found. Thirty-three genes which containing cMNR with a length of 10 or

more nucleotides were identified by a systematic database search. The most frequently mutated novel genes in MSI-H colorectal carcinomas were MARCKS (72%), FLJ11383 (74%) and TAF1B (82%). Biallelic inactivation in MARCKS (26%) and FLJ11383 (21%) was also frequent in the MSI-H colorectal carcinomas. Expression of mutant transcripts were identified in TGF-bRII, BAX, TCF-4 and MARCKS genes. Mutant transcripts of hMSH3, hMSH6 and hRad50 genes were nearly undetectable. However, these transcripts were identified in the presence of the translation inhibitor puromycin, implying that mutant hMSH6 and hRad50 mRNAs are degraded, possibly by the mechanism of nonsense mediated mRNA decay. SNU-C4 cell, which contains biallelic mutant MARCKS allele, did not express the detectable amount of MARCKS protein. However, mutant MARCKS protein was protected from proteolysis by adding specific proteasome inhibitors. In conclusion, we identified several target genes in MSI-H tumors and their mutant products were eliminated in RNA or protein level.

Taken together, these data demonstrate the mechanism of evoking intense immune reaction and also explain the immune escape mechanism in a subset of MSI-H tumors.
