폐경 후 여성에서 심혈관 질환의 유무에 따른 체지방 분포와 내분비*혈관 내피세포 기능의 차이



복부의 내장지방 축적, 대사이상, 혈관 내피세포 기능 손상은 심혈관계 질환의 위험성을 잘 대변한다. 본 연구는 건강하거나 관상동맥경화증이 있는 한국 폐경 후 여성에서 체지방 분포와 내분비*혈관 내피세포 기능의 차이를 알아보고자 하였다.

혈관조형술상 관상동맥경화증으로 판정된 폐경 후 여성환자(30명)과 이들과 나이 및 체질량지수가 유사한 건강한 폐경 후 여성(80명)을 대상으로 연구하였다. 관상동맥경화증 환자는 건강한 폐경 후 여성에 비해 free androgen index(40.5%)와 렙틴 농도(21.4%)가 높았으나 IGF-1은 건강한 여성에 비해 낮았다. 지방산 농도는 건강한 여성에 비해 69.7% 낮았다(p<0.001). L1 수준에서 총 지방 면적은 환자군에서 18.4% 높았으며 (p<0.01), 피하지방 면적은 건강한 여성에 비해 14.7% 높았다(p<0.05). Flow-mediated dimension (FMD) 와 nitroglycerin-mediated dimension (NMFD)는 관상동맥경화증이 있는 폐경 후 여성에서 건강한 여성에 비해 각각 37.8%, 31.2% 낮았다(p<0.05).

이상의 결과는 건강한 폐경 후 여성에 비해 관상동맥경화증이 있는 폐경 후 여성에서 복부 지방 축적과 공복혈당과 인슐린의 증가를 보여주는 다른 연구 결과들과 일치하며, 혈관 내피세포 기능이 감소한 것으로 나타났다.

Background Accumulation of intra-abdominal visceral fat, and abnormal endocrine and endothelial vasodilator functions have been shown to be effective markers for risk of cardiovascular disease (CVD). The objective of this present study was to investigate whether there are differences in body fat distribution, and endocrine and endothelial vasodilator functions between 2 groups: postmenopausal women without coronary artery disease (CAD) and postmenopausal women with CAD.

Methods and Results Postmenopausal patients (n=30) with CAD had angiographic evidence with ≥ 50% occlusion of one or more major coronary arteries and healthy postmenopausal women (n=80) with similar age and body mass index were selected for the control group. The CAD group had higher free androgen index (40.5%) and Leptin levels (21.4%) but lower insulin-like growth factor 1 levels (21.4%) than the healthy group. The CAD postmenopausal women had higher concentrations of fasting glucose and fasting insulin levels (p<0.05). But free fatty acid of CAD women were lower than healthy postmenopausal women by 69.7% (p<0.001). The visceral fat area was 23.4% higher in CAD patients than healthy subjects at the L1 vertebra (p<0.01)

and 21.4% higher at the L4 vertebra (p<0.001). The total fat area at the L1 vertebra was 18.4% higher in CAD women (p<0.01) and the subcutaneous fat area was 14.7% higher (p<0.05) than healthy postmenopausal women. Flow-mediated dimension (FMD) of CAD postmenopausal women was 37.8% lower (p<0.05) than the healthy postmenopausal women. The CAD women had also lower nitroglycerin-mediated dimension (NMD) by 31.2% than the healthy women (p<0.05).

Conclusion These results are consistent with the concept that abdominal fat accumulation, fasting levels of glucose and insulin are increased in the CAD postmenopausal women compared to healthy postmenopausal women, and it indicated that the endothelial vasodilator function decreased in the CAD postmenopausal women.
